1.0 PROJECT INFORMATION
1.1 Site Information
The Foothills Regional Municipal Solid Waste Landfill (MSWLF) is owned by Caldwell County and operated
by Republic Services ofNorth Carolina, LLC (Republic). The site is located at 2800 Cheraw Road in Caldwell
County, 2 miles northwest of Lenoir, North Carolina (Figure 1). The permitted facility boundary consists of
about 277 acres. The landfill was permitted for operation in August 1998.
Republic is currently expanding the existing MSWLF footprint by constructing the permitted waste unit Cell
No. 6B (Figure 2). One additional groundwater monitoring well (MW-12) was permitted to be installed as
part of the development plan. This report documents the well installation activities.
1.2 Geologic Setting
The subject site is located within the Inner Piedmont Belt of the Piedmont Physiographic Province. The
crystalline rocks of the Inner Piedmont Belt occur in generally northeast -southwest trending geologic belts
in the Carolinas and consist of a stack of highly metamorphosed thrust sheets bound on the northwest by
the Brevard Shear Zone and to the southeast by the Kings Mountain Shear Zone. The Inner Piedmont
includes high-grade metamorphosed sedimentary and igneous rocks that have been exposed to multiple
deformations (Horton and Zullo, 1991). Rock types that resulted from the multiple metamorphisms include
gneiss, schist and amphibolite with northeast/southwest trending foliation with varying degrees of dip.
The typical residual soil profile consists of clayey soils near the surface, where soil weathering is more
advanced, underlain by micaceous sandy silts and silty sands. Residual soil zones develop by the in situ
chemical weathering of bedrock and are commonly referred to as "saprolite." The boundary between soil
and rock is not sharply defined.
A transitional zone of partially weathered rock (PWR) is normally found overlying the parent bedrock.
Partially weathered rock is defined, for engineering purposes, as residual material with standard penetration
resistance in excess of 100 blows per foot (bpi). Fractures, joints, and the presence of less resistant rock
types facilitate weathering. Consequently, the profile of the partially weathered rock and hard rock is quite
irregular and erratic, even over short horizontal distances. Also, it is not unusual to find lenses and boulders
of hard rock and zones of partially weathered rock within the soil zone, well above the general bedrock
level.

2.0 FIELD EXPLORATION AND ACTIVITIES
2.1 General
Field activities were performed between November 2 and November 3, 2023. A North Carolina licensed
driller from Geologic Exploration, Inc. of Statesville, North Carolina performed the well installation activities
which were documented by a geologist from BLE. Survey for horizontal and vertical control of the completed
well was performed by WithersRavenel of Cary, North Carolina on November 8, 2023. The survey data for
the new well is included in Appendix A and is summarized on Table 1. A well installation form (NC Form
GW-1) was prepared for the groundwater monitoring well and is included in Appendix B.
2.2 Drilling Procedures and Subsurface Conditions Encountered
The boring was conducted using an ATV -mounted drilling rig employing hollow stem auger and air -
hammer drilling techniques. Drilling procedures are described in Appendix C. The soil and rock
descriptions are indicated on the boring log (Appendix D) and were based on visual examination by a North
Carolina registered geologist. An information sheet, which describes the Unified Soil Classification System
(USCS) terms and symbols used on the test boring log, is also included in Appendix D.
The subsurface conditions encountered at the MW-12 location consisted of rocky fill and residual soil
underlain by gneiss bedrock. This location encountered groundwater below the top of bedrock, therefore,
the screened interval of the monitoring well was set to encounter the uppermost water -bearing fractures.
2.3 Groundwater Monitoring Well Installation and Construction
A new groundwater monitoring well, MW-12, was installed at the facility in general accordance with the
North Carolina Well Construction Standards Rule 15A NCAC 2C, .0108 (Standards for Construction: Wells
other than Water Supply) and the facility's approved Environmental Monitoring Plan (EMP). The
groundwater monitoring well location is shown on Figure 2 and a well construction diagram is included in
Appendix D. It should be noted that NM-12 was installed approximately 200 feet north of its permitted
location due to an oversight by the grading contractor. We contacted Ms. Jaclynne Drummond of the North
Carolina Solid Waste Section (SWS), Division of Waste Management (DWM) on November 2, 2023 while
drilling was in progress to obtain a regulatory opinion on the relocation. Ms. Drummond provided verbal
approval with the understanding that the as -built location is within typical well spacing at the facility and is
hydraulically downgradient of the waste unit to be monitored.
Groundwater monitoring well MW-12 was constructed with 2-inch diameter polyvinyl chloride (PVC;
Johnson Schedule 40, NSF -rated) casing with flush -threaded joints inserted in a 6-inch nominal diameter
borehole. The bottom 15-foot section of the well is a manufactured well screen with 0.010-inch-wide
machined slots with a 0.25-foot-long sediment trap threaded onto the bottom of the screen section.
Silica filter sand (DSI, #0) was placed around the outside of the pipe in the borehole annulus to a level
above the top of the well screen. A 2-foot minimum thick hydrated bentonite seal (Baroid brand 3/8-inch
diameter chips) was installed on top of the filter sand to seal the monitoring well at the desired level. The
remaining well annulus was grouted with a 5% bentonite-cement mixture from the top of the bentonite seal
to within one -foot of the ground surface.

