None.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The facility is permitted to receive, consolidate, and transfer yard trash. The site receives and manages leaves
collected curbside by the city of Winston-Salem Sanitation Division. The site receives and stores the leaves
through the months of November through March.
2. The site is secured by a perimeter chain -link fence with site access controlled by a locking gate.
3. The entire site is paved with asphalt with four small concrete pads surrounding the operations area. At the time
of the inspection the asphalt was in good condition.
4. A sign is posted on the site's entrance gate. Ensure that the sign contains information regarding the types of
acceptable wastes, permit number, and emergency phone number.
5. Mr. King stated that the facility applied gravel to the damaged areas in the operations area before any waste was
brought on -site.
6. No leaf grinding or composting takes place on site.
7. The city contracts with Wallace Farms, Inc. to manage the operations on site.
8. The site is only open to the City of Winston-Salem trucks to deliver waste to the site.
9. City trucks unload leaves at the site where they are then pushed into piles. Wallace Farms personnel then
manage the leaf stockpiles during their temporary storage on -site.
10. Wallace Farms transports the leaves to Wallace Farms compost facility in Davie County (permit #3004) or to
the Forum 52 large Type 1 Compost Facility (permit #34-23) in Rural Hall.
11. The facility has painted a dashed white line used to mark the required buffer for the site. At the time of the
inspection the facility the buffer requirements.
12. The site is slopped so that surface water is directed away from the operation areas. No ponding water was
observed on site.
