Dear Ms. Blalock:
ATC Associates of North Carolina, P.C. (ATC) is pleased to submit the results of a water supply well
sampling event conducted on May 12, 2015, at the Judy Faulkner-Smith site located at 1300
Vicksboro Road in Henderson, North Carolina. A site topographic map is included as Figure 1.
Currently on-site is a single-story single-family residential dwelling. Adjacent to the property is the
former Faulkner Store that operated as a convenience store and retail petroleum facility. The
underground storage tank (UST) system were taken out of service in 1958 and removed from the site in
July 2012. The former Faulkner Store is an abandoned building. A site map is included as Figure 2. A
current site photograph is included in Appendix A. The sampling event was performed based on a
April 13, 2015, Task Authorization (TA-1) by the Trust Fund Branch of the UST Section, Division of
Waste Management, North Carolina Department of Environment and Natural Resources (NCDENR).
The work is being performed as part of the State Lead Contract #N14004i between NCDENR and
ATC.
As authorized in Task Authorization TA-1, ATC performed an initial file review, obtained historical
aerial photographs in an attempt to determine the possible source of contamination, and sampled three
water supply wells (3-6, 3-1, B-29). ATC obtained historical aerial photographs of the property and
vicinity from Environmental Data Resources, Inc. (EDR). Aerial photographs reviewed range from
1970 to 2012. Based on the review of the aerial photographs, no structures indicating other potential
sources of groundwater impact to the site vicinity other than the Faulkner Store were observed. A copy
of the aerial photographs is provided in Appendix B.
On May 12, 2015, ATC mobilized to the site to conduct water supply well sampling. ATC collected
water supply well samples from water supply wells 3-6, 3-1, and B-29. Field notes from the May 12,
2015, sampling event are included in Appendix C. Water supply well information is included in
Table 1 and water supply well locations are shown on Figure 2.
The water supply well samples were collected in laboratory-supplied, preserved containers; entered
into chain of custody control; placed on ice; and shipped to North Carolina-accredited Con-Test
