PURPOSE OF SITE VISIT: Routine Inspection
STATUS OF PAST NOTED VIOLATIONS:
The leak noted in the 10/21/2021 inspection report has been repaired and the repair remains in tact.
OBSERVED VIOLATIONS:
N/A
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) This facility consists of one 1,500 gallon concrete tank and one 3,500 gallon concrete tank for storage
and treatment of domestic septage, portable toilet waste, and grease trap pumpings.
2) 15A NCAC 13B .0838(b): The access road is well maintained for all weather conditions.
3) 15A NCAC 13B .0838(c): No holes or cracks which may allow the escape of septage were observed at the
time of inspection. As noted on the previous page, the repair of a leak previously noted appears to be in tact.
4) 15A NCAC 13B .0838(e)(2) and 15A NCAC 13B .0838(g): No vectors or odors were noted at the time of
inspection.
5) 15A NCAC 13B .0838(i): At the time of inspection, vegetation was present within 5-feet of the tanks.
Please ensure that vegetation remains cleared within 5-feet of the tank to meet the requirements of this
rule.
6) 15A NCAC 13B .0838(j): Septage appears to be transferred to and from the tanks in a manner that
prevents leaks or spills of septage onto the tank or ground.
7) 15A NCAC 13B .0838(k): Appropriate signage was noted at the site.
8) 15A NCAC 13B .0838(m): Setback requirements appear to have been met.
