The USTs and their appurtenances are owned by Circle K Stores, Inc. of Raleigh, North Carolina. Figure 2 is a site map which depicts the site and pertinent structures, including the location of the USTs and appurtenances. 2.3 AST Information There are no ASTs associated with the referenced facility. 2.4 Site Characteristics The subject property consists of one convenience store building. The UST’s and their appurtenances are currently active at the site. The subject property is fairly level. The subject property is located at the intersection of Pinecroft Road and W. Vandalia Road in Greensboro, Guilford County, North Carolina. 2.5 Release Information On January 24, 2020, during annual testing, the premium gasoline spill bucket for tank #2 was found to not pass its hydrostatic test. The spill bucket has subsequently been repaired and a UST-23A is provided in Appendix A. Phase I Limited Site Assessment Report September 8, 2020 Circle K Store #2704132 3 On April 13, 2020, ATC collected a soil sample adjacent to the premium spill bucket. On April 22, 2020, upon receipt of the soil analytical results, ATC submitted a UST-61: 24-Hour Release and UST Leak Reporting Form to the Guilford County Department of Health and Human Services office via e-mail correspondence. A Site Check Report was submitted to NCDEQ on April 29, 2020. In response to the Site Check Report, The Guilford County Department of Health and Human Services requested the submittal of a Phase I Limited Site Assessment (LSA). It should be noted a previous release is noted to have occurred on the property at an unknown date and it was issued UST incident #30338. This incident was closed on July 29, 2013. The file has been archived by Guildford County and was not able to be reviewed at the time of this report. 3.0 LIMITED SITE ASSESSMENT RISK CLASSIFICATION AND LAND USE In order to evaluate the following, a review of available site-specific, local and regional data was performed to include:  A well survey within a 1,500-feet radius of the subject facility, including a record inquiry.  Identification of wellhead protection areas.  Researching geology and hydrogeology of the region, including topography, soil types, and surface water drainage patterns.  Identification of surface waters within a 1,500-feet radius of the subject facility.  Land use observations (site reconnaissance) and zoning classifications.  A visual survey for subsurface structures and potential impacts from the source area.  Gathering information on adjacent property owners and occupants. Part I – Groundwater/Surface Water/Vapor Impacts High Risk: 1. Has the discharge or release contaminated any water supply well including any used for non-drinking purposes? NO 2. Is a water supply well used for drinking water located within 1,000 feet of the source area of the discharge or release? NO Water meters were noted on properties within 1,000 feet of the source area. Is a water supply well used for any purpose (e.g. irrigation, washing cars, industrial cooling water, filling swimming pools) located within 250 feet of the source area of the release or discharge? NO 4. Does groundwater within 500 feet of the source area of the discharge or release have the potential for future use in that there is no other source of water supply other than the groundwater? NO 5. Do vapors from the discharge or release pose a threat of explosion because of accumulation of the vapors in a confined space or pose any other serious threat to public health, public safety, or the environment? NO 6. Are there any other factors that would cause the discharge or release to pose an imminent danger to public health, public safety, or the environment? NO At the time of this report, factors have not been identified that would appear to pose an imminent danger to public health, public safety, or the environment. Intermediate Risk: 7. Is a surface water body located within 500 feet of the source area of the discharge or release? NO An unnamed surface water feature is located approximately 1,200 feet southeast of the source area and another unnamed surface water feature is located approximately 1,085 feet north of the source area. If yes, does the maximum groundwater contaminant concentration exceed the surface water quality standards and criteria found in 15A NCAC 2B .0200 by a factor of 10? NA 8. Is the source area of the discharge or release located within a designated wellhead protection area as defined in 42 USC 300h-7(e)? NO 9. Is the discharge or release located in the Coastal Plain Physiographic Region as designated on a map entitled “Geology of North Carolina” published by the Department in 1985? YES The property is an active convenience store and retail fuel outlet. 