(ATC) submits this Annual Groundwater Monitoring Report documenting the installation of one new monitoring well and sampling of four monitoring wells March 25, 2021 at the Belews Creek Grocery site. The report has been completed based on the November 5, 2020, Task Authorization (TA-1) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work was performed as part of the State Lead Contract # N122019A between the NCDEQ and ATC. As part of TA-01, ATC oversaw the installation of one new monitoring well (MW-05) and conducted a groundwater sampling event for four monitoring wells (MW-01, MW-03, MW-04, MW-05) and one water supply well (WSW-5) and updated the receptor survey. The purpose of installing MW-05 was to evaluate groundwater quality sidegradient of the source area. Monitoring well MW-02 was not sampled due to the presence of light non-aqueous phase liquid (LNAPL). Monitoring well MW-06 was not installed, since the owner of the 6047 Belews Creek Road property with water supply well WSW-2 denied access. The following site conditions are noted based on the recent assessment activities:  Municipal water is available to the site and surrounding area;  Sixteen water supply wells are located within 1,000 feet of the source area, including inactive well WSW-1 and presumed inactive wells WSW-2 located within 250 feet of the source property. One active potable well WSW-5 within 500 feet upgradient of the source area is located along Corvette Lane with no municipal availability;  Volatile Organic Compounds (VOCs) were not detected in the water supply well WSW-5 sample;  No surface water bodies are reportedly located within 500 feet of the source area;  Bedrock was not encountered to a depth of 50 feet below ground surface (ft-bgs), the termination depth of the boring for monitoring well MW-05 (screened from 40 to 50 ft-bgs);  Soils observed during the installation of well MW-05 consisted of sandy clay, silty clay, and clayey silt;  Groundwater levels ranged from 32.78 feet below top of casing (ft-toc) in well MW-04 to 34.16 ft-toc in well MW-05, and groundwater levels were above the well screen in each well;  Groundwater flow is to the east-northeast with a horizontal gradient of approximately 0.0091 feet per foot;  LNAPL (0.19 feet) was detected in well MW-2, where it had not been detected previously; and  Benzene and EDB concentrations in well MW-05 exceed the Gross Contamination Levels (GCLs). Additional VOCs were reported at concentrations exceeding the 15A NCAC 02L .0202 groundwater quality standards (2L Standards) in wells MW-01, MW-03, and MW- 05.  With the additional groundwater level data from monitoring well MW-5, the groundwater flow appears to be more to the east-northeast than indicated previously as being to the east- southeast. The lateral extent of LNAPL and VOC concentrations exceeding the 2L Standards and GCLs is not defined. Groundwater flow appears to be more to the east-northeast than indicated previously as being to the east-southeast. The owner of the property with inactive water supply well WSW-2 located southeast of the former USTs, denied access to his property. Annual Groundwater Monitoring Report April 29, 2021 Belews Creek Grocery, Walkertown NC, NCDEQ Incident #37059 ATC Project #SLC3705901 3 ATC recommends that an AFVR event be performed on MW-02 to remove the LNAPL and MW-05 to reduce VOC concentrations. ATC recommends installation a new monitoring well MW-06 to be located on the site property (northeastern corner near Belews Creek Road), and pursuing an access agreement with the property owner with water supply well WSW-3 for a new downgradient monitoring well MW-07. Additionally, ATC recommends that the inactive on-site water supply well WSW-1 be abandoned. 3.0 SITE DESCRIPTION 3.1 Site Characterization ATC, under contract to the NCDEQ, provided environmental consulting services at the Belews Creek Grocery site, which is located at 6050 Belews Creek Road, Walkertown, Forsyth County, North Carolina. Figure 1 depicts the site on a topographic map. The site is currently occupied by Belews Creek Grocery which operates as a convenience store, with no underground petroleum USTs. Previous consultants identified sixteen water supply wells within 1,000 feet of the source area. The nearest active potable well is WSW-5, which is located approximately 390 feet west-northwest (upgradient) of the source area and along Corvette Lane. Municipal water is not available along Corvette Lane. An additional eight properties within 1,000 feet of the source area are not connected to municipal water and presumed to have active water supply wells. Figure 2 indicates the locations of the water supply wells. On March 26, 2021, ATC personnel updated the locations of the water supply wells previously reported. Water supply wells WSW-6, WSW-8, WSW13, WSW-16, WSW-18, WSW-19 and WSW-22 were not visible from the road and their locations could not be confirmed. Information regarding the water supply wells is provided in Table 1. Municipal water is available in the area, excluding along Corvette Lane to the west and northwest. 