Based upon the historical reports, LNAPL has not been reported in the wells.  The maximum concentrations of dissolved groundwater contaminants reported in the monitor wells (MW- 1, MW-2, MW-3 and TW-1) at concentrations greater than the 15A NCAC 2L Groundwater Quality Standards (2L Standards) during this sampling event are summarized below and in Table 3. According to previous reports and information obtained during this sampling event, the following potential receptors were noted within 1,000 feet of the site. See Table 5 for additional information regarding these receptors. No target constituents were reported in the samples collected at concentrations exceeding the laboratory reporting limits. See Table 4 for complete results.  A Phase I Limited Site Assessment (LSA) was conducted at the site in August of 2003 by Best Geological and Environmental Consulting Inc., which included installing monitoring well MW-1. A concentration of methylene chloride greater than the soil-to-water Maximum Soil Contaminant Concentration (MSCC) was reported in one of the two soil samples collected during the installation of MW-1. However, methylene chloride is a common laboratory contaminant and was not believed to be present as a result of the release. No target constituents were reported in the soil samples at concentrations greater than the corresponding soil-to-groundwater MSCCs. Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 3  To achieve incident closure, the following options are presented:  Reduce dissolved constituents to concentrations less than the 2L Standards by remedial measures or by Natural Attenuation, and assess soil impacts relative to the soil-to-groundwater MSCCs  Pursue connection of municipal water to the properties with active, at-risk, potable water supply wells located within 1,000 feet, disconnect the wells from the structures, and file a Notice of Residual Petroleum with groundwater and soil restrictions. 3.0 Scope of Services 3.1 Contract Information The scope of services for this report was performed in general accordance with S&ME’s proposal number 431800711, dated August 29, 2018, and the scope of services was authorized by the North Carolina Department of Environmental Quality (NCDEQ’s) Task Authorization TA #01, dated September 13, 2018, and within contract number N17002. 3.2 Approved Scope of Services S&ME’s approved Scope of Services for this Task Authorization included:  Update the receptor survey;  Collect groundwater samples from four existing monitor wells (MW-1, MW-3, MW-3, TW-1);  Measure depth to water in monitor wells MW-2, MW-5 and MW-6;  Collect groundwater samples from four water supply wells (WSW-2, WSW-3, WSW-4, WSW-6); and,  Prepare and submit this report. These services were completed as proposed, with the following exceptions:  The residence at WSW-3 was confirmed as being connected to the municipal water service and the water supply well is inactive; therefore, WSW-3 was not sampled;  Permission was not obtained to sample WSW-6; therefore WSW-6 was not sampled;  S&ME was unable to locate monitor well MW-6; therefore, the depth to water was not measured in monitor well MW-6. 4.0 Receptor Survey Update S&ME mailed Well Survey Questionnaire forms to the owners of seven properties located within 1,000 feet of the source area. No survey forms were returned undelivered. Completed survey forms were returned by the owners of two properties. Copies of the returned forms are provided in Appendix II, and provided information is summarized on Table 1. Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 4 On November 15, 2018, S&ME personnel performed a reconnaissance of the area located within approximately 1,000 feet of the source to confirm and identify water supply wells. Apparent well observations are summarized on Table 5 and are shown on Figure 5. On December 6, 2018, S&ME interviewed Ms. Donna Claybrook of Rockingham County Engineering by phone. Ms. Claybrook stated that 110 Ralph E. Road has an active water account. Ms. Claybrook also confirmed that the properties served by WSW-9, WSW-11, WSW-12 and WSW-13 do not have active water accounts. The Bethany Civic Center is a public meeting place located approximately 500 feet from the source area. The NCDEQ connected 5871 NC 65 (Myers) to municipal water service in 2015, and water supply well WSW-5 remains as an irrigation well located more than 250 from the source area. S&ME attempted to obtain an access agreement to abandon water supply well WSW-1 located at 5881 NC 65 (Capps) in 2015, but access was denied by the property owner. The new owner and resident of 5894 NC 65, Ian Boles, confirmed during an interview performed during a site visit on November 17, 2018, that the residence is served by the municipal water service and water supply well WSW-3 is inactive; however, Mr. Boles was non-committal at that time to the request for an access agreement to abandon WSW-3. Based on information provided on returned well survey forms, interviews and on reconnaissance observations, up to seven active, or suspected active, potable water supply wells are located within 1,000 feet of the source area, including three active wells located within 250 feet of the source area. 5.0 Current/Historical Assessment Information The Moore’s Exxon site is located at 5891 NC Highway 65, Reidsville, North Carolina (Figures 1 and 2). A UST system operated at the site from the 1940s to the 1960s, and these USTs were