Six underground storage tanks (USTs) historically operated at the site. A petroleum release was discovered during site assessment activities in 1999. Two 8,000-gallon gasoline, two 4,000-gallon gasoline, one 1,000-gallon kerosene, and one 4,000-gallon diesel USTs were removed from the site in 2018. The purpose of this report is to document monitoring well installation and the latest groundwater sampling event. On November 15, 2022, CATLIN Engineers and Scientists (CATLIN) personnel advanced three borings for construction of Type II monitoring wells (MW-01, MW-02, and MW-03) to 30 feet below land surface (BLS). CATLIN returned to the site on November 25, 2022 to sample the newly installed wells. Groundwater was measured at approximately 21.5 feet deep across the site. Groundwater samples from the three monitoring wells were collected. The samples were submitted for laboratory analysis of volatile organic compounds (VOCs) per Standard Method (SM) 6200B plus ethylene dibromide (EDB), methyl tert-butyl ether (MTBE) and diisopropyl ether (DIPE). Multiple VOCs were detected in the MW-01 sample at concentrations greater than those established in Title 15A of the North Carolina Administrative Code, Subchapter 2L, Section .0202 Groundwater Quality Standards (2L GWQS). Benzene and EDB were detected above their Gross Contaminant Levels (GCL) in the MW-01 sample. Only benzene was above its 2L GWQS in the MW-03 sample. CATLIN recommends installing additional monitoring wells in an attempt to delineate the horizontal and vertical extent of GCL impacted groundwater and determine vertical gradient. It is also recommended to sample the three active water supply wells within 1,000 feet of the site. 3.0 SCOPE OF SERVICES The scope of services for this report was performed in general accordance with CATLIN proposal number 221185.2P, dated October 19, 2022. Services were authorized by the North Carolina Department of Environmental Quality’s (NCDEQ’s) Task Authorization (TA) #02, approved on October 19, 2022, and Contract Number N122019D. The approved scope of services for this Task Authorization included to: • Coordinate access with the property owners’ lawyers; • Update the site-specific Health and Safety Plan (HASP); • Install three Type II monitoring wells up to 30 feet BLS; • Survey the top of casing (TOC) elevations of the new wells; • Collect three groundwater samples (one from each new well); NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 3 December 2022 • Analyze five water samples (three monitoring wells, one field blank, and one trip blank) per SM 6200B (plus EDB, MTBE, and DIPE); and • Prepare a monitoring report summarizing the findings. The scope of services was conducted in general accordance with the approved TA. 4.0 SITE HISTORY AND CHARACTERIZATION The Former Texaco Food Mart #2 site is located at 1700 Cotton Grove Road, Lexington, Davidson County, North Carolina (Figures 1 and 2). Summarized available historical sampling and gauging data from previous consultants are provided in Appendix A. Recent site photographs are presented in Appendix B. 4.1 UST OWNER AND OPERATOR INFORMATION Holding Bros, Inc. (Holding Bros) owned the six USTs historically operated on-site. The tanks were operated by Holding Bros for petroleum retail sales until 2003, when the property and tank ownership were transferred to United Oil of the Carolinas, Inc. (United Oil) (NCDEQ, 2020). Holding Bros is now dissolved, and it is not clear if another release occurred during United Oil’s ownership (Stoner, 2019). The site was accepted into the North Carolina UST Section Trust Fund Branch State Lead Program in 2021 (NCDEQ, 2021B). Available information regarding UST owners/operators is presented on Table 1. 4.2 UST INFORMATION Two 8,000-gallon gasoline, two 4,000-gallon gasoline, one 1,000-gallon kerosene, and one 4,000-gallon diesel USTs were historically operated on- site. The tanks were installed in 1984 (Tanks 1, 2, and 5), 1989 (Tanks 3A and 4A), and 1998 (Tank 6) and reportedly operated until October 9, 2018. The tanks were removed from the site on October 15 and 16, 2018 (NCDEQ, 2018). A previous UST system consisting of one 2,000-gallon gasoline and one 1,000-gallon diesel USTs were installed in 1965. They were reportedly removed in 1989 and replaced with Tanks 3A and 4A. These tanks were not historically associated with the release (NCDENR, 2020). Available UST system information is presented in Table 1 and the approximate former UST system locations are illustrated on Figure 3. NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 4 December 2022 4.3 CONTAMINANT SOURCE INVENTORY An on-site release was discovered in 1999 during site assessment activities. Soil sample analytical results revealed total petroleum hydrocarbons (TPH) gasoline range organics (GRO) and diesel range organics (DRO) above their respective State Action Levels (Ogden, 1999). 