The surface completion of each well consisted of a PVC cap and a lockable 4-inch x 4-inch x 5-foot standup
protective steel cover, with a 3-foot by 3-foot concrete pad at the base of the steel cover. A well
identification placard was affixed to the protective steel cover. The well was constructed with a vent hole
in the PVC casing near the top of the well and a weep hole near the base of the outer protective steel cover.
2.3.1 Groundwater Monitoring Well Development
Monitoring well MW-12 was purged and developed to reduce the groundwater turbidity to the lowest practical
level utilizing conventional development techniques (surging and pumping) for approximately three (3) hours.
The well had a moderate yield and approximately 70.0 gallons of groundwater was purged from the well.
Turbidity was measured during the well development using a Hach 2100Q Portable Turbidimeter. The well
had very high initial turbidity but was developed until further development did not substantially reduce
turbidity levels. The final turbidity value achieved was 2.79 NTUs.

One (1) new groundwater monitoring well was installed (MW-12) in general accordance with the facility's
approved EMP and the applicable North Carolina well construction standards. BLE personnel developed the
new monitoring well upon installation.
4.0 QUALIFICATIONS OF REPORT
The activities and evaluative approaches used in this scope of work are consistent with those normally
employed for services of this type. Our services have been performed based on our understanding of the site
and the observations made during our work.
This report has been prepared on behalf of and exclusively for the use of Republic. This report and the findings
contained herein shall not, in whole or in part, be used or relied upon by any other parry (excluding the North
Carolina Division of Waste Management - Solid Waste Section) without BLE's prior written consent.

Drilling Procedures
Hollow -Stem Auger Drillin
Soil borings were advanced by mechanically twisting a continuous flight of 8.25-inch OD steel hollow -
stem augers into the soil.
Air -Hammer Drillin
Where consolidated subsurface material was encountered that was impenetrable to hollow -stem augers, a
6-inch OD down -hole pneumatic air -hammer was used to advance the borehole. The pneumatic drill
hammer advanced through the subsurface materials by rapidly striking the rock while the drill pipe was
slowly rotated; compressed air was used to remove the soil and rock cuttings from the borehole. The drill
hammer was constructed of alloy steel with tungsten -carbide inserts that provide the chipping or cutting
surfaces. An in -line air filter was attached to the air compressor on the rig to remove oil from the air and
to prevent oil contamination in the borehole.
General
A North Carolina registered driller performed the drilling and well installation activities. A North Carolina
registered geologist examined the soil and rock samples and prepared boring logs.
