Department of Human Resources Division of Health Services Solid Waste Management Unit Post Office Box 2091 Raleigh, North Carolina 27602 Attention: Mr. Jerry Perkins Subject: Gentlemen: Proposal Investigation of Proposed PCB Disposal Site Warren County, North Carolina S&ME Job No. RG-730 Soil and Material Engineers, Inc. appreciates the opportunity to provide an investigation at the proposed PCB disposal Site near Afton in Warren County, North Carolina. This letter describes our scope of work and presents the estimated cost and our unit fees. Near surface soils at the site will be investigated by drilling about thirteen borings. We expect to drill nine borings within the proposed trench excavation. Four borings ·will be drilled around the perifery of the trench excavation to evaluate the near surface soils in the area. Emphsis will be placed upon obtaining bulk samples of the soils in the upper ten feet. Bulk samples will be obtained using a hollow square auger which permits visual determination of soil strata changes. The borings near the corner of the disposal trench will be extended to refual or a maximum of 45 feet with a standard six (6) inch diameter hollow stem auger, to determine the water level and to evaluate the potential for encountering rock within the trench excavation. Department of Human Resources February 5, 1979 Page 2 Soil samples will be analyzed in our laboratory to determine the grain size distribution, Atterberg Limits (liquid limit, plastic limit, plasticity index), in-situ moisture, compaction characteristics and permeability. After completion of the drilling and laboratory work, our engineering report will be prepared which will present the test data and discuss our conclusions and evaluations of both the quality and quanity of near surface clayey soils. The total cost will depend upon actual work performed. Some variations in our proposed scope of work may be required, depending upon conditions encountered. We have estimated the cost of the work to be about $4,400.00 based upon the assumed amount of work that will be required. The following is a breakdown of this estimated cost. I. Drilling, 20 hours at $65.00/hour ------$1,300 .00 II. Laboratory Services: (a) Atterberg Limits; 20 at $28.00 each------------------------------$ 560.00 (b) Grain Size (Hydrometer); 20 at $55.00 each-----------------------$ 1,100.00 (c) Standard Proctors; 4 at $55.00 each------------------------------$ 220.00 (d) Falling Head Permeabilities; 4 at $75.00 each-----------------------$ 300.00 III. GENERAL CDNDITIONS INSURANCE -Soil & Material Engineers, Inc. maintains Workmen's Q:mpensation and Employer's Liability Insurance in conformance with state law. In addition, we maint ain Canprehensive General Liability Insurance and Autaoobile Liability Insurance with bodily injury limits of $100,000.00 -$300,000.00 and property damage limits of $100,000.00. A certificate of insurance can be supplied evidencing such coverage which contains a clause providing that ten days written notice be given prior to cancellation or reduction of coverage. Cost of the above coverage is included in our quoted fees. If additional coverage or increased limits of liability are required, we will endeavor to obtain the requested insurance and charge separately for costs associated with addit ional coverage or increased limits. CHAN:iED CDNDITIONS -The outlined scope of services will be accanplished in a timel y, v.orkrmnlike, and professional manner by anployees of Soil & Material Engineers, Inc. at the unit fees quoted. If during the execution of the rork we are required to stop operations as a result of changes in the scope of \\Ork, such as requests by the owner or requiranents of third parties, additional charges will be applicable. SAMPLIKi OR TFSTIKi LOCATIONS -'Ihe unit fees included in this proposal do not include costs associated with surveying of the site or the accurate horizontal and vertical locations of tests. Fi eld tests or boring locations described in our r eport or shown on our sketches are based on specific infonnation furnished to us by others or estimates made i n the field by our technicians. Such dimensions, depths or elevations should be considered as approximations unless otherwise stated in the report. DAMAGE 'IO EXISTIKi MAN MADE OBJH:"I'S -It shall be the responsibility of the owner or his duly authorized representative to disclose the presence and accurate locat ion of all hidden or obscure man made objects, relative to field test or boring locat ions. Our field personnel are trained to recognize clearly identifiable stakes or markings in the field, and without special written instructions, to initiate field testing, drilling and/or sampling within a few feet of each designated location. If we are cautioned, advised or given data in writing that reveal the presence of under ground or overground obstructions, such as utilities, special instructions will be given our field personnel by us. As evidenced by your acceptance of this proposal, you agree to indannify and save Soil & Material Engineers, Inc. harmless fran all claims, suits, losses, personal injuries, death and property liability resulting fran unusual subsurface conditions or darmges to subsurface structures, owned by you or third parties, occuring in the prosecution of the proposed rork, whose presence and exact locations were not revealed to us in writing, and to reimburse Soil & Material Engineers, Inc. for expenses in connection with any such claims or suits, including reasonable attorney's fees. SAMPLE DISIUSAL AGREEMENT -Test specimens or samples (excluding drilling samples) will be disposed of imnediately upon canpletion of tests. All drilling samples will be disposed of 60 days after subnission of our report unless otherwise requested.