3.2 Site Background Pyramid Environmental & Engineering, P.C. (Pyramid) conducted a Limited Soil Assessment at the Belews Creek Grocery site in 2007. Five soil samples were collected of which two samples were reported have concentrations of total petroleum hydrocarbons-gasoline range organics (TPH-GRO) in excess of the NCDENR action level of 10 milligrams per kilogram (mg/kg). The on-site UST system was removed from the ground by Terracon Consultants Inc. (Terracon) on June 29 through July 2, 2015. The UST system consisted of one approximate 4,000-gallon gasoline UST, one approximate 2,000-gallon gasoline UST, and one approximate 1,000-gallon gasoline UST, and an approximate 750-gallon UST of unknown contents. Approximately 558 tons of impacted soil was removed during UST closure activities. The western extent of the excavation was limited by structural concerns associated with the on-site store building. Confirmation soil samples from the side walls and base of the excavation at 16 feet indicated exceedances of Soil- to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs) and Residential MSCCs. Terracon mobilized to the site on October 28 and 29, 2015 to install groundwater monitoring well MW-01 and conduct associated soil and groundwater sampling. Eight petroleum constituents were detected in monitoring well MW-01 above the 2L standards. Terracon submitted a Phase I Limited Site Assessment for the site on February 12, 2016. Based on Terracon’s site reconnaissance and information provided by City of Winston-Salem’s Public Annual Groundwater Monitoring Report April 29, 2021 Belews Creek Grocery, Walkertown NC, NCDEQ Incident #37059 ATC Project #SLC3705901 4 Works Department, 27 water supply wells are located within 1,500 feet of the site. Ten water supply wells located within 1,000 feet of the source area were either identified as potable water sources, or were assumed to be potable water sources. On September 15, 2016 (WSW-12), October 19, 2016 (WSW-5), and October 26, 2016 (WSW- 11), Terracon collected groundwater samples from three water supply wells. Petroleum constituents were not detected above laboratory detection limits in the samples collected from these water supply wells. Terracon submitted a Subsequent Monitoring Report dated November 7, 2017. Per this report, Terracon installed MW-2, MW-3, and MW-4 in July and August 2017. Groundwater samples were collected from the monitoring wells and from water supply wells WSW-4 and WSW-5. Ten petroleum constituents were detected at concentrations above the 2L standards in one or more monitoring wells, with the highest concentrations in monitoring well MW-02 east of the former UST system. VOCs were not reported in the water supply well samples. Per the Subsequent Monitoring Report dated July 23, 2018, by Terracon, groundwater samples were collected on February 27, 2018, from four monitoring wells (MW-01 through MW-04) and two water supply wells (WSW-4 and WSW-5). Ten petroleum constituents were detected at concentrations above the 2L standards in one or more monitoring wells, with the highest concentrations in monitoring well MW-02 east of the former UST system. Terracon submitted a Subsequent Monitoring Report dated December 13, 2019. Monitoring wells MW-1 through MW-4 and water supply well WSW-5 were sampled on July 31, 2019. Ms. Lillian Lynch, the owner of on-site inactive WSW-1 agreed to allow NCDEQ to abandon WSW- 1. VOCs were not reported in the water supply well sample WSW-5, except for chloroform at an estimated concentration of 5.7 micrograms per liter (µg/L). Twelve petroleum constituents were detected at concentrations above the established 2L standards in one or more monitoring wells, with the highest concentrations in monitoring well MW-02 east of the former UST system. 3.3 Site and Regional Geology According to the Geologic Map of North Carolina complied by the North Carolina Geologic Survey in 1985, the site lies in the Charlotte Belt of the Piedmont Physiographic Province of North Carolina. The rock type in this area has been identified as an inequigranular biotite gneiss and schist interlayered and gradational with quartz feldspar gneiss interlayered with calc-silicate rock, sillimanite mica-schists, and amphibolite. The shallow subsurface in most areas of the Piedmont contains residual soil overburden, including structure-free residuum, saprolite, and partially weathered rock (PWR) that derive from in-place weathering of the crystalline bedrock. Occasional areas containing recent deposits of alluvium in the uppermost subsurface are found near streams and rivers. Saprolite and PWR typically contain some relict structures from the original rock material. Depth to rock ranges from ground surface at occasional outcrops to depths of greater than 100 feet in areas of easily