4.4 SITE CHARACTERISTICS The property is owned by Cook Out – Cotton Grove, Inc. and the property is occupied by a Cook Out fast-food restaurant. The parcel is comprised of the restaurant building and paved parking areas. Site photographs are presented in Appendix B. 4.5 OWNERS AND OCCUPANTS OF ADJOINING/NEARBY PROPERTIES AND LAND USE Properties in the site vicinity are a mix of commercial businesses, residences, and undeveloped wooded land. The site is bound by Cedar Lane to the north and Cotton Grove Road to the west. Other fast-food restaurants are located to the south and north (across Cedar Lane). A gas station/convenience mart and hotel are located across Cotton Grove Road to the west and northwest, respectively. Wooded land adjoins the site to the east. 4.6 RECEPTOR INFORMATION The most recent receptor survey for the site was conducted by CATLIN in 2021. A summary of pertinent receptor information is described below. Potential receptors are illustrated on Figure 2 and receptor information obtained from CATLIN (2021) is shown on Table 2. Based on correspondence with the City of Lexington Water Resources department, municipal water should be available to all properties in the site vicinity. Twelve water supply wells have been identified within 1,500 feet of the site. No water supply wells were within 500 feet of the site. Within 1,000 feet of the site, three active potable wells and eight inactive wells were identified. One active potable and one inactive well are located between 1,000 and 1,500 feet from the site. No surface water bodies were identified within 1,500 feet of the site during the last receptor survey. 4.7 PREVIOUS INVESTIGATIONS AND REMEDIAL ACTIVITIES The release was discovered in 1999 during a Preliminary Site Assessment (PSA) for the North Carolina Department of Transportation (NCDOT). Soil samples obtained from the right-of-way (ROW) adjoining the site reportedly contained TPH GRO up to 1,200 milligrams per kilogram (mg/kg) and TPH NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 5 December 2022 DRO up to 1,300 mg/kg (Ogden, 1999). A subsequent Site Check Report in May 2001 identified soil contamination above State Action Levels in the vicinity of the kerosene UST and dispenser, and the eastern gasoline dispenser (NCDEQ, 2020). A Phase II Limited Site Assessment (LSA) published in October 2001 identified groundwater contamination in two water supply wells (WSW-1 and WSW-4) within 1,000 feet of the site (NCDEQ, 2020). Shield Engineering, Inc. (Shield) provided a Comprehensive Site Assessment (CSA) in May 2002. A total of six shallow/Type II monitoring wells (MW-1 through MW-6) and one deep/Type III monitoring well (T-1) were installed. Soil sampling analytical results revealed multiple VOCs above their Soil-to- Groundwater (STGW) Maximum Soil Contaminant Concentrations (MSCCs). 1,2-Dibromoethane was also reported above its Residential MSCC. Laboratory analysis of the groundwater samples reported several VOCs, volatile petroleum hydrocarbons (VPH), and/or extractable petroleum hydrocarbons (EPH) above their 2L GWQS in the samples from MW-1, MW-2, MW-3, and T-1. Benzene, 1,2-dichloroethane (1,2-DCA), and EDB also exceeded their GCL in the sample from MW-2 (Shield, 2002). A Corrective Action Plan (CAP) was also issued in May 2002 and received by the North Carolina Department of Environmental and Natural Resources (NCDENR, now NCDEQ) in October 2003. An Air Sparge and Soil Vapor Extraction (AS/SVE) system was recommended for remediation (NCDEQ, 2020). Multiple groundwater monitoring and soil sampling events were conducted by various parties between 2007 and 2018, including a CSA Addendum in 2010. The highest contaminant concentrations have been in samples from MW-2 and MW-3. Dissolved phase petroleum constituents at concentrations above 2L GWQS persisted