The annual reporting form for your facility is available at the web link shown below. Please
note that permitted North Carolina Solid Waste Management facilities are required to
complete and submit a solid waste activities annual report under NC General Statute § 130A-
309.09D and in compliance with your facility permit conditions. The date for completing and
submitting the appropriate facility reporting form to the Division is August 1; 2023.
Your reporting form is web -based and available to you by copying and pasting following link
into your web browser:

A new feature are self -populating fields on the first page including data for Contacts and
Mailing addresses based on your choice in the drop -down box. Please check and make
corrections as needed for those fields. Please keep in mind that the entered contact and
address information will be used by this office to communicate with you for the next year.
You may Save a Working Draft [but, only 1 saved report per email address]. When you
complete and submit the form, you may Download [recommended], Print or Email the
finished document.

Accurate and complete responses are needed not only to comply with the regulations, but to
also provide value to various public and private sector entities for their long-term planning
purposes. If any question is unclear, you might review last year's report or you may contact
Sarah Moutos, Environmental Senior Specialist for your facility, at either 919.707.8287 or by
email at sarah.moutos@deq.nc.gov for guidance.
When data entry is complete, please do the following:
1. Press the Submit button on the final page.
2. Click the Download option and save a copy of the completed Annual Report.
3. A copy of the form is required to be sent to the County Manager of each
county from which your facility received waste.
4. A copy of the report will automatically be forwarded to your inspector when
the form is submitted.
5. The person submitting the form will receive an email confirming the report is
accepted when Solid Waste staff complete their review.
