Active, Life of Site Permit issued March 27, 2019 and expires August 28, 2056. This permit is in the names of: Currituck County (owner) and Republic Services of North Carolina, LLC (operator). PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS: 01 - This Comprehensive Facility Compliance Inspection was conducted with Leighton Earwood, Currituck Solid Waste Director. 02 - Minimal windblown trash was noted during this inspection around the transfer station main yard and the
trailer storage area during working hours. 03 - Tipping floor and loading bay were clean as they can be during working hours. 04 - Fire prevention consist of 2 charged fire extinguishers and hoses on the tipping floor. 05 - Facility has plenty of empty trailers onsite. No leaks noted on loaded trailers. 06 - Facility has 5 certified Transfer Station Operations Specialist: Martyn Mitchell, expires 11/14/25, Chris Simbo, expires 2/18/2025, Damon Battle, expires 5/10/2026, Gregory Simpson, expires 2/3/2026 and Horace Griffin, expires
7/24/2024. 07 - PTO, Operations Plan and Screening records were not reviewed during this inspection. 08 - Roads are constructed of all-weather materials and maintained well. 09 - From Last Report: Facility needs to add (Required) Name of Facility Operator, Emergency contact information and types of waste the Permit to Operate has listed on the Entrance signage. May want to add (not required) the owners information as well. 10 - The Yard Waste has just been mulched and is being removed. 11 - Fuel barrel has secondary containment and is equipped with a charged fire extinguisher. Inspection tag was present and up to date. 12 - Concrete barriers are placed around the scrap metal, white goods & container area to keep waste on the concrete pad. This area is being managed very good. 13 - Scrap metal was being removed during this inspection. 14 - White goods containing CFC were all up right until the CFC has been removed and then marked and placed on the scrap metal pile. 15 - Leachate tanks appear to have good storage. Need to replace 1 lid.
16 - Scrap tires are laced into box containers. 17 - The exterior issues on the building noted on the last inspection have been repaired.
18 - Overall facility management is good.
19 - Digital pictures were taken during this inspection with some attached to this report.
