(ATC) is pleased to submit the results of a groundwater and water supply well sampling event conducted on December 2, 2015, for the Judy Faulkner-Smith site located at 1300 Vicksboro Road in Henderson, North Carolina. Adjacent to the Judy Faulkner-Smith property is the former Faulkner Store (source property) that operated as a convenience store and retail petroleum facility. The underground storage tank (UST) system at the former Faulkner Store was taken out of service in 1958 and removed from the site in July 2012. The property is currently developed with an abandoned building. A site topographic map is included as Figure 1. A site map is included as Figure 2. The sampling event was performed based on an October 6, 2015, Task Authorization (TA- 2) by the Trust Fund Branch of the UST Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of the State Lead Contract #N14004i between NCDEQ and ATC. On December 2, 2015, ATC personnel mobilized to the site to supervise the installation of temporary well TW-1 advanced in the former UST basin of the former Faulkner Store. Regional Probing, Inc., a North Carolina-licensed drilling contractor, advanced boring TW-1 using hand auger methods from the ground surface to 5 feet below ground surface (bgs) and direct-push Geoprobe rods from 5 feet bgs to 20 feet bgs. ATC utilized a peristaltic pump to collect a sample from boring TW-1. The sample was collected in laboratory-supplied, preserved containers; entered into chain of custody control; placed on ice; and shipped to Con-Test Analytical Laboratory (Con-Test) in East Longmeadow, Massachusetts. The samples were analyzed by Environmental Protection Agency (EPA) Method 6200B for volatile organic compounds (VOCs), volatile petroleum hydrocarbons (VPH) by the Massachusetts Department of Environmental Protection (MADEP) method, and lead by EPA Method 6020A. Following collection of the sample the boring was abandoned. The location of TW-1 is shown on Figure 2. Field notes from the event are included as Appendix A. On December 2, 2015, ATC also conducted a water supply well sampling event. ATC collected samples from water supply wells 3-6 and 3-1.