4. Do children visit the property? YES Children visit the property consistent with its use. 5. Is access to the property reliably restricted consistent with its use (e.g., by fences, security personnel or both)? NO The site is not restricted. 6. Do pavement, building, or other structures cap the contaminated soil? YES If yes, what mechanisms are in place or can be put into place to ensure that the contaminated soil will remain capped in the foreseeable future? The UST basin and dispenser island areas are currently capped with concrete. The remaining areas of the parking lot are capped with asphalt. 7. What is the zoning status of the property? The site is zoned GB (General Business) by Guilford County. 8. Is the use of the property likely to change in the next 20 years? NO The site is an active convenience store with no known plans for redevelopment. Property Surrounding Source Area of Discharge or Release: The questions below pertain to the area within 1,500 feet of the source area of the discharge or release (excludes property containing source area of the release): 9. What is the distance from the source area of the release to the nearest primary or secondary residence (permanent or temporary)? The nearest residence was identified approximately 300 feet north of the source area. 10. What is the distance from the source area of the release to the nearest school, daycare center, hospital, playground, park, recreation area, church, nursing home, or other place of public assembly? The closest place of public assembly is New Life Ministries Church located approximately 300 feet east of the source area. Pinecroft Baptist Church is located approximately 720 feet west of the source area and a daycare, Kid’s Academy Child Care Center, is located approximately 830 feet south of the source area. 11. What is the zoning status of properties in the surrounding area? The surrounding properties are zoned GB (General Business). Properties to the south, east, and west are predominantly commercial (small businesses and churches). Properties to the north are either vacant or residential. 4.0 RECEPTOR PATHWAY INFORMATION As defined by NCDEQ, a receptor is “any human, plant, or animal that is or has the potential to be adversely affected by the release or migration of contaminants”. With respect to the subject facility, the following sections present information regarding potential receptor pathways for the migration of contaminants and pathway relationships to the facility. In an effort to efficiently gather complete and accurate data, ATC personnel performed a receptor survey of the area. Activities included a visual reconnaissance survey within 1,500 feet radial distance of the site, obtaining utility information from the City of Greensboro Water Resources Department, and obtaining property use information from the Guilford County Tax Department. The results of the survey, regarding the potential identification of water wells located within 1,500 feet of the site, are summarized below. 4.1 Water Supply Wells ATC conducted a water supply well survey in the surrounding vicinity of the site. As part of the survey, ATC performed the following:  ATC conducted a visual reconnaissance on August 5, 2020. The reconnaissance was performed by driving and/or walking throughout the surrounding area and visually assessing the individual parcels of land for evidence of water supply wells, well houses, and/or water meters within a radius of 1,500 feet from the source area.  ATC contacted the City of Greensboro Water Resources Department to determine which properties within 1,500 feet of the site were connected to the public water system (i.e., receive a water bill). Based on the above reconnaissance, it was determined that the source property and properties within 1,500 feet of the source area are connected to the public water system. ATC identified one inactive water supply well located approximately 1,300 feet south of the source area. According to the property owner, the property is connected to the public water system and the well has been inactive and capped since 1977. Water supply well information is provided on Table 1 and the approximate location of the inactive water supply well is provided on Figure 3. Phase I Limited Site Assessment Report September 8, 2020 Circle K Store #2704132 7 4.2 Public Water Supplies Public water supply to the area is provided by the City of Greensboro Water Resources Department. 