S&ME’s approved Scope of Services for this Task Authorization included conducting the initial project review and setup, preparing a health and safety plan, completing a site reconnaissance and receptor survey update, collecting samples from three water supply wells, and preparing this report. The scope of services was completed as proposed. Receptor Survey and Groundwater Monitoring Report S&ME Project No. 1354-12-055 TF-11828 Judy Faulkner-Smith Property April 2, 2012 2 2.0 SITE HISTORY The subject site is located at 1300 Vicksboro Road in Henderson, Vance County, North Carolina. The site is a single family residential home. The approximate central coordinates of the subject site are 36º 19’ 01.85” north latitude and 78º 21’ 32.29” west longitude. A vicinity map of the site is shown on Figure 1, and the site and pertinent features are shown on Figure 2. Based on a North Carolina Department of Environment and Natural Resources (NCDENR) Pollution Incident/Leak Reporting Form dated July 7, 1993, concentrations of several volatile organic compounds (VOCs) were detected in the water supply well at the Faulkner-Smith residence. One of the VOCs (benzene) was detected in a concentration exceeding the North Carolina 2L Groundwater Standard (2L Standard). The source of the contamination was believed to be an old gasoline underground storage tank (UST) located on an adjacent property. The UST was owned by Mr. William Faulkner (deceased), and was reportedly taken out of service in 1958, but was never properly abandoned. According to a January 27, 2004 Receptor Survey and Site Reconnaissance Survey Report by Groundwater Management Associates, Inc. (GMA), the water supply well at the Faulkner-Smith residence was sampled again in November 1993. The laboratory analytical results of the sample collected in November 1993 indicated that no concentrations of VOCs were detected above the laboratory detection limits. The results of the receptor survey indicated that there were at least three water supply wells located within 500 feet and public water service was not available in the area. Twenty-three water supply wells and two surface water features were identified within 1,500 feet of the site. On July 12, 2004, GMA submitted a Water Supply Well Monitoring Report. The report indicated that three water supply wells located within 500 feet of the site (Faulkner-Smith 3-1, Faulkner-Smith 3-6, and Roberts B-29) were sampled on June 3, 2004. The laboratory analytical results of the samples indicated that benzene was detected in a concentration exceeding the 2L Standard in the Faulkner-Smith 3-1 supply well (site well). Several other VOCs were also detected in the Faulkner-Smith 3-1 and one VOC well below their respective 2L Standards. One VOC (methyl tert-butyl ether (MTBE)) was detected below the 2L Standard in the sample collected from Faulkner-Smith 3-6. According to a February 3, 2006 Water Supply Well Sampling Report by GMA, the three water supply wells were sampled in November 14, 2005. The laboratory analytical results of the water samples indicated that benzene was present exceeding the 2L Standard and several other VOCs below the 2L Standard in the Faulkner-Smith 3-1 water supply well. No target contaminant constituents were detected in the other two water supply wells sampled. Receptor Survey and Groundwater Monitoring Report S&ME Project No. 1354-12-055 TF-11828 Judy Faulkner-Smith Property April 2, 2012 3 3.0 RECEPTOR INFORMATION S&ME, Inc., (S&ME) performed an updated receptor survey. Information pertaining to the receptors identified within 1,000 feet of the subject site is summarized below. The vicinity and site are shown on Figure 1 and Figure 2. The locations of adjacent property owners and potential receptors located within approximately 1,000 feet of the subject site are shown on Figure 3. 