STATUS OF PERMIT:
In 1973. Landfill operations began in Phase 1 on this property. Facility Permit 51-03 is a combination of facility permits
51-01 and 51-02.
Permit to Construct Johnston County MSW Landfill, Phase 6 issued May 8, 2020.
Permit to Operate 5103-MSWLF Phase 4A and 5103-CDLF Area 2 Cells 1 and 2 issued May 8, 2020.
Permit for Closure for MSW Phases 1 through 4 and C&D Area 1 issued May 8, 2020.
Life of Site Permit for 5103-MSWLF and 5103-CDLF issued May 8, 2020 and will expire August 8, 2057.
Permit to Construct Johnston County MSW Landfill Phase 6 issued November 30, 2021.
Permit to Operate 5103-MSWLF Phase 4A, 5, and 6A and C&D Area 2, Cells 1 and 2 issued November 30, 2021.
Permit for Closure MSW Phases 1 through 4 and C&D Area 1 issued November 30, 2021.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
ADDITIONAL COMMENTS
Facility Inspection
1. The facility is fenced and is secured by a locking gate when it is not open for operation.
2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
3. The facility accepts waste Monday through Saturday from 8:00 am until 4:45 pm and closed on Sunday.
4. The permit, operations plan, waste screening, and training records were not reviewed during this inspection.
5. The perimeter roadways around the active MSWLF and closed CDLF are being properly maintained.
6. Permanent edge of waste (EOW) markers were observed around the active portion of the MSWLF and closed
CDLF.
Yard Waste
7. The yard waste was windrowed and being prepped for grinding. Mr. Beasley stated that the yard waste would be
ground in the next few months.
8. No areas of standing water were observed in the western portion of the yard waste area. Continue to monitor and
repair the yard waste area as needed.
9. A painted pallet was observed within the white wood stockpile. Mr. Beasley removed the painted pallet for proper
disposal during this inspection.
Phase 4A MSWLF
10. The southern side slope of Phase 4A is the last remaining side slope to be covered and closed. The other side slopes
have been seeded and some vegetation has begun to grow. Mr. Beasley stated that the side slopes would be
monitored, repaired, and revegetated as needed.
11. Phases 1 through 4A have received authorization for closure. Monthly meetings have been scheduled to provide
status updates during the process.
12. One well was observed to be unlocked during this inspection. In a follow-up conversation with Mr. Beasley, the
well was determined to be an abandoned well, however, a new lock was placed on the well.
13. The stormwater basins located around Phase 4A have been mowed and the woody vegetation removed. Continue
to ensure that the stormwater basins are mown and maintained in working order.
Phase 5 MSWLF
14. The slopes of Phase 5 have been covered. Grasses have begun to grow on the eastern side slope. Once the final
grade has been achieved on the western side slope, the area will be seeded.
15. Additional cover was being applied to the top of Phase 5 to meet slope requirements. The top will be seeded after
grading has been completed.
16. Windblown waste was actively being picked up during this inspection. Continue to ensure that the windblown waste
is picked up daily.

Phase 6 MSWLF
17. The working face of Phase 6 was small and compact. The working face was actively being covered during this
inspection.
Phase 4 Closure Meeting
16. The closure meeting provided a status update for the closure of Phases 1 through 4. The closure status is on schedule
for the project.