4.3 Surface Water Bodies As shown on Figure 1, an unnamed surface water feature is located approximately 1,200 feet southeast of the source area and another unnamed surface water feature is located approximately 1,085 feet north of the source area. 4.4 Wellhead Protection Areas At the time of this investigation, the source area is not located within an approved wellhead protection area as defined in 42 USC 300h-7(e). 4.5 Regional Geology and Hydrogeology According to the Geologic Map of North Carolina (1985) produced by Philip Brown, et al, the property is located in the Piedmont Physiographic Province and is underlain by the Carolina Slate Belt. The regional geology in the vicinity of the site is metamorphosed granitic rock which is characterized as megacrystic and well foliated. The occurrence and movement of groundwater in the Piedmont is within two separate but interconnected water-bearing zones. A shallow water-bearing zone occurs within the surficial soils and a deeper zone within the underlying bedrock. Groundwater in the shallow soil zone occurs in the interstitial pore spaces between the grains comprising the surficial soils. Groundwater in this zone is typically under water table or unconfined conditions. Groundwater movement is generally lateral from recharge areas to small streams that serve as localized discharge points. The occurrence and movement of groundwater in the underlying water-bearing zones within the crystalline bedrock is controlled by secondary joints, fractures, and faults within the bedrock. On a regional scale, the direction of groundwater flow is typically from uplands to major streams and groundwater sinks. Phase I Limited Site Assessment Report September 8, 2020 Circle K Store #2704132 8 4.6 Subsurface Structures Subsurface structures identified at the subject facility include sanitary sewer, municipal water lines, and storm water lines. A water meter was observed on-site. Other utilities may be present. Basements were not observed on adjacent properties. 4.7 Land Use Immediate surrounding properties are utilized for commercial or residential purposes. Adjacent property use information is summarized in Table 2. 5.0 LIMITED SITE ASSESSMENT ACTIVITIES The limited site assessment activities performed to collect data for this report included researching site-specific, local and regional data, performing soil and groundwater sampling, and submitting soil and groundwater samples for analytical testing. Information was also obtained from prior reports, prepared by ATC. The following sections discuss specific methodologies used in conducting these limited site assessment activities. 5.1 Monitoring Well Installation Monitoring well MW-1 was installed by personnel with Geologic Exploration (GEX), under supervision of personnel from ATC, on August 4, 2020. The location of the monitoring well is depicted on Figure 2. Monitoring well MW-1 was installed to a depth of approximately 20 feet below ground surface (bgs). The well was installed using hollow-stem augers and constructed of two-inch diameter well casing made of flush-joint, threaded schedule-40 poly vinyl chloride (PVC) pipe. The PVC well screen is 0.010-inch slot with a screened interval of 10 feet. A coarse sand pack (filter pack) was used to backfill the well annulus to a depth of approximately 2 feet above the well screen. A two-foot thick bentonite seal was placed above the filter pack, and portland cement was used to fill the remainder of the well annulus to ground surface. The well was installed as flush-mounted and provided with a sealing and locking cap. A protective, steel, traffic-bearing cover was placed over the well upon completion. Phase I Limited Site Assessment Report September 8, 2020 Circle K Store #2704132 9 5.2 Soil Sampling On August 4, 2020, ATC personnel collected two soil samples from the borehole of monitoring well MW-1 at approximate depths of nine to ten feet bgs and 13 to 14 feet bgs. The soil samples (labeled SMW-1A and SMW-1B) were placed into laboratory-approved containers, packed in ice, and delivered under chain-of-custody to SGS for analyses. The soil sample was analyzed by Environmental Protection Agency (EPA) Method 8260 and Massachusetts Department of Environmental Protection (MADEP) Method for volatile petroleum hydrocarbons (VPH). 