in samples from MW-1, MW-2, MW-3, and T-1. Petroleum contaminant concentrations have fluctuated in samples from monitoring wells MW-4 and MW-5. Light non-aqueous phase liquid (LNAPL, aka free-phase product) has not been previously detected on-site. Benzene, 1,2-DCA, and EDB were consistently above their GCL in samples from MW-2 (NCDEQ, 2020; Shield, 2018). Remediation activities have included multi-phase mobile extraction (MMPE) and air sparging events. The 2009 MMPE removed a total of 32.48 gallons of VOCs from the subsurface, and in 2018 1,588 gallons of impacted groundwater were recovered. Eighteen bi-weekly air sparging events were conducted between 2013 and 2015 (NCDEQ, 2020; Shield, 2018). The on-site USTs were removed in 2018 by Geological Resources, Inc. (GRI). Approximately 139.69 tons of soil were excavated during UST closure activities, including removal of overburden and over-excavated soils from the 8,000-gallon gasoline UST basins and the kerosene dispenser. Confirmation NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 6 December 2022 soil samples revealed TPH GRO and/or DRO concentrations above their regulatory action levels in the samples from beneath the gasoline kerosene UST (Tank 5) and one 8,000-gallon gasoline UST (Tank 2). Concentrations of TPH GRO were reported up to 1,952 mg/kg and TPH DRO up to 3,567 mg/kg (GRI, 2018). During the receptor survey conducted by CATLIN in 2021, it was observed that the subject site was undergoing redevelopment to build a new restaurant. Subsequently, all previously installed monitoring and AS/SVE wells appeared to have been destroyed (CATLIN, 2021). Summarized available historical sampling and gauging data from previous consultants are provided in Appendix A. 5.0 REGIONAL AND SITE GEOLOGY AND HYDROGEOLOGY 5.1 SOIL AND BEDROCK As defined in the Geologic Map of North Carolina, published by the North Carolina Department of Natural Resources and Community Development (1985), the site lies within the Piedmont physiographic province. The site is underlain by a metamorphosed mafic rock formation within the Charlotte Belt. According to the 1985 Geologic Map of North Carolina, metagabbro, metadiorite, and mafic plutonic-volcanic complexes crop out in the site area. Monitoring well installation (MW-01, MW-02, and MW-03) boring logs and field observations indicate that the paved asphalt surfaces are immediately underlain with gravel and sand to one foot BLS, then silty clay is present to approximately 15 feet BLS. Clayey silt was encountered from approximately 15 to 32 feet BLS. Silty, highly plastic clay was observed at further depths. 5.2 HYDROGEOLOGY Groundwater – According to the 2002 CSA, there is a relatively flat hydraulic gradient at the site of 0.0033 feet per foot. Slug tests identified a groundwater velocity of 0.186 feet per year (ft./year) in MW-1 and 1.11 ft./year in MW-2, and an average estimated velocity for the entire site of 0.65 ft./year (Shield, 2002). Surface Water – No surface water bodies were identified within 1,500 feet of the site. 6.0 SITE ASSESSMENT INFORMATION Field activities included installing three monitoring wells (MW-01, MW-02, and MW-03), surveying the TOC elevations of the new wells, and collecting three NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 7 December 2022 groundwater samples (one from each new well). Prior to completing any field activities, a site-specific HASP was prepared. Daily safety briefings detailing hazards identified in the task specific job hazard analysis (JHA) sheets were conducted prior to initiating field work. The JHAs are included in Appendix C. 6.1 MONITORING WELL INSTALLATION On November 15, 2022, three borings were advanced to 30 feet BLS for monitoring well construction by a North Carolina Certified Well Contractor, utilizing hollow stem augers and a Diedrich D-50 model drill rig. Cuttings generated during drilling activities were spread in a grassy area behind the building. Split spoon samples were collected in approximately two-foot intervals in general accordance with NCDEQ Guidelines and Environmental Protection Agency (EPA) Region 4 Operating Procedures. Soil samples were field screened using a photoionization detector (PID). The PID concentrations are available in the Well Logs (Appendix D). No samples were collected for laboratory analysis. The wells (MW-01, MW-02, and