Issued June 10, 2019 and expires May 17, 2055. This facility permit is listed as Perquimans-Chowan-Gates Landfill Commission (owner) Republic Services of North Carolina, LLC / Stella Environmental (operator). PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 5 ADDITIONAL COMMENTS: 01 - This facility has a locking Gate at roadside entrance for security. 02 - Signage has required information, except for adding the Operators name and contact number (Stella). Roads are constructed of all-weather materials. 03 - Facility is permitted to receive waste from Perquimans, Chowan and Gates Counties. 04 - Waste must be transported to The East Carolina Regional Landfill (0803-MSWLF-1993) in Bertie County. 05 - Windblown trash was not an issue. A small amount to be picked up by the end of the working day. 06 - No leaks were noted in loaded trailers. The outgoing loads are being loaded well and tarped. 07 - The tipping floor and loading bay floor were clean. Some repairs are badly needed on the concrete at the tipping floor, tarmac and loading bay wall. It appears these areas are getting in worse condition (cracking, water seepage and falling/crumbling concrete). Repair as soon as possible. Keep ALL WASTE inside of the transfer station building. None should be unloaded on the tarmac. 08 - I-Beam in load out bay catches a lot of waste, please clean daily. 09 - Screening records were reviewed during this comprehensive facility inspection and were found to be in compliance. 10 - Repairs still needed in several areas of exterior metal. 11 - 2 (charged) fire extinguishers and a water hose are located on the tipping floor for fire suppression. 12 - Leachate system appears to be in good working order. 13 - ALL 3 leachate collection boxes and lids have been replaced. 14 - Scrap metal area is getting full. All white goods containing CFC are upright. 15 - Concrete area looks good. 16 - Yard waste is being managed well, except you MUST keep the fire lane open to 25 ft. to ensure firefighting equipment can get around the un-processed windrow in the event of a fire. Compost looks good. 17 - Scrap tires area is being managed well. 18 - Plastic jug recycling documents were not reviewed but appear to be managed well. 19 - Electronics are being collected at this site and are being managed well. 20 - Overall management of this facility is good. 21 - Certified Operators: Gregory Simpson, expires 2/3/2026 & Johnny Thomas, expires 3/10/2026. 22 - Required training has been taking place. Has 5 months of paperwork that training was provided. NO HOURS/TIMES LISTED PER CLASS. 23 - Matt Einsmann (Republic Services) was notified of this inspection the day before to give Republic Services an option to attend. 24 - Digital pictures were taken during this inspection with some attached to this report.
