PURPOSE OF SITE VISIT: Comprehensive Inspection – Follow Up STATUS OF PAST NOTED VIOLATIONS: NA OBSERVED VIOLATIONS: NA
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. John Nguyen, Permitting Engineer assigned to Hoke County, and David Powell with the Section visited Hoke
County facilities for a follow up inspection. Mrs. Locklear assisted them during the inspection. Mrs. Locklear also informed that, as requested, a separate file with Closed MSW documents had ben created and updated for inspection next time. Please keep any relevant documents for the Closed MSW in this file. Good job. 2. The landfill was mowed and looked good as viewed from east and north sides. Shrubs and trees didn’t seem to be an issue. As requested, additional EOW markers were reinstalled marking the limits of disposal on the east side. Please be sure and maintain these markers around its perimeter. Also, be sure and manage in sinking areas or erosion as weather and conditions allow.

3. Mrs. Locklear informed that MW-1 has not had lock, data plates installed, and area cleared yet, but it is scheduled for very soon. The contractor used for maintaining the wells onsite is scheduled to come in next few weeks and should install a lock, data plate with identification and should clear vegetation around the well. A couple photos showing lock, identification and vegetation cleared from well should be sent to David Powell via email ASAP.
All monitoring wells at Hoke County site should be locked, have identification plates with correct identification name in accordance with 15A NCAC 2C. Please fix all wells lacking these items. Any proposed modification to an approved plan must be submitted to the Section Hydrogeologist for a review and approved prior to implementation. Please be sure any and all changes to wells, such as new installs or abandonment, onsite have been submitted to DEQ William Sugg at (919) 707 - 8258 or perry.sugg@deq.nc.gov.

4. Mrs. Locklear indicted she had not spoken with Section Hydro. Mr. Ben Kelly. Ms. Locklear needs to contact Ben Kelly, benjamin.kelly@deq.nc.gov or 919-707-8327, in regard to sampling data for Hoke Closed MSW and submitting via Solid Waste Section's new Equis EDD format. The county should begin doing so immediately AND contact Ben Kelly for guidance on how to meet this requirement. The county should also be sure their consultant/engineer firm is aware of this new contact and requirement as well.
Previous inspection photo of sampling report for 8/2023. 5. Frequent inspection of cap and edge of waste should be completed to inspect for missing EOW markers, shrub growth, erosion, settling and potential leachate seeps. Reminder: Any areas discovered should be repaired as the weather allows site staff to do so without further degrading. Any repairs to the site should
be documented in your records and made available to Section staff for review.

6. Corrective measures are necessary as a result of this inspection. Corrective action should be completed within 30 days of receipt of this inspection report.
