Dear Mr. Berger:
S&ME, Inc. (S&ME) submits this letter report for the Moore’s Store incident site located
near Reidsville in Rockingham County, North Carolina. S&ME performed these services
in accordance with S&ME proposal number 43-1500641 dated July 8, 2015, and under
NCDENR’s Contract No. N14005i, Task Authorization No. 01.
1.0 WATER SUPPLY WELL SAMPLING
S&ME was authorized to collect water samples from three water supply wells, including
WSW-2, WSW-4 and WSW-6, each located within 500 feet of the incident site, as
identified on the attached Table 1 and as shown on Figure 1. S&ME received written
authorization to collect water samples from water supply wells WSW-2 and WSW-4.
Copies of the corresponding Request for Permission to Sample Water Supply Well forms
are provided in Appendix I.
S&ME had two telephone conversations with the owner and mailed two copies of the
Request for Permission to Sample Water Supply Well form to the owner of the property
located at 5901 NC Highway 65 (water supply well WSW-6). The owner provided
verbal permission to sample WSW-6, but a signed form was not returned, and no one
appeared to be at home during our site visit on September 28, 2015.
Water supply wells WSW-2 and WSW-4 serving 5897 NC Highway 65 (subject site) and
a residence located at 5912 NC Highway 65, respectively, were sampled on September
28, 2015. Each well was purged for 15 minutes prior to sampling. The groundwater
samples were placed in laboratory-supplied containers, stored in a cooler with ice, and
delivered by dedicated courier under chain-of-custody procedures to Pace Analytical for
laboratory analysis. The water supply well samples were analyzed for volatile organic
compounds (VOCs) by EPA Method 6200B and for Ethylene dibromide by EPA Method
504.1. Copies of the water supply well sampling forms are included in Appendix II.

Water supply well WSW-4 was previously identified with the address of 5958 NC
Highway 65. S&ME was informed by the property owner, Iris Sharp, that 5958 NC 65 is
the address for the property listed on the corresponding tax records, but that the address
for the residence served by water supply well WSW-4 is 5912 NC Highway 65.

2.0 ANALYTICAL RESULTS
The water supply well sampling results are summarized in Table 2. No compounds were
detected in the samples collected from the water supply wells WSW-2 and WSW-4. A
copy of the laboratory report is included in Appendix II. The North Carolina
Department of Environmental Quality (NCDEQ) provided analytical results to the well
owners and has offered to connect well users in the area to the municipal water system.
3.0 WATER CONNECTION BIDS
S&ME submitted a Request for Quotation dated September 14, 2015, to three licensed
plumbing contractors related to connecting 5871 NC Highway 65 (Frances Myers;
WSW-6) to the available municipal water service. A copy of the request is provided in
Appendix III. S&ME received a response from each request. Based on this response
and on fee information provided by Rockingham County Environmental & Engineering
Services, S&ME will submit a Task Authorization to install the water line connection and
to disconnect the residence from water supply well WSW-6. We understand that WSW-6
may continue to be used for irrigation purposes. Copies of the submitted quotes are
included in Appendix III.
4.0 WELL ABANDONMENT AGREEMENT REQUESTS
S&ME mailed letters, dated September 18, 2015, to the owners of 5881 NC Highway 65
(Kenneth Capps; WSW-1) and 5894 NC Highway 65 (Clark/Nehring; WSW-3) to inquire
about their interest in abandoning the inactive water supply wells on their properties.
S&ME received no response from our mailed inquiries or phone calls to Kenneth Capps.
S&ME understood that Mr. Capps desired to abandon WSW-1 during our September 28,
2015, site visit; however, Mr. Capps informed S&ME with a phone message (September
29, 2015) that he does not desire to abandon his inactive water supply well (WSW-1). A
copy of the phone message was forwarded to Herb Berger of the NCDEQ on September
29, 2015.
S&ME visited 5894 NC Highway 65 on September 28, 2015, and left an additional copy
of our inquiry letter at the front door. Occupants in the home at the time of the visit did
not answer the door. S&ME was unable to identify a phone number for the owners at
that time. Subsequent to the site visit, Mr. Clark contacted S&ME by telephone, and we
understood that Mr. Clark also communicated with Herb Berger of the NCDEQ. We
understand that the NCDEQ offered to abandon the inactive water supply well at 5894
NC 65 and that Mr. Clark has not authorized the NCDEQ to abandon the well. We also
understand that Mr. Clark is considering reconnecting the well for residential use and that
the NCDEQ recommended that Mr. Clark not reconnect the well.
