(Dame Of Waste Hauler) hereinafter referred to as Hauler, to discharge septic tank sludge or waste from portable toilets into the wastewater treatment facilities at the designated point, located at the Danville Northside Wastewater Treatment Plant (hereinafter referred to as POTW). This discharge is subject to the following limitations and conditions: 1. This permit shall be valid for a period of two (2) years from the date of issuance, provided that such permit shall be subject to revocation by the City for any violation of any of the provisions of this permit or other applicable provisions of Federal, State or local law. 2. This permit shall be limited to the discharge of septic tank wastes, which contain no industrial waste. Portable toilet wastes are also acceptable, The City may refuse to accept any truckload of waste when it appears that the waste could interfere with the effective operation of the POTW. 3. The Hauler may NOT dispose of wastewater composed of waste fats, oils and grease (FOG) from commercial grease traps. This includes grease from restaurants, institutional food service providers (ex: school cafeterias) and automotive service providers. 4. The Hauler must dispose of all wastes only at the designated point at the POTW and must keep this area properly maintained. Haulers may dispose of waste Monday through Friday during the hours of 7.00 am to 7:00 pm and on Saturday during the hours of 10:00 am to 2:00 pm. In addition, Haulers may only discharge wastewater between the hours of 10:00 am to 2:00 pm on the following holidays: New Year's Day, Martin Luther King, Jr. Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day (Thursday and Friday), and Christmas flay. Any exceptions to these times are at the discretion of the Project Manager of the company contracted by the City of Danville to operate the POTW. 5. The Hauler must provide the Operator on duty at the POTW an accurate and properly prepared manifest form showing the origin of the septic tank waste. The Operator on duty must observe the Hauler collect a sample of the wastewater for pH analysis. #f the PH is less than 6.0 S.U. or more than 12.0 S.U., the Operator will refuse to allow the disposal. it is the responsibility of the Hauler to adjust the wastewater contained in their truck to ensure that the pH is within the approved range. 6. Discharge of any waste other than septic tank waste, is forbidden without special permission from the City and then only after laboratory analysis of the waste is performed prior to discharge. The Hauser shall pay the cost of the laboratory analysis. Spot checks of septic tank waste delivered by the Hauler to the POTW shall be made at the discretion of the City and laboratory analysis performed. Should any waste be found in violation of any of the provisions of this permit or any Section of Chapter 34 of the Code of the City of Danville or any other provisions of Federal, State or local law, the following civil penalties may be assessed the Hauler: 15i Offense $100.00 Civil Penalty 2"d Offense $250.00 Civil Penalty 3" Offense Revocation of Discharge Permit S. A Hauler may appeal the revocation of his permit to the City's Division Director of Water and Wastewater Treatment. Following a hearing conducted by the Director considering the facts and circumstances relative to the violations, the Director may permit the Hauler to continue to dump waste at the POTW; however, ail such septic waste will be checked and analyzed until City is satisfied that no further violations will occur. The Hauler shall pay the cost of such laboratory analysis. 9. Disposal Rates Rates are revised periodically. Therefore, the rates charged when the permit is issued may change at any time before the permit expires. The Hauler will be required to pay the new rates as they become effective. The following waste disposal rates become effective on the date of issuance of the Permit. There is a minimum charge of $17.00 per truckload up to 500 gallons of septic waste. Haulers disposing a load of more than 500 gallons shall pay $17,00 for the first 500 gallons and $4.00 per 100 gallons over 500 gallons. 10. NO TOXIC OR HAZARDOUS WASTE WILL BE ACCEPTED. No person shall discharge or cause to be discharged into any sewer any of the following: (1) Any storm water, surface water, ground water, roof runoff, subsurface drainage, or non -contact industrial cooling waters; except as specifically approved by the PSA. (2) Any water or wastewater which contains more than 150 ppm, by weight, of fat, oil, grease; except as specifically approved by the PSA- (3) Any gasoline, benzene, naphtha, fuel oil, or other flammable or explosive liquid, solid, or gas; including pollutants which create a fire or explosion hazard, including but not limited to wastewaters with a closed cup flashpoint of less than 140 degrees Fahrenheit using the test methods specified in 40 C.F.R.261.21. (4) Any garbage resulting from the preparation, cooking, or dispensing of food which has not been shredded so that it will flow freely through standard building sewers and sewers of the PSA. (5) Any ashes, cinders, sand, mud, straw, shavings, metal, glass, rags, feathers, tar, plastics, adhesives, or any other solid or viscous, glue like substance capable of causing obstruction to the flow in sewers or interference with the proper operation of the PSA's sewage works. (6) Any wastes or other substances having a pH