(ATC) has completed a Groundwater Monitoring Report for the Baux Mountain 76 site. This report documents monitoring well installation and a groundwater sampling event conducted at the site in February 2022. The assessment activities were performed based on an April 7, 2022 Task Authorization (TA-02) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of State Lead Contract #N122019A between NCDEQ and ATC. Baux Mountain 76 is located at 4801 Old Rural Hall Road in Winston Salem, Forsyth County, North Carolina. Six monitoring wells (MW-1 through MW-6) are associated with the site. Municipal water is available to the site and site vicinity via the City of Winston Salem. The site currently operates as Ortega’s Garage a vehicle repair facility and the former UST system was removed in 2008. ATC personnel supervised the installation of one type II monitoring well (MW-6) at the site on February 11, 2022 in order to delineate the eastern extent of the contaminated groundwater plume at the site. Monitoring well MW-6 was installed to a total depth of 35 feet below ground surface and screened from 15 to 35 feet below ground surface. ATC personnel conducted a groundwater sampling event at the site on February 16, 2022. During the sampling event, groundwater samples were collected from six monitoring wells (MW-1, MW-2, MW-3, MW- 4, MW-5, and MW-6). Samples were analyzed for VOCs by SM 6200B. LNAPL was not observed during the sampling event, nor has LNAPL been detected historically. The direction of groundwater flow at the site is generally to the north-northeast. The results of the sampling event indicate the following:  Three active water supply wells (WSW-5, WSW-8 and WSW-11) used for drinking water and one active water supply well (WSW-30) used for non-potable use are located within 1,000 feet of the site. Municipal water is available in the site vicinity by the City of Winston Salem; o WSW-6 was previously listed as active but the property is vacant and the property owner confirmed that the well was not in use; o WSW-8 and WSW-11 are located greater than 500 feet up-gradient from the site; o WSW-30 is located 725 feet down-gradient from the site; o WSW-5 is considered the most “at-risk” water supply well since it is located 200 feet down- gradient from the site. The property owner for WSW-5 did not return the signed access agreement and attempts to sample WSW-5 during the sampling event were unsuccessful due to occupant absence;  Multiple VOCs were reported in MW-1, MW-5, and MW-6 exceeding the Title 15A NCAC 02L .0202 Groundwater Quality Standards (2L Standards);  Concentrations of EDB also exceeded the Gross Contamination Level (GCL) in MW-1;  Tetrachloroethylene (PCE) had been previously detected at the site and may be associated with the current vehicle repair activities; and  Impacted groundwater appears to be concentrated to the vicinity of the former 4,000-gallon UST near monitoring well MW-1 and extends eastward toward the newly installed monitoring well MW- 6; and  The overall VOC concentrations appear to be decreasing in monitoring well MW-1 but the horizontal extent of the impacted groundwater plume is not defined towards the north or east. Based on the results of the recent investigation, ATC recommends the installation of one on-site delineation monitoring well north of MW-5 and one off-site monitoring well east of MW-6. In addition, ATC recommends continued attempts to obtain an access agreement with property owner of WSW-5 to sample the water supply well and to discuss connecting the property to municipal water. If WSW-5 is connected to municipal water, the site risk can be re-evaluated. Baux Mountain 76 Incident No. 37353 April 22, 2022 Page 3 of 7 ATC Project No. SLC3735302 NCDEQ Trust Fund Branch 3.0 SITE DESCRIPTION 3.1 SITE CHARACTERIZATION The Baux Mountain 76 is located at 4801 Old Rural Hall Road in Winston Salem, Forsyth County, North Carolina. The site is associated with Incident #37353. The property is currently owned by Mr. and Mrs. Salvador Sanchez. Mr. and Mrs. Salvador Sanchez are the owners of the former UST system at the site. The site was accepted into the NCDEQ State-Lead Program in 2009. The Baux Mountain 76 site is bound to the east by Baux Mountain Road with residences beyond, to the west by Old Rural Hall Road with residences beyond, to the north by commercial and residential properties, and to the south by residential properties. The site vicinity is characterized as commercial and residential. Municipal water is supplied to the site and site vicinity by the City of Winston Salem. Twenty water supply wells are located within 1,000 feet of the site. Of the twenty water supply wells, only three are active for potable use and one is active for non-potable use. Water supply well ownership information is included in Table 1. A site topographic map is included as Figure 1 and a site plan depicting pertinent site features is included as Figure 2. Water supply well locations are shown on Figure 3. 