Facility Permit 51-03 is a combination of facility permits 51-01 and 51-02. Permit to Construct Johnston County MSW Landfill, Phase 6 issued May 8, 2020. Permit to Operate 5103-MSWLF Phase 4A and 5103-CDLF Area 2 Cells 1 and 2 issued May 8, 2020. Permit for Closure for MSW Phases 1 through 4 and C&D Area 1 issued May 8, 2020. Life of Site Permit for 5103-MSWLF and 5103-CDLF issued May 8, 2020 and will expire August 8, 2057. Permit to Construct Johnston County MSW Landfill Phase 6 issued November 30, 2021. Permit to Operate 5103-MSWLF Phase 4A, 5, and 6A and C&D Area 2, Cells 1 and 2 issued November 30, 2021. Permit for Closure MSW Phases 1 through 4 and C&D Area 1 issued November 30, 2021. Pagel of 4 NORTH CARnLINAD_E Q�� OepaNnent W Environnnnbl puel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS Facility Inspection 1. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts waste Monday through Saturday from 8:00 am until 4:45 pm and closed on Sunday. 4. The permit, operations plan, waste screening, and training records were not reviewed during this inspection. 5. The perimeter roadways around the active MSWLF and closed CDLF are being properly maintained. 6. Permanent edge of waste (EOW) markers were observed around the active portion of the MSWLF and closed CDLF. Yard Waste 7. The yard waste was windrowed and actively being ground during the inspection. 8. No areas of standing water were observed within the yard waste area. Phase 4A MSWLF 9. The liner closure has begun on Phases 1 through 4A. Monthly meetings have been scheduled to provide status updates during the process. 10. Mr. Beasley stated that during the installation of the liner, a leachate seep was identified. The seep was investigated and repaired. Continue to monitor for and repair leachate seeps as needed. Phase 5 MSWLF 11. Erosion rills were identified along the western side slope of Phase 5. It was recommended that the rills be repaired to prevent further erosion. 12. During this inspection, methane collection wells were being installed. Mr. Beasley stated that the contractor excavates for the collection line, installs the line, then covers the line and waste prior to the end of the operating day. The area will be graded and seeded following the installation of the methane collection system. Phase 6 MSWLF 13. The working face of Phase 6 was small and compact. The working face was actively being covered during this inspection. 14. Minimal windblown waste from the working face was observed at the base of the western side slope of Phase 5. Workers pick up the windblown waste throughout the operating day to ensure that the windblown is picked up by the end of the operating day.