value lower than 5-0 or higher than 9.0 or having any other corrosive property capable of causing damage or hazard to structures, equipment, or personnel of the PSA. Base substances (pH above 9.0) may be approved at the discretion of the PSA. (7) Any substance or wastes containing a toxic or poisonous element in sufficient quantity to injure or interfere with any sewage treatment plant process, as pose a hazard to humans or animals, or create a hazard in the receiving waters of the PSA sewage treatment plant. (R) Any pollutant, including BQD or other oxygen -demanding substance, released at a flow rate and/or concentration which will cause interference with the proper operation of the PSA's sewage works. (9) Any waste containing suspended solids or BOD in excess of 300 ppm. Wastes in excess of 300 ppm, but not exceeding 1000 ppm, may be accepted by the PSA pursuant to a specific agreement between the permittee and the PSA. Such agreement shall provide for the payment of a surcharge set by the PSA at it's sole discretion to recover the costs of treatment of such additional levels of pollutants. (10) Any noxious odorous gas or substance capable of creating a public nuisance. (l 1 ! (13) Any wastewater having a color value greater than 300 ADMI units. The PSA may accept color greater than 300 ADMI; however, such wastes will be subject to cost surcharges Pursuant to the provisions of the PSA's Rules and Regulations. (14) Any trucked or hauled pollutants, except at discharge points designated by the Authority, unless the discharge has been approved by the Authority. (.15) No person shall otherwise introduce industrial wastes into the PSA's sewers with pollutants at a concentration of flow rate which will cause pass through pollution in the receiving stream or interference with the operation performance of the sewage works. (16) Any wastes containing metals or other pollutants in excess of specific concentrations or Loadings established as locally -derived limits by the PSA within its Rules and regulations. Monitoring and Retards All wastes most he accompanied by a completed waste manifest farm. The form shall contain information regarding the wastes origin. The Perm ittee or its agent must sign the form indicating that he has accepted no wastes other than those listed. Failure to accurately record every load or falsification of data shaD result in revocation of this permit and/or Penalty of up to $1000.00 per violation. Such revocation or penalty is at the sole discretion of the PSA. Spot Checks of hauled wastes may be performed at any time, without prior notice by the Authority or its designee. Charges Rates charged by the Authority for services provided pursuant to this Permit are subject to periodic revision at the sole discretion of the Authority. The Hauler will be required to pay the new rates for such discharges or dumping immediately subsequent to any such change in rates. Charges as the effective date of this permit are $0.03 per gallon or $30.00 for every 1,000 gallons_ flPage - HENRY COUNTY PSA - WASTE HAUttR PF-RMIT Payments 1. PSA bills are due 30 days from invoice date. If that date falls on a holiday or weekend. then the bill is due on the next business day. 2. Bills not received by the PSA on or before the date indicated on the front of your bill will be subject to a penalty. (See DELINQUENT ACCOUNT POLICY) 3. Cheeks returned for insufficient funds or other reasons will be regarded as no payment of account. Additional tees will be charged for all returned checks. Reoccurring returned checks may result in the account being placed on a cash payment status. 4. Payments may be mailed to Henry County PSA at P.O. Delinquent Account Police 1. if your delinquent amount is not paid by the due date indicated on your PSA invoice, you are subject to having your permit revoked that allows ,you to legally discharge septage waste into the PSA sanitary sewer system at the Septage Receiving Station- Fieldale Facility or any other approved location. 2. if your permit to discharge waste is terminated, PSA staff will immediately remove your company's ability to utilize the septage receiving station in Fieldale. 3. The permit can be re -issued after 1) cash, money order or certified check payment of the entire account balance, 2) a $20 permit reissuanee fee and 3) a deposit fee equaling three (3) average months usage is paid to PSA. 4. The $20 permit re -issuance fee will increase by $10 for each subsequent permit re4ssuance in any 12 month period. 5. Permit re -issuance is made as soon as possible after payment, but not necessarily the same day as payment. Customers can pay an additional $50 fee to ensure same -day permit re -issuance. 6. Also, we have record of your attendance at the New Operator Class on March 10, 2022. However, your Application for Permit to Operate a Septage Management Firm will not be processed until the vehicle(s) have been inspected for compliance with the rules and approved by the Division. When the Permit to Operate a Septage Management Firm is issued, the Firm Permit Number will be NCS- 01634. Please note that this letter is not a permit. Requirements for the pumper vehicle(s) can be found within 15A NCAC 13B .0835 of the Septage Management Rules. As noted within Rule .0835 (b), the firm name, town name, phone number, and permit number, NCS-01634, must be visible and permanently attached on both sides of the pumper vehicle.
