Waters Edge Environmental, LLC (Waters Edge) is pleased to present the attached Initial Assessment Report (IAR) for the October 18, 2022 recovered used oil and wastewater release at the Shamrock operation located at 6106 Corporate Park Drive in Browns Summit, North Carolina
(see Figure 1). After remedial activities were completed, two confirmatory soil samples were
below both TPH-GRO and TPH-DRO thresholds of 50 mg/kg and 100 mg/kg respectively. Since the release was in excess of 25 gallons, this was reported to the NRC in Atlanta, Georgia. We have included a UST-62 form and Initial Site Assessment form which are all documented in Appendix A.

1 Introduction
Waters Edge has prepared this Initial Assessment Report (IAR) for the October 18, 2022 recovered
used oil and wastewater release at the Shamrock operation located at 6106 Corporate Park Drive in Browns Summit, North Carolina (see Figure 1). This IAM Report follows the NCDEQ UST Section Guidelines for Investigation and Remediation of Contamination from Non-UST Petroleum Releases-with the site information cover sheet. We have also included UST-62 24-hour
Notification of Discharge Release Form. Since the release was in excess of 25 gallons, Shamrock
also reported the incident to the NRC in Atlanta, Georgia. All of this is documented in Appendix A. Herein, are the remaining sections of the report.
2 Initial Assessment Report
2.1 Site History and Characterization 1. List, describe, and indicated location of AST systems and of all spill or other non-UST
releases-
On October 18, 2022, at approximately 16:45 hours, Shamrock Environmental Corporation experienced a release of oil water mixture containing approximately 50 gallons of heavy used oil recovered from oily wastewater. The release was reported as an equipment failure due to fouled pump transferring oil from the 500-gallon flow through tank to the storage
tank. Some preliminary information is summarized below:
• Type facility: Industrial Wastewater Pre-treatment (Discharge to Greensboro POTW). • Facility address: 6106 Corporate Park Dr Browns Summit (Greensboro), NC 27214 • City of Greensboro Significant Industrial User Permit P065. • SW General Permit: NCG110000 COI NCG110157
2. Describe initial abatement activities, assessment activities and corrective action performed to date and list all reports previously submitted- There were no previous reports. The oil was released to a gravel area adjacent to the transfer tank servicing the wastewater
clarifier at the Company’s industrial wastewater treatment facility located at 6106
Corporate Park Drive Browns Summit, Guilford County, NC. The release was determined to be an RQ release (> 25 gallons oil released, no sheen migration off site, no RQ release for other contaminants). Immediately following discovery, recovery and cleanup operations were undertaken and the release reported to the USCG National Response
Center and call verified as received by NC Emergency Management. USCG NRC Report
NRC#1350274 (see Appendix A). A spill location map is shown in Figure 2. The release was reported as an equipment failure due to fouled pump transferring oil from the 500-gallon flow through tank to the storage tank. Mitigation efforts began immediately
on discovery and included stopping of the release and dispatching vacuum trucks to recover
used oil and impacted gravel. Mixed wastewater, oil and gravel were collected from the impacted area using vacuum trucks. On a subsequent additional visual inspection, it was determined that a small but visible
portion of the oil had migrated through the gravel and under an adjacent containment liner.
Portable tanks were cleaned and moved, the liner pulled back and an additional area approximately 4 feet by 10 feet and 18 inches in depth was excavated using vacuum trucks (see Figure 2 and disposal manifest in Appendix B). In summary, approximately 4,000 gallons and 34 tons of used oil, wastewater, gravel and soil were removed and disposed as
industrial solid waste (see manifest in Appendix B).
The regional area would be considered residential. The immediate are would be considered commercial/light industrial.
Shamrock subsequently contracted with Waters Edge to perform confirmatory sampling.
On November 23, 2022, Water Edge mobilized to the site. We collected two (2) samples with a hand auger (SS-1 and SS-2-see Figure 2). The samples were collected at 0.5’ bgs with a hand auger and submitted to the laboratory according to EPA-accepted procedures. Based on Shamrock guidance, this sample was analyzed for both gasoline-ranged and
diesel-ranged total petroleum hydrocarbons (TPH-GRO/DRO) according to SW-846
Method 8015 (results summarized in Table 1 and documented in Appendix C). 3 Conclusions and Recommendations
Based upon the soil assessment findings, we would conclude the following:
• Both samples were below the TPH-GRO and TPH-DRO thresholds of 50 mg/kg and
100 mg/kg respectively. As such, we would conclude that no further remediation is
required. With submittal of the UST-62 Form, we would conclude that the release has been reported to NCDEQ.

PQL - Practical Quantitation Limit.
RL - Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and
bias for a specific analyte in a specific matrix.
S - Surrogate
1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is
a combined concentration.
Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values.