MW-03) were constructed of new, threaded PVC casing and screen. No glues or cements were used in joining PVC components. New nitrile gloves were worn while handling all well materials. The monitoring wells were constructed with a pointed PVC sediment trap, 15 feet of 2-inch diameter, 0.010-inch slotted PVC well screen and approximately 14.5 feet of 2-inch diameter solid PVC riser to 0.5 feet below the surface. The annular space was filled with coarse sand pack to approximately two feet above the well screen and bentonite chips to within 0.5 feet (MW-01), one foot (MW-02), and 0.6 feet (MW-03) of the ground surface. The bentonite chips were poured from the surface while simultaneously pouring water to facilitate hydration. The wells were finished with a concrete pad and flush-mount steel wellhead and locking well cap. After installation, the monitoring wells were developed by over-pumping. The TOC elevations were measured using traditional methods (Rod and Level). Monitoring well construction information is summarized on Table 3 and a site map with monitoring well location is illustrated on Figure 3. Site photographs are presented in Appendix B. The TOC survey sheet and field boring logs are included in Appendix C. The Well Logs and Well Construction Records were submitted to DEQ on December 2, 2022; copies are provided in Appendix D. 6.2 GAUGING DATA Historically, depth to water in the shallow monitoring wells has ranged from 16.38 feet below top of casing (BTOC) (MW-4) to 24.76 feet BTOC (MW-5). LNAPL has not been previously detected at the site. Groundwater flow direction has been previously illustrated converging towards the west (Shield, 2018). NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 8 December 2022 During the November 2022 event, CATLIN personnel measured depth to water in three monitoring wells using an electronic water interface probe. To prevent cross contamination, the probe and cabling were decontaminated according to manufacturer’s recommendations between use in each well. Gauging data were recorded on an electronic groundwater sampling form using a tablet device. Gauging procedures were performed in general accordance with NCDENR and NCDEQ Guidelines, and EPA Region 4 Operating Procedures. Depth to water ranged from 20.66 feet BTOC (MW-02) to 22.40 feet BTOC (MW-03). LNAPL was not encountered during this monitoring event. Based on groundwater elevations calculated for this monitoring period, groundwater flow direction is towards the south. Monitoring well gauging results for the November 2022 event are presented in Table 3. Groundwater elevations and flow direction are illustrated on Figure 4. Groundwater sampling forms with gauging data are provided in Appendix C. 6.3 SAMPLING DATA Following gauging, the monitoring wells were sampled in general accordance with NCDENR and NCDEQ guidelines and EPA Region 4 Operating Procedures. Prior to obtaining representative groundwater samples, each monitoring well was purged by removing a minimum of three well volumes using a new, disposable bailer. Monitoring well groundwater field parameter data were measured at one-minute intervals using an In-Situ Aqua TROLL 600 Multiparameter Sonde (Aqua TROLL). Conductivity, pH, oxidation reduction potential (ORP), dissolved oxygen, and temperature measurements were recorded utilizing the VuSitu mobile application (version 1.17.12). The Aqua TROLL was decontaminated according to manufacturer’s recommendations and rinsed with distilled water between use at each monitoring well. Groundwater parameter data were transferred from the VuSitu mobile application to the electronic groundwater sampling forms. Copies of the groundwater sampling forms are available in Appendix C. After a minimum of three well volumes were removed (or the well was bailed dry), groundwater samples were collected from the wells and poured into appropriate laboratory-provided, pre-preserved glassware, labeled, and placed on ice in an insulated cooler. A laboratory provided trip blank was maintained in the cooler for the duration of the sampling event. A field blank was collected by pouring water from a new, sealed gallon of distilled water directly into laboratory-provided, pre-preserved glassware. The glassware was labeled and placed on ice in an insulated cooler. The groundwater samples, field blank, and laboratory provided