3.2 SITE BACKGROUND On November 3, 2008, soil contamination was discovered by Trigon-Kleinfelder, Inc. during the removal of two (2) 8,000-gallon gasoline USTs, one (1) 4,000-gallon gasoline UST, one (1) 550-gallon used oil UST, and one (1) 275-gallon heating oil UST. As documented in the December 30, 2008 UST Closure Report, laboratory results of the soil samples collected during UST closure activities indicated exceedances of the NC Action Level for Gasoline Range Organics (GRO) in the soil located beneath the 4,000-gallon gasoline UST, the western 8,000 gallon-gasoline UST, and the 275 gallon heating oil UST. The sample collected from beneath the 275-gallon heating oil UST exceeded the NC Action Level for Diesel Range Organics (DRO). As a result, a 24-hour Release and UST Leak Reporting Form was submitted. A Notice of Violation was issued to the owners and of the UST system, Mr. and Mrs. Salvador Sanchez, on February 24, 2009. On March 31, 2010, a Phase I Limited Site Assessment Report was prepared by Agra Environmental, Inc. During the assessment, two groundwater monitoring wells were installed at the site (MW-1 and MW-2). Soil samples were collected during installation of monitoring wells MW-1 and MW-2. The results of the soil sampling indicated soil impacts exceeding the Soil-to-Groundwater Maximum Soil Contaminant Concentration (MSCC) Levels from 10 to 20 feet below ground surface near MW-1 and from 5 to 20 feet below ground surface near MW-2. Groundwater samples collected from monitoring wells MW-1 and MW-2 indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Lead and chromium were detected above the 2L Standards in monitoring well MW-2. 1,2- Dibromoethane (EDB) was detected at a concentration exceeding the established GCL in monitoring well MW-1. Agra Environmental, Inc., identified fourteen water supply wells within a 1,000 foot radius of the site during the assessment. On June 7, 2010, a Phase II Limited Site Assessment Report was prepared by Agra Environmental, Inc. During the assessment, three additional groundwater monitoring wells were installed at the site (MW-3, MW-4, and MW-5) and a groundwater sampling event was conducted on April 22, 2010. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Lead, chromium, and bromodichloromethane were detected above the 2L Standards in monitoring well MW-2. PCE was detected in monitoring well MW-5 exceeding the 2L Standard. Benzene and EDB were detected at concentrations exceeding the established GCLs in monitoring well MW-1. On June 13, 2011, a Groundwater Monitoring Report was submitted by Agra Environmental, Inc. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Benzene and EDB were detected in monitoring well MW- 4 exceeding the respective 2L Standards. PCE and EDB were detected in monitoring well MW-5 exceeding the respective 2L Standards. EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. On March 21, 2012, a Groundwater Monitoring Report was submitted by Agra Environmental, Inc. The laboratory results from the groundwater sampling event indicated that numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Benzene and EDB were detected in monitoring well MW-4 exceeding the respective 2L Standards. PCE was detected in monitoring well MW- 5 exceeding the respective 2L Standard. EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. On July 20, 2012, a Groundwater Monitoring Event Letter Report was submitted by Crawford Environmental Services. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Benzene was detected in monitoring well MW-4 exceeding the respective 2L Standard. PCE was detected in monitoring wells MW-2 and MW-5 exceeding the respective 2L Standard. EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. On February 26, 2013, a Groundwater Monitoring Report was submitted by Crawford Environmental Services. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Benzene was detected in monitoring well MW-4 exceeding the respective 2L Standard. PCE was detected in monitoring well MW-5 exceeding the respective 2L Standard. EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. On March 9, 2015, a Groundwater Sampling and Hydraulic Conductivity Letter Report was submitted by S&ME, Inc. A groundwater sample was collected from monitoring well MW-1 and rising head slug tests were performed on MW-1 and MW-2 on December 3, 2014. Laboratory results indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. EDB was detected at a concentration exceeding the established GCL in MW-1. Based on the results of the rising head slug tests, the hydraulic conductivity was estimated at 5.308 feet per day for monitoring well MW-1 and 1.832 feet per day for monitoring well MW-2. On January 28, 2019, a Groundwater Monitoring Report was submitted by Terracon Consultants, Inc. