Smith GRI Project No. 4911 ii EXECUTIVE SUMMARY North State Telephone Co. - North Office is an operating telecommunications facility. The site is located at 3212 North Main Street in High Point, Guilford County, North Carolina. The adjoining properties are commercial and residential. Municipal water is available to the site. The site contained one UST basin located on the northern side of the site building. The UST basin contained one 4,000-gallon heating oil UST. The UST was closed in-place with inert foam on October 12, 2020. During closure in-place activities, six soil samples were collected around the UST and at the fill port. Limited access to the south and east of the UST was available. Therefore, perimeter soil samples were collected on the downgradient, northern side and the western side of the UST. The RALs were above the concentrations of TPH-GRO and TPH-DRO reported in all of the soil samples analyzed for such. The soil to water MCCs were above the concentrations of all analyzed constituents reported for the two soil samples collected from the monitoring well boring location. One Type II monitoring well (MW-1) was installed in the location of soil samples MW-1 (8-10’) and MW-1 (18-20’), downgradient of the UST, in accordance with the NCDEQ UST guidelines for closure in-place. Monitoring well MW-1 was installed to a terminal depth of 40 ft. bgs with a 20-foot screened interval. MW-1 was gauged, purged, and sampled on October 12, 2020 in accordance with the NCDEQ guidelines. The MACs were above the reported concentrations of all analyzed constituents in the ground water samples collected from monitoring well MW-1. Based on the laboratory results, no petroleum compounds were reported at concentrations exceeding NCDEQ soil or ground water standards. UST Owner and Operator Information: In accordance with the UST Closure Report (UST-12 format) guidelines, the underground storage tank (UST) owner and operator information has been summarized in Table 1. 2. UST System Information: In accordance with the UST Closure Report (UST-12 format) guidelines, the UST system information has been summarized in Table 2. A copy of the Registered Tank Report is shown below: 3. Non-UST Information: No aboveground storage tanks (AST) are located at the site. 4. Comprehensive Description of the Release: Based on the soil and groundwater results being below the unrestricted regulatory levels, no further assessment should be required at the site. 5. Description of the Site Characteristics: The North State Telephone Co. - North Office site is an operating telecommunications facility located at 3212 North Main Street in High Point, Guilford County, North Carolina (Figure 1). The site is bordered to the west by North Main Street and to the east by US Highway 311 - Business. A site map has been included as Figure 2. 6. Summary of Initial Abatement Actions, Assessment Activities, and Corrective Actions: On July 15, 2020, the NCDEQ granted permission for the one 4,000-gallon heating oil UST to be closed-in- place. Preparation for Closure: All appropriate officials (i.e. fire marshal) were notified by Petroleum Equipment & Services, Inc. (PES) prior to the UST closure. Tailored Foam, Inc. performed the UST in-place closure activities. A Notice of Intent: UST Permanent Closure (UST-3) Form was submitted to the NCDEQ and the Guilford County Department of Health and Human Services (Guilford Co.) on October 6, 2020. A copy of the UST-3 Form has been included as Appendix B. A copy of the Site Investigation Report for Permanent Closure or Change-in-Service of UST Form (UST-2A Form) is included as Appendix C. 2. Closure Procedure: On October 12, 2020, one 4,000-gallon heating oil UST was closed in-place with inert foam. No dispensers were associated with the UST and the associated product piping was removed. All product piping exiting the UST to the inlet into the site building was above grade. No soils were removed from the site. A photo of the location of the UST is shown below: 3. Residual Material: Prior to the initiation of the UST closure activities, product and residual liquid were removed from the UST and product lines. 4. Release Information: UST closure soil samples were collected from around the UST and at the fill port. Based on the soil results being below the unrestricted regulatory levels, no further assessment should be required at the site. 5. Soil Excavation Activities: No soils were removed from the site during the October 2020 UST in-place closure activities. C. SITE INVESTIGATION PES contracted GRI to supervise UST closure by closure in-place activities of one 4,000-gallon heating oil UST at the site. The closure activities were conducted at the site on October 12, 2020. The UST was located to the north of the site building and covered with mulch and soil. The 4,000-gallon heating oil UST was assumed to be approximately 64 inches in diameter by 24 feet in length. The UST is reported to be constructed