B. Site History
1. Provide UST Owner and Operator Information
See Table 1.

No ASTs or surface spills are known to exist at the site.
4. Describe relation of location of current UST system to historical UST releases.
A previous release is noted to have occurred on the property at an unknown date and it was
issued UST incident #30338. This incident was closed on July 29, 2013. The file has been
archived by Guilford County and was not able to be reviewed at the time of this report.
5. Provide a Brief Description of Site Characteristics
The subject property consists of one convenience store building and petroleum fueling
equipment. The subject property and surrounding area is fairly level. The subject property is
located in an intermixed commercial and residential area of Greensboro. Public water is
supplied to the area as water meters were noted on the subject property and adjacent developed
properties. An unnamed surface water feature is located approximately 1,200 feet southeast of
the source area and another unnamed surface water feature is located approximately 1,085 north
of the source area. A topographic map and site map are attached as Figures 1 and 2,
respectively.
C. Site Check Procedures
1. UST System Testing
On January 24, 2020, during annual testing, the premium gasoline spill bucket (tank #2) was
found to not pass its respective hydrostatic test. Circle K is currently repairing the spill buckets
and information regarding the repairs is not available at the time of this report.
On April 22, 2020, upon receipt of the soil analytical results, ATC submitted a UST-61:
24-Hour Release and UST Leak Reporting Form to the Guilford County Department of Health
and Human Services office via e-mail correspondence.
Copies of the testing results and the referenced notification are provided in Attachment I. \

2. Site Check Procedure
On April 13, 2020, ATC personnel contracted with Carolina Soil Investigations, LLC (CSI) to
advance one soil boring (labeled SB-1) near the premium gasoline (tank #2) spill bucket. Soil
boring SB-1 was advanced to thirteen (13) feet below ground surface (bgs). The soil sample
was then submitted for laboratory analyses. The location of the soil sample is provided on
Figure 2.
It should be noted the soil sample was collected as close as possible to the premium gasoline
(tank #2) spill bucket.
3. Soil Excavation Activities
Soil excavation was not performed as part of this site check.
D. Site Investigation
1. Field Screening
Soil sample SB-1 was screened with a photoionization detector (PID) following sample
collection. The respective PID reading is provided on Table 3.
2. Soil Sampling Information
Soil sample SB-1 was collected at approximately thirteen feet bgs on April 13, 2020. The soil
encountered at the sampling depth consisted of a red clayey silt. The boring log for SB-1 is
provided in Attachment II.
The soil sample was shipped to a N.C. certified laboratory in a chilled cooler following proper
chain-of-custody procedures. Soil sample SB-1 was requested to be analyzed by TPH-Gasoline
Range Organics (GRO) and Environmental Protection Agency (EPA) Method 8260B.
3. Groundwater and Surface Sampling Information
Surface water and groundwater samples were not collected during site check activities.
4. Quality Control Measures
The soil sample was placed in laboratory provided jars and vials. The sample was immediately
placed on ice and contained in a cooler during transportation. The sample was transported by
courier under a chain-of-custody to the laboratory for analyses.
5. Investigation Results
Laboratory analytical results of soil sample SB-1 detected a dibromochloromethane
concentration above its established Soil-to-Groundwater (STGW) Maximum Contaminant
Concentration (MSCC). Other target compound concentrations were detected above laboratory

method detection limits but below established STGW-MSCCs. The results are provided in
Table 3. A copy of the laboratory report and chain-of-custody documents are provided in
Attachment III.
E. Conclusion and Recommendations
Laboratory analytical results of the soil sample collected near the premium gasoline (tank #2)
spill bucket detected a dibromochloromethane concentration above applicable standards. Based
on the soil analytical data, it appears a release of petroleum has occurred from the premium
gasoline (tank #2) spill bucket.