weathered rock. The boring log from the installation of MW-05 indicates that the site subsurface generally consists of brown and red-brown sandy clay, silty clay, and clayey silt observed to a depth of 50 ft-bgs. Bedrock was not encountered during installation of MW-05 to a depth of 50 ft-bgs. Annual Groundwater Monitoring Report April 29, 2021 Belews Creek Grocery, Walkertown NC, NCDEQ Incident #37059 ATC Project #SLC3705901 5 3.4 Site and Regional Hydrogeology The shallow aquifer occurrence varies in depth from ground surface at springs, creeks, and rivers to as deep as 50 feet or more beneath upland surfaces in some parts of the Piedmont. Water in the alluvium or unconsolidated residual material, including saprolite and PWR, usually behaves as an unconfined aquifer, or water table, and will yield water with head elevation equivalent to the first elevation where water was encountered. Permeability varies with lithology and is typically relatively low in residual soils, with higher permeability in saprolite or PWR due to relict rock texture and variable susceptibility to weathering exhibited by different minerals in the rock. Groundwater flow in residual soils or alluvium is usually in rough concurrence with local topographic conditions and is toward local drainage features. Based on gauging of the monitoring well network, groundwater flow across the site is generally toward the east. Depth to groundwater in the overburden aquifer at the site has historically ranged from approximately 32 to 44 ft-bgs. The bedrock fractures or other planar features of the Piedmont Physiographic Province generally constitute the bedrock aquifer, with the surrounding rock material being effectively impermeable. Along with fractures, contacts between rock bodies probably constitute zones of significant groundwater occurrence in the bedrock. The surrounding material and overlying residuum tend to make the bedrock aquifer a semi-confined aquifer. That is, the overlying water and soil weight normally results in pressure that causes water in a borehole, which intersects a fracture or other feature, to rise above the elevation of the fracture or feature. Modern water supply wells in the Piedmont are typically installed with a surface casing through the unconsolidated material, and an open borehole into the bedrock where water-bearing fractures are encountered. 4.0 GROUNDWATER CHARACTERIZATION 4.1 Monitoring Well Installation On March 25, 2021, ATC oversaw the installation of one Type II monitoring well (MW-05) located northeast of the source area and on the same property with the former UST system. The well was installed to evaluate the sidegradient extent of VOCs in the groundwater. The proposed well MW- 06 was not installed since the owner of the 6047 Belews Creek Road property denied access. The location of the well MW-05 is indicated on Figure 3. Environmental Drilling and Probing Services (EDPS, Inc.), North Carolina Certified Well Contractors, drilled the boring and installed the monitoring well. The boring for MW-05 was drilled to a depth of 50 ft-bgs using a hollow-stem auger drill rig with 4.25-inch diameter OD auger. During well installation soils were observed to be moist at a depth of approximately 40 feet, and saturated between 45-50 feet bgs. The well was constructed of 2-inch diameter, Schedule 40 PVC screen (0.010-inch slots) from 40 to 50 ft-bgs and Schedule 40 PVC riser from 40 ft-bgs to ground surface. Filtered sand was installed within the annulus to approximately 2 feet above the top of the screen, with approximately 2 feet of bentonite above the sand pack, and grout to near land surface. The well was completed within a 2-foot by 2-foot concrete pack and 8-inch diameter well vault, and capped with a lockable water- tight cap. The well was developed after installation. During the soil boring for well MW-05, soil samples were collected to evaluate the lithology and to screen the soils for relative volatile organics using a photoionization detector (PID). The soil is generally described as red-brown sandy clay to silty clay to a depth of 10 ft-bgs, and clayey silt to a depth of 50 ft-bgs. PID readings were less than 1 part per million (ppm) to a depth of 40 ft-bgs (with no apparent petroleum odors), 20 ppm between 40 and 45 ft-bgs (with apparent slight petroleum odors), and 1,610 ppm between 45 and 50 ft-bgs (with apparent strong petroleum odors). Table 2 contains monitoring well construction details. Photographs of the site conditions are also presented in Appendix A. 4.2 Groundwater Levels On March 25, 2021, monitoring wells MW-01, MW-02, MW-03, MW-04 and MW-05 were gauged for liquid levels from the well top of casings. LNAPL (0.19 feet) was detected in monitoring well MW-02 for the first time. The depth to water in MW-5 was 34.16 ft-toc (approximately 4 feet above the top of well screen) at the time the well was gauged. The total concentration of benzene, ethylbenzene, toluene, and