of single wall steel with aboveground copper product lines. During closure activities on October 12, 2020, GRI personnel collected soil samples for field screening using a photoionization detector (PID) and laboratory analysis. Field screening results are presented in Table 3. Five soil samples were collected adjacent to the closed-in-place UST (SB-1 (8’), SB-2 (8’), SB-3 (8’), MW-1 (8-10’), and MW-1 (18-20’)) using a decontaminated hand auger or direct push technology (DPT). One soil sample (SB-5 (3’)) was collect adjacent to the UST fill port using a decontaminated hand auger. Soil samples SB-1 (8’), SB-2 (8’), SB-3 (8’), and SB-5 (3’) were submitted to a certified North Carolina laboratory for analysis of total petroleum hydrocarbons gasoline range organics (TPH-GRO) and diesel range organics (TPH-DRO) using the Environmental Protection Agency (EPA) Methods 8015C. Soil samples MW-1 (8-10’) and MW-1 (18-20’) were submitted for analysis of volatile organic compounds (VOCs) using EPA Method 8260B, semi-volatile organic compounds (SVOCs) using EPA Method 8270D, and volatile petroleum hydrocarbons (VPH) and extractable petroleum hydrocarbons (EPH) using the Massachusetts Department of Environmental Protection (MADEP) Methods. Limited access to the south and east of the UST was available. Therefore, perimeter soil samples were collected on the downgradient northern side and the western side of the UST. Subsurface boring logs are included as Appendix D. D. FREE PRODUCT INVESTIGATION AND RECOVERY REPORT Free product was not encountered during the October 2020 UST closure in-place activities. E. GROUND WATER INVESTIGATION On October 12, 2020, one Type II monitoring well (MW-1) was installed downgradient of the UST, in accordance with the NCDEQ UST guidelines for closure in-place. Monitoring well MW-1 was installed to a terminal depth of 40 feet below ground surface (ft. bgs) with a 20-foot screened interval. MW-1 was gauged, purged, and sampled on October 12, 2020 in accordance with the NCDEQ guidelines. The ground water samples collected from MW-1 were submitted to a certified North Carolina laboratory for analysis of VOCs by EPA Method 602, SVOCs by EPA Method 625, and VPH and EPH by the MADEP Methods. The location of monitoring well MW-1 is shown on Figure 2. Well construction information is included in Table 4. A well construction record is included as Appendix D. The Guilford Co. Monitoring Well Construction Permit is included as Appendix E. Initial Response Actions: One 4,000-gallon heating oil UST was closed in-place with inert foam. No soil was removed from the site.  Source Control Actions: One 4,000-gallon heating oil UST was purged of residual liquids and closed in-place.  Contaminant Migration Control Measures: No contaminant migration control measures were necessary.  Measures Taken to Mitigate Fire/Safety Hazards: Prior to closure, the appropriate officials were notified (i.e. Fire Marshal). Free product was not encountered during closure activities. There did not appear to be any immediate fire or safety hazards present during closure activities.  Measures Taken to Identify and Mitigate Pollution Hazards: No petroleum compounds exceeding NCDEQ soil or ground water standards were reported. No soil was removed from the site. 2. Initial Abatement Actions:  Contaminated Soil Storage/Treatment and/or Disposal: No soil was excavated during closure activities. Extent of Soil Contamination: No soil was removed from the site during the October 2020 UST closure in-place activities. 2. Excavation Process: No soil was excavated during the closure in-place activities. 3. Sampling Description: All soil samples were collected using a decontaminated hand auger or by DPT advanced by GRI personnel. Samples were collected while wearing clean, disposable nitrile gloves and immediately placed in laboratory supplied glassware and placed on ice. H. SOIL AND GROUNDWATER SAMPLE RESULTS  Soil: The regulatory action levels (RALs) were above the concentrations of TPH-GRO and TPH-DRO reported in all of the soil samples analyzed. The soil to water maximum contaminant concentrations (MCC) were above the concentrations of all analyzed constituents for the two soil samples collected from the monitoring well boring location. A summary of soil sample analytical results is presented in Table 3. A Soil Sample Location Map has been included as Figure 3. A Soil Quality Map has been included as Figure 4. A copy of the laboratory report is included as Appendix F. SUMMARY AND CONCLUSIONS  The North State Telephone Co. - North Office site is located in High Point, Guilford County, North Carolina. The site is an operating telecommunications facility.  On October 12, 2020, one 4,000-gallon heating oil UST was closed in-place with inert foam.  Six soil samples were collected around the UST and at the fill port. Tanks disposed of illegally in fields or other dumpsites can leak petroleum products and sludge into the environment.
