(S&ME) submits this report of groundwater sampling and hydraulic conductivity activities for the above-referenced property. The scope of services performed for this report was performed in general accordance with S&ME’s Proposal No. 43-1401038, dated November 21, 2014, and as authorized by NCDENR’s Task Authorization No. TA-01, dated November 25, 2014, and within NCDENR’s Contract No. N14005i. BACKGROUND A petroleum release was documented at the site in November 2008 when five underground storage tanks, including two 8,000-gallon gasoline, one 550-gallon used oil, and one 275-gallon heating oil, were removed. A Phase I Limited Site Assessment, completed by Agra Environmental in March 2010, documented the presence of petroleum compounds in the site soil at concentrations above soil-to-groundwater Maximum Soil Contaminant Concentrations (MSCCs), and petroleum constituents in the site groundwater at concentrations exceeding North Carolina groundwater quality standards (2L standards). Ethylene dibromide (EDB) was reported in the site groundwater at monitor well MW-1 at a concentration greater than the Gross Contaminant Level (GCL). No active water supply wells were identified within 1,000 feet of the site. Additional assessment was performed at the site in 2011, 2012 and 2013. Groundwater quality data reported for these additional sampling events indicate that EDB has been the only target petroleum constituent reported in the site groundwater at concentrations greater than GCLs. Groundwater Sampling and Hydraulic Conductivity Letter Report S&ME Project No. 4305-14-267 Baux Mountain 76, Incident #37353, Winston-Salem, NC March 9, 2015 2 SCOPE OF SERVICES MONITOR WELL SAMPLING On December 3, 2014, S&ME visited the site to collect depth to water measurements in monitor wells MW-1, 2, 3, 4 and 5, and to collect a groundwater sample from monitor well MW-1. The site location is shown on Figure 1. Monitor well locations are shown on Figure 2. Depth to water measurements are included in Table 1. After measuring the depth to water in monitor well MW-1, the well was purged with a disposable PVC bailer and a groundwater sample was collected for laboratory analysis. The sample was submitted to Pace Analytical Services, Inc. for analysis using Standard Method (SM) 6200B. Copies of the field sample log and laboratory report are attached. The laboratory results are presented in Table 2. Table 2 also includes previous groundwater data from sampling events conducted in 2012 and 2013. Contaminant concentrations continue to exceed the 2L Standards; however, concentrations appear to be decreasing. Ethylene dibromide (EDB) was the only target constituent reported at a concentration greater than the GCL. HYDRAULIC CONDUCTIVITY CALCULATION Rising head slug tests were performed on shallow monitor wells MW-1 and MW-2 on December 3, 2014. Using the Bouwer-Rice method, a slug test method for determining the hydraulic conductivityof unconfined aquifers with complete or partiallypenetrating wells, values for hydraulic conductivitywere calculated using the data collected on December 3, 2014. The values for hydraulic conductivityfor monitor wells MW-1 and MW-2 were 5.308 feet per day, and 1.832 feet per day, respectively. The slug test calculations for each well tested are attached. Groundwater Sampling and Hydraulic Conductivity Letter Report S&ME Project No. 4305-14-267 Baux Mountain 76, Incident #37353, Winston-Salem, NC March 9, 2015 3 RECOMMENDATIONS Based on the absence of known, active water supply wells within 1,000 feet of the site, the release incident may be eligible for closure after target petroleum constituent concentrations are documented to be less than GCLs. S&ME recommends the following: 1.Sample monitor well MW-1 on an annual frequency, with the next sampling to be conducted in January 2016. 2.Analyze the sample for volatile organics, (SM 6200B), lead, and MADEP Method Volatile Petroleum Hydrocarbons (VPH).