reportedly closed in place near the dispenser island of the existing UST system. Three 2,000-gallon gasoline USTs installed in 1966 were closed by removal in 1990. No evidence of a release was reported during the tank closure activities conducted in 1990. Two 6,000-gallon gasoline USTs were installed at the site in 1990 and are currently in service (Table 1). An aboveground heating oil storage tank was observed behind the store building. A release at the site was suspected in May of 2003 after the Rockingham County Health Department sampled water supply well WSW-1 located on the Capps’s property, which adjoins the site to the east. Concentrations of benzene, toluene, ethylbenzene, xylenes, isopropyl ether and 1,2-dichloroethane were reported in the sample collected from the Capps well. The concentrations of benzene and 1,2-dichloroethane reported in the sample exceeded North Carolina’s 15A NCAC 2L groundwater quality standard (2L Standard). The results were confirmed when the well was re-sampled in June 2003. An Initial Abatement Measures/Site Check performed at the site in June of 2003 confirmed that a release had occurred at the site. Total petroleum hydrocarbons (TPH) in the gasoline range exceeded the regulatory action levels in a soil sample collected adjacent to the pump island. Six groundwater monitor wells are located on the site and one monitor well is located on the adjacent property to the south. Dissolved constituent impacts exceeding the 2L Standards have been reported historically in monitor wells MW-1, MW-3, MW-4 and MW-5 Photographs of the site are provided in Appendix I. Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 5 5.1 Current Liquid Levels On November 19, 2018, S&ME personnel were onsite to measure the depth to groundwater in the monitor wells MW-1 through MW-5 and TW-1. The depth to groundwater ranged from 36.15 feet below top of casing (ft.-toc) in MW-2 to 42.31 ft.-toc in MW-4. The depth to groundwater measurements and groundwater elevations are provided in Table 2 and a groundwater contour map based on the November 19, 2018 measurements is depicted in Figure 3. 5.2 Groundwater Monitoring Event After collecting depth to groundwater measurements on November 19, 2018, S&ME personnel purged and sampled monitor wells MW-1, MW-2, MW-3 and TW-1 using new, disposable, polyethylene, bailers. Each well was purged approximately three well casing volumes. Field parameters pH, specific conductivity, and temperature were measured after the third casing volume and recorded on the field sample forms records provided in Appendix III. Prior to the sampling event, S&ME mailed Request for Permission to Sample Water Supply Well forms to the property owners of WSW-2, WSW-4 and WSW-6 to obtain written permission to sample the wells. S&ME also mailed a Site Access Agreement to the new owner of the property served by water supply well WSW-3. Copies of the forms are provided in Appendix III. Permission was obtained to sample WSW-2 and WSW-4. S&ME personnel collected a water sample from water supply wells WSW-2 and WSW-4 on November 19, 2018, and November 17, 2018, respectively. Field sample summary records are provided in Appendix III. During the sampling event, a Field Blank was prepared using laboratory supplied deionized water. Groundwater samples and the Field Blank sample were collected directly into laboratory supplied containers, placed in a cooler on ice and shipped under standard chain-of-custody record to Con-Test Analytical Laboratory (Con-Test) for analysis of volatile organic compounds (VOCs) by EPA Method 6200B +MTBE, IPE and EDB. Groundwater samples were also submitted for analysis of Ethylene dibromide (EDB, or 1-2-Dibromoethane) by EPA Method 504.1. 5.3 Summary of Groundwater Analytical Results The laboratory analytical results of the groundwater samples collected from monitor wells MW-1, MW-2, MW-3 and TW-1 indicated that the following target constituents were reported at concentrations exceeding the corresponding 2L Standards.  Naphthalene in MW-3  EDB in MW-3 One additional target constituent was reported in monitor well MW-1 at an estimated concentration exceeding the laboratory reporting limit, but less than the respective 2L Standard. No target constituents were reported in monitor well samples MW-2, MW-4, or the Field Blank sample, at concentrations exceeding their laboratory detection limits. A summary of the groundwater analytical results is presented in Table 3 and shown in Figure 4. A copy of the laboratory analytical report is provided in Appendix IV. Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 6 5.4 Summary of Water Supply Well Analytical Results No target constituents were reported in the water samples collected from water supply wells WSW-2 and WSW-4. On November 30, 2018, upon receipt of the laboratory analytical report, S&ME contacted Mr. Herbert Berger of the NCDEQ by email to provide the results of the water supply well sampling. A summary of the groundwater analytical results for the water supply wells is presented in Table 4 and shown in Figure 5. A copy of the laboratory analytical report is provided in Appendix IV. 5.5 Regional and Site Geology and Hydrogeology 5.5.1 Soil and Bedrock Soils underlying the site are characterized as Cecil-Pacolet-Appling, with a loamy surface layer and a loamy and clayey subsoil. The 2003 Boring Logs for site check soil probes and soil boring MW-1 described subsurface soils as sandy clayey silt (fill), underlain by sandy silt (residuum). According to the 1985 USGS Geologic Map of North Carolina, the site is underlain by the Milton Belt of the Piedmont Physiographic Province. Bedrock in this area is characterized as inequigranular Biotite Gneiss and Schist. Bedrock was reportedly encountered by soil boring TW-1 at a depth of 58 feet below ground surface (ft.bgs). 