5.3 Groundwater Sampling On August 5, 2020, a groundwater sample was collected from monitoring well MW-1. The water level was recorded from monitoring well MW-1 (12.15 feet) utilizing an oil/water interface probe. Prior to sample collection, the well volume was calculated, and a minimum of three well volumes was purged from the well column to ensure replacement of stagnant water with representative formation water. The groundwater sample was collected into laboratory-approved containers, packed in ice and delivered under chain-of-custody to SGS for analyses. The groundwater sample was analyzed for volatile organic compounds (VOCs) via EPA Method 6200B to include methyl tert butyl ether (MTBE), diisopropyl ether (IPE), ethanol, and oxygenates, EPA Method 504.1 for ethylene dibromide (EDB), EPA Method 6020 for lead, and MADEP Method for VPH. 6.0 SITE GEOLOGY AND HYDROGEOLOGY 6.1 Description of Shallow Subsurface Geology According to the United States Department of Agriculture Natural Resources Conservation Services Web Soil Survey, the site lies within the Mecklenburg-Urban Land Complex. This complex is typically found on hillslopes on ridges. The parent material is saprolite derived from diorite and/or gabbro and/or diabase and/or gneiss. The soil is typically loam for the first 8 inches, clay from 8 to 25 inches, clay loam from 25 to 36 inches, and a sandy clay loam from 36 to 80 inches. The slope is 2 to 10 percent and the soil is well drained and runoff is medium. Depth to the water table is more than 80 inches and the available water capacity is high. Soil encountered during site activities generally consisted of a reddish-brown clayey silt from 0.5 feet to 20 feet bgs. Phase I Limited Site Assessment Report September 8, 2020 Circle K Store #2704132 10 6.2 Groundwater Occurrence On August 5, 2020, the water level was measured in monitoring well MW-1 at a depth of 12.15 feet bgs. Table 3 provides a summary of well construction and depth-to-groundwater measurement. 6.3 Groundwater Flow Direction Based on limited groundwater elevation data from only one monitoring well, a groundwater contour map could not be generated at this time. Based on a review of the topographic map (Figure 1), it appears groundwater may flow north toward the unnamed surface water feature located approximately 1,085 feet north of the source area. 7.0 ANALYTICAL RESULTS 7.1 Soil Analytical Results Laboratory analytical results of the soil samples collected from the borehole of monitoring well MW-1 detected benzene and dibromochloromethane concentrations above established Soil-to-Groundwater (STGW) Maximum Soil Contaminant Concentrations (MSCCs) but below established Residential MSCCs. A summary of the soil analytical data is presented in Table 4. The location of the soil sample is provided on Figure 2. A copy of the laboratory report and chain-of-custody documents are included in Appendix C. 7.2 Groundwater Analytical Results Laboratory analytical results of the groundwater sample collected from monitoring well MW-1 detected several dissolved target compound concentration above established 2L Standards but below GCLs. A summary of the groundwater analytical data is presented in Table 5. Dissolved contaminants of concern above 2L Standards are depicted on Figure 4. A copy of the laboratory analytical report is included as Appendix C. 8.0 CONCLUSIONS AND RECOMMENDATIONS 8.1 Conclusions Based on the information obtained during this LSA, it appears the site has been impacted from a petroleum release.  The subject facility and surrounding area are connected to the public water supply provided by the City of Greensboro Water Resources Department.  Active water supply wells were not identified within 1,500 feet of the source area. According to the property owner, the property is connected to the public water system and the well have been inactive and capped since 1977.  The subject facility is not located within an approved Wellhead Protection Area.  Surface water was not identified within 500 feet of the source area. The nearest surface water feature is an unnamed surface water feature located approximately 1,085 feet north of the source area.  Free product was not detected in monitoring well MW-1.  Laboratory analytical results of the soil samples collected from the borehole of monitoring well MW-1 detected benzene and dibromochloromethane concentrations above established Soil-to-Groundwater (STGW) Maximum Soil Contaminant Concentrations (MSCCs) but below established Residential MSCCs.  Laboratory analytical results of the groundwater sample collected from monitoring well MW-1 detected several dissolved target compound concentrations above established 2L Standards but below GCLs. 8.2 Recommendations The results of the LSA activities indicate that the subject facility meets the criteria for Low Risk classification. 9.0 LIMITATIONS This report has been prepared for the exclusive use of Circle K Stores, Inc. The opinions included herein are based on information obtained during the study, on our experience in accordance with currently accepted hydrogeologic and engineering practices, and relevant regulatory guidelines at this time and location. Other than this, no warranty is implied or intended.