trip blank were submitted to Con-Test Analytical Laboratory (Certification NC 652 and NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 9 December 2022 NC-DW 25703) in East Longmeadow, Massachusetts, under proper chain of custody (COC) protocol and analyzed per SM 6200B (including EDB, MTBE, and DIPE). The laboratory analytical data and COC are included in Appendix E. 6.4 SUMMARY OF GROUNDWATER INFORMATION Historically, samples from former monitoring wells MW-2 and MW-3 have contained the highest petroleum constituent concentrations. During the previous sampling event conducted by Shield (2018), samples from shallow wells MW-1, MW-2, MW-3, MW-4, and deep well T-1 contained VOCs at concentrations above the 2L GWQS. In the sample from MW-2, the concentrations of benzene, 1,2-DCA, and EDB also exceeded their respective GCL. Summarized available historical sampling and gauging data from previous consultants are provided in Appendix A. The latest groundwater sampling laboratory results for SM 6200B analysis are summarized in Table 4. Monitoring well sample summarized analytical results are illustrated on Figure 5. A copy of the complete laboratory report and COC documentation are in Appendix E. Monitoring Wells During the November 2022 sampling event, three monitoring well groundwater samples (MW-01, MW-02, and MW-03) were collected and submitted for analysis via SM 6200B (including EDB, MTBE, and DIPE). Laboratory analysis did not detect any constituents above their respective 2L GWQS in the sample from MW-02. Benzene and EDB were detected above their respective GCL in the MW-01 sample. Analytical results reported the following constituents at concentrations above their 2L GWQS in samples MW-01 and MW-03: • MW-01: benzene (GCL exceedance), n-butylbenzene, EDB (GCL exceedance), ethylbenzene, isopropylbenzene, MTBE, naphthalene, n-propylbenzene, toluene, 1,2,4-trmethylbenzene, 1,3,5-trimethylbenzene, and total xylenes; and • MW-03: benzene. Trip Blank and Field Blank One laboratory provided trip blank and one field blank were submitted for analysis per SM 6200B (including EDB, MTBE, and DIPE). An estimated (“J”- value) concentration of acetone (a common laboratory artifact) was detected in the field blank sample. A low concentration of chloroform was reported in the trip blank sample, but it was well below the 2L GWQS. No other targeted analytes were detected in the samples. NCDEQ; 22332_Former Texaco Food Mart #2 CATLIN Engineers and Scientists CATLIN Project No. 221185 10 December 2022 6.5 EVALUATION OF ASSESSMENT INFORMATION Extent of Contamination Several petroleum constituents were detected above their respective 2L GWQS in sample from MW-01 and benzene was above 2L GWQS in the MW-03 sample. Benzene and EDB were reported above GCL in the MW-01 sample. A summary of monitoring well groundwater sample analytical results are illustrated on Figure 5. The contaminant plume is only horizontally delineated to the south and has not been vertically delineated. Samples from the former deep well T-1 historically had multiple contaminants above 2L GWQS. The highest contaminant concentrations have historically been reported in samples from former monitoring wells MW-2 and MW-3. Contaminant concentrations in the sample obtained from MW-01 (installed in the vicinity of former well MW-2) appear to be stable in relation to the concentrations previously reported (Shield, 2018). Cleanup Levels Achieved Soil – To date, approximately 139.69 tons of impacted soil have been removed from the site. Soil samples obtained in 2018 indicate that TPH GRO concentrations are up to 1,942 mg/kg and TPH DRO up to 3,567 mg/kg (GRI, 2018). Groundwater – Groundwater contaminant concentrations are above GCL at the site. LNAPL – LNAPL was not measured during this monitoring period. LNAPL has not been historically detected at the site. 7.0 CONCLUSIONS AND RECOMMENDATIONS Conclusions Groundwater monitoring findings for this period are summarized below: • Three shallow/Type II monitoring wells (MW-01, MW-02, and MW-03) were installed to 30 feet BLS. • A groundwater sample was collected from each well and analyzed for VOCs per SM 6200B. • Multiple contaminants were reported in the MW-01 sample above their 2L GWQS. • Benzene and EDB were above their GCL in the MW-01 sample. • In the MW-03 sample, only benzene was reported above its 2L GWQS.