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. PCE was detected in monitoring well MW-5 exceeding the respective 2L Standard. EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. On November 8, 2021, a Groundwater Monitoring Report was submitted by ATC. The laboratory results from the groundwater sampling event indicated numerous compounds were detected at concentrations exceeding the 2L Standards in monitoring well MW-1. Benzene, EDB, and PCE were detected at concentrations exceeding the 2L Standards in monitoring well MW-5. EDB as detected at a concentration exceeding the GCL in monitoring well MW-1. 4.0 GROUNDWATER CHARACTERIZATION 4.1 MONITORING WELL INSTALLATION ATC personnel supervised the installation of one Type II monitoring well (MW-6) at the site on February 11, 2022 in order to delineate the eastern extent of the contaminated groundwater plume at the site. Prior to drilling activities, ATC notified the North Carolina one-call. Baux Mountain 76 Incident No. 37353 April 22, 2022 Page 5 of 7 ATC Project No. SLC3735302 NCDEQ Trust Fund Branch On February 11, 2022, ATC and a North Carolina-registered well driller, Geologic Exploration, mobilized to the site. Monitoring well MW-6 was installed to a total depth of 35 feet below ground surface. MW-6 was installed as a Type II, 2-inch diameter, schedule 40 PVC monitoring well with 20 feet of 0.010-inch factory- slotted well screen from the base of the borehole and riser casing to ground surface. ATC measured the TOC elevations for MW-6 using an existing monitoring well MW-1 as a reference elevation of 100 feet. The location of monitoring well MW-6 is shown on Figure 2 and the well construction record and boring log are included in Appendix C. 4.2 GROUNDWATER LEVELS Groundwater level measurements were collected from monitoring wells MW-1 through MW-6 on February 16, 2022. Monitoring well construction details and groundwater elevation data are summarized in Table 2. LNAPL was not observed during the sampling event, nor has LNAPL been detected historically. Field parameters were measured during the February 16, 2022 sampling event and are summarized in Table 3. The readings are generally consistent, with no apparent indication of biological degradation. The direction of groundwater flow at the site is generally to the north-northeast. A groundwater elevation contour map is included as Figure 4. Field notes from the February 16, 2022 site visit are included in Appendix A. Photographs of the site are included in Appendix B. 4.3 GROUNDWATER QUALITY On February 16, 2022, groundwater samples were collected from monitoring wells MW-1 through MW-6. The samples were collected in laboratory-approved containers, packed in ice and delivered under chain-of-custody protocol to SGS North America Inc. (SGS) for analyses. The groundwater samples were analyzed for VOCs by SM 6200B. Laboratory analytical results from the February 16, 2022, groundwater sampling event indicate the following compounds exceeded the 2L Standards:  Benzene: MW-1, MW-5, MW-6  Toluene: MW-1 and MW-6  Ethylbenzene: MW-1 and MW-6  Total Xylenes: MW-1 and MW-6  MTBE: MW-6  Naphthalene: MW-1, MW-5, and MW-6  n-Propylbenzene: MW-1  1,2,4-Trimethylbenzene: MW-1 and MW-6  EDB: MW-1 and MW-5  1,2-Dichloroethane: MW-5 and MW-6  2-Hexanone: MW-6  PCE: MW-5 EDB was detected at a concentration exceeding the established GCL in monitoring well MW-1. PCE had been previously detected at the site and may be associated with the current vehicle repair activities. Groundwater analytical data are summarized in Table 4. A benzene isoconcentration contour map is included as Figure 5. Other chemicals of concern are presented in Figure 6. The laboratory analytical report and chain-of-custody forms are included in Appendix D. Historical groundwater analytical data is summarized in Table 5. As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the laboratory report was conducted. During the review, it was concluded that the samples were received by the laboratory in acceptable condition and properly preserved. The samples were analyzed within the correct holding times, and the chain of custody was properly completed by all parties. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for each analytical method. Refer to the laboratory report in Appendix D for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the conclusions presented in this report. 4.4 WATER SUPPLY WELL SAMPLING ATC attempted to collect samples from the two closest water supply wells (WSW-5 and WSW-6) during the February 2022 groundwater monitoring event. Signed access agreements were not returned to ATC for either of the water supply wells by the date of the groundwater monitoring event. ATC personnel knocked on the door of the residence at 4511 Baux Mountain Road (WSW-5) but the owner was not home and the well was not sampled. ATC observed the water supply wells during the event. The property where WSW-6 is located was a vacant lot and the property owner informed ATC that the water supply well was not in use.
