In accordance with your l~tter of requent received Havamber 12, 1982, we
are fon,arding herewith Pamit No. 7634-R dated November 19, 1982, to the
Department of Crime Control &Qd Public Safety for the operation of th@
•ubject altisting nondi~cbario type wastewater treatment facilities.
Thi• pendt shall be effectiw frOTt the date of issuance until July l,
1983, ffhall be subject to the conditions and limitations as ap~cified therein,
and shall void Permit No. 7634, iesued September 14, 1982.
If any parts, requiremc!nts, or limitations contained in this psmit a~e
urutcceptable to you, you. have the right to an adjudicatory bearing before a
hearing officer upon written demand to th• Director within thirty (30) daya
following receipt of thia permit, identifying th• specific ieeuee to be
contended. Unless such dem.and ie mitda, this permit shall be final cnu
binding.

pursuant to the letter of request received November 12, 1982, and in conformity
with tbe project plane, apecif1aation•, and other eupportins data, eubaequently
filed and approved by the Department of Natural Re~ources and Coomtunity
Develop1Mt1t and considered a part of this permit.
This pendt shall be effective from the d~te of issuance until July 1, 1983,
•hall void Permit No. 7634, i•sued September 14, 1982, and shall be subject
to the following Sl)ecified c(mdition• and limitation■i
1. This p9nait shall baeOIM voidable unless the facilities were constructed in
accordance with tha apprOTed plane, specifications and other •upportins
d11.ta.

described in the application and other supporting data.

4. The wastewater collected by thia ayatn ahall be ad•~uately treated in
the Watten County PCB Landfill Wastewater Treatlllent Facility and analysed
for PCB conc.eutration prior to king diacharged onto th• receiring
landfill surface irrigation area.

7. In event the faciliti@& fail to perform Matiefacto~ily. the Pendttee shall
take such immediate corrective action a• may ha required by thia Department.
O. Solids, sludr,ee, or other pollutant8 removed or reeulting from the wastewater
treatment proc••• shall be contained and disposed of in such a unner
as to prevent any contamination of tha surface or grotmd waters of the
St&ta.
9. The Pomittee ah.all employ a certified wastewater trestment plant operator
in reeponaible charge of the wastewater treatment facilities. Such
oporator must hold a cartificate of the gTftde at lea~t equiTalent to the
cla•sification aaei~ned to the waetevater treatnent facilities by the
Certification Colllllission.
10. Diversion or bypaaoiug of the untreated wast.-,~ater frOtll the treatment
facilities is prohibited.
11. All of the effluent waBtewatar from the treatnrent facilities shall be
discharged onto the Warren County PCB Landfill •urfAce irrigation area
and no wastevater shall be diecharged to th• aur!ace water• of the State.
12. The aand and carbon filter media and top 3-inches ot soil in the effluent
wastewster tetantion pond eha.11 bo disposed of iu the Varren County
PCB Landfill or at a •uitable and apprOYed hazardou• wAate di•po•al site.
13. Any monitoring dAn,.ed necessary by tbe DiYision of nnviroumental Management
to insure aurfaca and groundwator protection vi.11 be a•tabli■hed and an
acceptable sampling and reporting •chedule ahall be followed.
14. The ie•uance of this perait eball not roliave the Department of Crime Con-
trol and Public Safety of the responeibility for damages to •urface or
groundwaters resulting from the operation of this facility.

15. The facilities shall be effectively maintained and operated as a non-
discharge type system to prevent the discharge of any wastewater into
the surface waters of the State.
16. The concentration of PCB in the effluent wastewater to be land applied
by these spray irrigation facilities shall not exceed 1.0 PPB.
17. A suitable year round vegetative cover shall be maintained and sown prior to spraying.
18. Irrigation shall not be performed during inclement weather or when the
ground is in a condition that will cause runoff such as within a 24-hour
period after a total rainfall event of 0.5 inches or greater within the
previous 24-hour period or when the wind velocity exceeds 15 mph.
19. The application rate shall not exceed 0.25 inches per acre per week.
20. Freeboard in the storage lagoon shall not be less than 2-feet at any
time.
21. No type of wastewater other than that from the Warren County PCB Landfill
shall be treated by this facilities and apray-irrigated onto the irriga-
tion area.
22. No root or leaf crops for public consumption shall be raised on this
site.
23. The spray irrigation area shall not be grazed nor shall the cover crop
be harvested or removed from the site.
24. Spray irrigation will only be conducted on the designated site 0\~1ed
by the North Carolina Department of Crime Control and Public Safety and
shall not be conducted within 25-feet of the property lines.
25. Adequate records and an operational log shall be maintained at these
facilities detailing the following information:
a. date of irrigation and date wastewater analyzed last,
b. amount of wastewater irrigated,
c. location and amount of area irrigated, and
d. general weather conditions
26 . Within thirty (30) d.:1ys after expir.:1tion of this permit or the end of
these facilities operation, c1 repor t s ummarizing the total amount of
wastewater spray irrigated, analysis of the wastewater, analysis of the
irrigation site soil, analysis of the irrigation s ite cover crop, a nd
groundwater monitoring wells sample analysis shall be submitted for
review' by the Division.
