The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1. The required signage is posted at the entrance of the facility.
2. The facility is secured by an automatic gate at the entrance to prevent unauthorized access.
3. The facility is open from 8 am to 4:30 pm Monday through Friday.
4. The facility is permitted for the treatment of medical waste with steam sterilization, as well as the storage and
processing of treated and untreated medical waste.
5. The facility is permitted to use four steam sterilization (autoclave) units. Currently there are two sterilization
units on -site and in use.
6. The facility was clean and sanitary.
7. The operations floor, compactor area and unloading dock are cleaned daily.
8. Floor drains in the operational area were clean and free of debris. Floor drains are connected to City of
Gastonia sanitary sewer system.
9. The facility utilizes The Pest Control Authority quarterly as a vector control measure. The Pest Control
Authority was on -site on September 1, 2023 and are scheduled December 1, 2023 for the last quarter of 2023.
10. Trilogy MedWaste Southeast leases three vehicles for the pickup and transportation of medical waste in North
Carolina and South Carolina. The trucks are leased from Ryder and are equipped with spill kits.
11. Trilogy MedWaste Southeast LLC leases two compactors from Tower Compactor Rentals, Scottsdale, AZ.
Republic Services is scheduled to pick up the compactors every Monday, Wednesday and Friday and
transported to CMS Landfill in Concord for disposal..
12. The area around the compactors was clean and free of windblown waste.
13. No windblown waste observed in the loading dock area.
14. The facility installed a cooler inside the building in 2018. Current temperature of the cooler during the
inspection was 45°F.
15. Pathological waste is transported every eight (8) weeks by Curtis Bay Energy to their facility in Baltimore,
Maryland for incineration. Curtis Bay Energy's last picked up was on July 31, 2023.
16. Hazardous waste is transported by Heritage, LLC to their Heritage -WTI facility in East Liverpool, Ohio for
incineration.
17. All medical waste is processed within 24 to 48 hours from the date of delivery.
18. Radiation detectors are mounted above each of the two receiving bay doors to screen for radioactive materials.
19. All recycled containers are washed, disinfected, and inspected for damage prior to being returned to customers.
20. All treatment carts are relined after each autoclave cycle.
Records Review
21. The facility staff had the current permit and operations plan in their files.
22. The facility utilizes Truenorth online for internal training.
23. Every Tuesday each autoclave is tested to ensure that sterilization is being achieved. These tests are logged on
a "Variclave Periodic Spore Test Log". Spore test data records were reviewed from October 31, 2022, to
September 25, 2023.

24. Each load of waste that is received is tracked electronically on a "Waste Manifest" form which details the date
the waste was received, the source of the waste, the amount of waste and the type of waste received.
25. Landfill disposal tickets were reviewed from October 31, 2022, to August 31, 2023.
26. The facility processed 359.32 tons from Nov 2022 through December 2022. The facility has processed
1,087.98 tons from January 2023 to August 2023.