5.5.2 Hydrogeology S&ME calculated the groundwater elevations for the monitor wells gauged on November 19, 2018 using the depth-to-water measurements and the top of casing elevations. The groundwater elevations are provided in Table 2 and a groundwater contour map based on the November 19, 2018 measurements is depicted in Figure 2. The indicated groundwater direction is generally to the northwest with slightly divergent flow conditions. Since the most recent sampling event performed in 2014, the groundwater water table has increased in elevation between approximately 1.2 feet to 1.6 feet. 5.6 Evaluation of Soil, Groundwater, and LNAPL Assessment Information 5.6.1 Current Extent and Severity of Contamination According to the Initial Abatement Measure/Site Check Report prepared by Best Geological and Environmental Consultants, dated July 8, 2003, soil sample P-3 collected from a depth of 9 ft. bgs beside the dispenser island contained Total Petroleum Hydrocarbons-Gasoline Range Organics at a concentration of 960 milligrams per kilograms (mg/kg). Analytical results of soil sample P-3 by the risk-based methods reported carbon-chain fraction concentrations less than their respective soil-to-groundwater MSCCs, and VOCs were not detected. Monitor wells MW-1 and MW-3, which are located in apparent downgradient and upgradient positions, respectively, relative to the current and former UST basins, have historically contained the highest dissolved concentrations of constituents. Constituent concentrations have not exceeded the GCLs and LNAPL has not been reported. Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 7 Contaminant concentrations were also reported previously at concentrations greater than the 2L standards in monitor well MW-5; however, MW-5 has been dry and was not sampled during multiple monitoring events. MW- 3 was not dry during the current monitoring event; however, groundwater occurrence appeared to be only 6 inches or less above the lower most section of slotted screen in the well. Dissolved contaminant concentrations in monitor wells MW-1 and MW-3 appear to be decreasing. Target constituents were not reported in monitor wells MW-1 and MW-2 at concentrations greater than the respective 2L Standards during the current monitoring event. The dissolved constituent plume appears to be defined vertically by the current analytical results representing monitor well TW-1. The horizontal extent of the dissolved constituent plume has been defined historically to the west, north and south by monitor wells MW-2, MW-4 and MW-6, respectively. The dissolved plume remains undefined by monitor well MW-5 located east of the source area. Previously impacted water supply well WSW-1 is located approximately 40 feet east of monitor well MW-5; however, the residence served by WSW-1 is connected to the municipal water source and WSW-1 is inactive. No detectable target constituents were reported in the water supply well sample collected from water supply wells WSW-2 and WSW-4. No target constituents have been reported at concentrations greater than their respective 2L Standards in the water supply wells sampled during previous monitoring events, with the following exceptions.  WSW-5; however, the residence served by WSW-5 is connected to the municipal water source and WSW- 5 is used as an irrigation well (located greater than 250 feet from the source area).  WSW-8, however, WSW-8 is located more than 1,000 feet from the source area, and the impacts may be related to another source. 5.6.2 Cleanup Levels Achieved  Soil (Soil to Groundwater) No  Groundwater (2L standard concentrations) No  LNAPL-LNAPL has not been measured at the site since 1997 Yes 6.0 Conclusion and Recommendations Based on the current and historical assessment information for the site, the following conclusions are presented:  The UST Incident meets the following conditions for a High Risk classification: A. Active potable water supply wells are located within 1,000 feet of the source area. B. VOCs were reported in monitor well MW-3 at concentration greater than the corresponding 2L Standards. Based on the above, the following recommendations are presented: Receptor Survey Update and Groundwater Monitoring Report Moore’s Exxon, Incident # 30199 5891 NC Highway 65, Reidsville, Rockingham County, NC S&ME Project No. 4305-18-201 January 15, 2019 8  Continue periodic monitoring of monitor wells, including MW-5 following recharge of the shallow aquifer system.  Continue periodic monitoring of water supply wells. Water supply wells likely to be most at risk from the release include, but may not be limited to, WSW-2, WSW-4, WSW-6, WSW-7, WSW-9 and WSW-11.  Reduce dissolved constituents in monitor wells MW-3 and MW-5 to levels less than the 2L Standards, by remedial measures or by natural attenuation. Analyze groundwater samples collected from selected monitor wells for natural attenuation parameters to assess if conditions are favorable for natural attenuation.  Collect soil samples representing the source area for analyses by the risk-based methods.
