Market St., Greensboro, Guilford County, North Carolina NCDENR Incident No. 6367 Dear Mr. Brown: This letter provides a report of monitoring and remediation well abandonments completed in May 2023 for the TransMontaigne-Greensboro terminal (former Southeast Terminals) facility in Greensboro, North Carolina. The well abandonments were completed in conjunction with a request outlined in an April 13, 2023 letter from Piedmont Geologic to North Carolina Department of Environmental Quality (NCDEQ) Winston-Salem Regional Office (WSRO) (Re: Request to Abandon Monitoring and Remediation Wells). The request was approved by NCDDEQ WSRO via an April 18, 2023 letter to TransMontaigne. The site location is shown in Figure 1 (Attachment 1). The site layout, including locations of the fifty-two site groundwater monitoring wells and twenty-six remaining groundwater remediation wells existing prior to the May 2023 abandonments is shown in Figure 2. Well-construction details are listed in Table 1 (Attachment 2). The well abandonments were completed from May 22-26, 2023 by a North Carolina-licensed well driller employed by MW Drilling, under supervision by Piedmont Geologic, in accordance with 15A NCAC 2C. A total of twenty-six of the existing fifty-two site monitoring wells plus all twenty-six existing site remediation wells were abandoned1. Site wells retained versus site wells abandoned are highlighted as such in Figure 2. Well abandonment procedures were as follows. NCDEQ well-abandonment records are provided in Attachment 3. A photolog documenting the well abandonments is provided in Attachment 4. • The wells were sounded to verify the completion depth and that no blockages were present. • Remediation piping/tubing in remediation well vaults was removed to the extent possible/practicable and disposed. Remaining piping/tubing ends were capped. 1 As requested in the April 18, 2023 NCDEQ letter, the abandonments included MW-6 and MW-51, whereas monitoring-wells MW-12, MW-35, MW-37, MW-38, and SME-10 were abandoned. Site Monitoring and Remediation Well Abandonments TransMontaigne-Greensboro Terminal June 12, 2023 2 PIEDMONT GEOLOGIC, P.C. • The well screens/casings were tremie grouted to grade with cement/bentonite slurry. • With the exception of remediation wells in the paved terminal loading-rack area, well manholes and stickup casings were removed and disposed, along with the associated concrete pads. Stick-up well casings were cut down to grade. The ground surface at each well location was regraded to match surrounding conditions. The remediation-well manhole casings in the paved loading rack area were retained in place and filled with concrete to grade (i.e., to minimize concrete pavement repairs). Site monitoring wells remaining following the well abandonments are shown in Figure 3. Well-construction details for the remaining wells are listed in Table 2.