xylenes (66,900 to 96,900 µg/L) in prior sampling events indicated the likelihood of residual LNAPL in MW-2. The depth to water ranged from 32.78 ft-toc in MW-4 to 34.16 ft-toc in MW-5. Figure 3 depicts a site map with well locations. Depth to groundwater elevation data and historical water level data are summarized in Table 3. Groundwater flow was estimated to be toward the east with a horizontal gradient of approximately 0.0091 feet per foot. With the additional groundwater level data from well MW-5, the groundwater flow direction appears to be more to the east-northeast than previously indicated, with well MW-5 in the downgradient direction from the northern portions of the former USTs. Field notes from the March 25, 2021 groundwater monitoring event are presented in Appendix A. Figure 4 illustrates the groundwater surface contours and groundwater flow direction. 4.3 Groundwater Quality On March 25, 2021, monitoring wells MW-01, MW-03, MW-04, and MW-05, and water supply well WSW-5 sampled. During sampling activities, Mr. Herb Mabe was contacted and granted access to sample WSW-5 located at 7450 Corvette Lane. A copy of the signed access agreement is included in Appendix B. During sampling activities, the monitoring wells were purged of three standing well-volumes, or purged until dry and allowed to recharge, prior to sampling, and the water supply well was purged for approximately 15 minutes prior to sampling. Upon completion of purging activities, water quality field parameters were measured in each monitoring well. Field parameters are included in Table 4. Dissolved oxygen (DO) concentrations were generally low (less than 3.5 milligrams per liter or mg/L) in samples from wells MW-01, MW-03, and MW-05 with elevated concentrations of VOCs, while the DO concentration in well MW-04 with no VOCs reported above laboratory method detection limits was 7.1 mg/L. Water samples were collected in laboratory-supplied, preserved containers; entered into chain-of- custody control; placed on ice; and delivered Pace Analytical Laboratory in Huntersville, NC. The groundwater samples were analyzed by EPA Method 6200B for VOCs including ethanol and oxygenates. The analytical report is included as Appendix C. Groundwater analytical results from the March 25, 2021 monitoring event are summarized in Table 5. Historical groundwater analytical results are summarized in Table 6. Figure 5 depicts dissolved- phase compounds detected above 2L Standards in sampled monitoring wells. Groundwater analytical results indicated the following:  The highest VOC concentrations were reported in well MW-5, with benzene and EDB exceeding the GCLs, and the total concentration of benzene, ethylbenzene, toluene, and xylenes (65,240 µg/L) indicating the possible presence of residual LNAPL. VOCs were not reported in well MW-04 at concentrations above the laboratory method detection limits.  Oxygenates methyl tert-butyl ether, tert-amylmethyl ether and tert-butyl alcohol were reported in wells MW-01, MW-03, and MW-05 (tert-butyl alcohol reported as less than 5,360 µg/L in well MW-05).  VOCs were not detected in the sample from water supply well WSW-5. The extent of VOCs exceeding the 2L Standards and GCLs is not defined to the east and northeast of the source area, with LNAPL in well MW-2 and exceedances of the 2L Standards and GCLs in well MW-5. As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the laboratory report was conducted. During the review, it was found that the samples were received from the laboratory in acceptable condition and properly preserved. The samples were analyzed within correct holding times and reported within the laboratory report, and the chain-of-custody was properly completed by all parties. The laboratory QC report shows that an internal blank was analyzed for EPA Method 6200B. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for EPA Method 6200B. A field blank was also analyzed during the groundwater monitoring event and VOCs were not were detected in the field blank. Refer to the laboratory report in Appendix C for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the conclusions presented in this report. 5.0 CONCLUSIONS AND RECOMMENDATIONS On behalf of the Trust Fund Branch of the UST Section, ATC has completed a Groundwater Monitoring Report documenting monitoring well installation and sampling activities for the Belews Creek Grocery site, located at 6050 Belews Creek Road, Walkertown, Forsyth County, North Carolina. Table 7 presents the Conceptual Site Model for the site, based on the assessment activities conducted to date. The following site conditions are noted based on the recent and historical assessment activities:  Municipal water is available to the site and surrounding area;  Sixteen water supply wells are located within 1,000 feet of the source area, including inactive well WSW-1 and presumed inactive well WSW-2 located within 250 feet of the source property.