3.1 Site Reconnaissance S&ME personnel completed the site reconnaissance on March 8, 2012. The site is located at 1300 Vicksboro Road in Henderson, Vance County, North Carolina and currently consists of the Faulkner-Smith residence. The adjacent property to the east is the location of a former store and gasoline station. The USTs presumably have not been located or removed and are the likely source of the contamination in the Faulkner-Smith residence water supply well. During the site reconnaissance, S&ME personnel observed what appeared to be fill ports and vent pipes along the east side of the former store structure. The ground surface is raised in the area in which the pipes were found, and are possibly the location of the USTs. The surrounding property is primarily residential areas and farmland. Photographs of the site and surrounding area are included in Appendix I. 3.2 Water Supply Wells On March 8, 2012, S&ME performed a visual reconnaissance survey for public and private water supply wells within 1,000 feet of the subject site. S&ME identified 19 possible private water supply wells within 1,000 feet of the site (Figure 3). The adjacent property owner and water supply well information is included in Table 1. 3.3 Public Water Supplies Public water is currently not available in the vicinity. 3.4 Surface Water According to the 2010 Vicksboro USGS Quadrangle Map, an unnamed pond is located approximately 1,500 feet to the southwest of the site. No other surface water bodies were observed within 1,500 feet of the site during S&ME’s area reconnaissance. Receptor Survey and Groundwater Monitoring Report S&ME Project No. 1354-12-055 TF-11828 Judy Faulkner-Smith Property April 2, 2012 4 4.0 WATER SUPPLY WELL SAMPLING Site activities were performed in general accordance with a site-specific health and safety plan (HASP) prepared by S&ME. The assessment was conducted in general accordance with the procedures outlined in the NCDENR UST Section, Guidelines for the Assessment and Corrective Action for UST Releases, July 2008 (Guidelines). 4.1 Water Supply Well Sampling On March 8, 2012, S&ME sampled three water supply wells at and adjacent to the Faulkner-Smith residence (3-6 Faulkner-Smith (site water supply well), 3-1 Faulkner-Smith, and B-29 Roberts). Water supply wells were sampled at the outside spigot closest to the water supply well. The spigot was opened and allowed to run for a minimum of 10 minutes before sampling, after which S&ME reduced the flow and the water was placed directly into laboratory-supplied containers. The samples were stored in a cooler with ice, and shipped under chain-of-custody procedures to Pace Analytical Laboratories (Pace) of Huntersville, North Carolina, a North Carolina certified laboratory for analysis. The groundwater sample was analyzed for VOCs by EPA Method 6200B. 4.2 Monitor Well Sampling Results The laboratory analytical results of the water supply well samples collected on March 8, 2012 reported one VOC, MTBE, above the laboratory reporting limit but below the 2L Standard in water supply well 3-6. No other constituents were detected at or above the laboratory method detection limits in any of the wells sampled. A summary of groundwater analytical results and a comparison to NCDENR’s regulatory standards is included on Table 2. The groundwater constituent concentrations are presented on Figure 4. A copy of the laboratory analytical results and chain-of-custody form is included in Appendix II. Receptor Survey and Groundwater Monitoring Report S&ME Project No. 1354-12-055 TF-11828 Judy Faulkner-Smith Property April 2, 2012 5 5.0 CONCLUSIONS AND RECOMMENDATIONS A groundwater monitoring event and receptor survey update was conducted at the Faulkner-Smith site located at 1300 Vicksboro Road, Henderson, North Carolina to assess potential petroleum impact to water supply wells from a former UST system.  On March 8, 2012, S&ME performed a receptor survey update to identify public and private water supply wells, and other receptors within 1,000 feet of the subject site. S&ME identified 19 private water supply wells within 1,000 feet of the site. No surface water bodies were identified within 1,000 feet of the site. According to the City of Henderson, municipal drinking water is not available to the site and properties within 1,000 feet of the site.  On March 8, 2012, S&ME personnel collected groundwater samples from three water supply wells. One constituent, MTBE, was reported in water supply well 3-6 above the laboratory method detection limit, but below the 2L Standard.  During the site reconnaissance, S&ME observed possible UST fill ports and vent pipes adjacent to the former store. S&ME Recommends:  Conducting a geophysical survey to verify the locations of the USTs observed along the eastern side of the former store;  Upon confirmation of the tank locations, S&ME recommends the removal of the USTs and associated soil sampling; and,  Until the source area is assessed and remediated, if needed, periodic sampling of adjacent water supply wells is also recommended.
