This permit shall be effective from the date of issuance until January 1, 1983 . -·--ancl-shar1.-b-e subject to the conditions and limitations ·:is specified therein. If any parts, requirements, or limitations contained in this permit are unacce~t- able to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within thirty (30) days following receipt of this permit, identifying the specific issues to be contended. Unless such demrtnd is made, this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded t6 you. Helms Director NORTH CAROLINA ENVIR0NMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMa;r RALEIGH P E R M I T For the Discharge of ~:ewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 1~3~ General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISS :ON IS HEREBY GRANTED TO Department of Crime Control and Public Safety Warren County FOR THE operation of 0.80 MGal. nondischarge type wastewater treatment facility consisting of a 125 GPM influent pump with leachate collection system and sump, a 52 square feet sandfilter, a 26 square feet activated carbon filter, a 0.80 MGal. holding pond with rainwater diversion pipe and valve, and a 125 GPM portable effluent recycle- irrigation pump to serve the Warren County PCB Landfill with no discharge of waste- --wifter-fi:> ___ the surface waters of the -state, · pursuant to the application received September 14, 1982, and in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until January 1, 1983, and shall be subject to the following specified conditions and limitations: 1. 2. 3. 4. 5. This permit shall become voidable unless the facilities were constructed in accordance with the approved plans, specifications and other supporting data. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. The facilities shall be properly maintained and operated at all times. . \ -. -The wastewater collected by this system shall be treated in the Warren County PCB Landfill Wastewater Treatment Facility prior to being discharged onto the receiving landfill surface area. This permit is not transierable. Pe~it No. 7634 Page 2 6. This permit shall become voidable in the event of failure of the landfill soil to adequately absorb the wastes, and may be rescinded unless the facili- ties were installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 7. In event the facilities fail to perform satisfactorily, the Permittee sh~ll take such irnrnediat:e corrective action as may be required by this Departm~nt. 8 . Solids, sludges, or other pollutar.ts r emoved or resulting from the wastewater treatment process shall be contained and disposed of in such a manner as to prevent any contaminat ion of the surface or ground waters of the State. 9. The Permittee shall employ a certified wastewater treatment plant operator in responsible charge of the wastewater treatment facilities. Such operatc r must hold a certificate of the grade at least equivalent to the classificaticn assi gn- ed to the wastewater treatment facilities by the Certification Commission. 10. Diversion or bypassing of the untreated wastewater from the treatment facilities is. prohibited. 11. All of the effluent wastewater from the treatment facilities shall be discharged onto the Warren County PCB 'Landfill open-pit area and no wastewater shall be discharged to the surface waters of the State. 12. The sand and carbon filter media and top 6-inches of soil in the effluent waste- water retention pond shall be disposed of in the Warren County PCB Landfill or -~~a-'--'t~_a Sll_itable and approved hazar.9ous waste __ disp0$~1_-~ite. 13. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling and reporting schedule shall be followed. 14. The issuance of this permit shall not relieve the Department of Crime Control and Public Safety of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 15. The facilities shall be effectively maintained and operated as a nondrscharge type system to prevent the discharge of any wastewater into the surface -waters of the State.
