Permit For Closure – Phase 1 (partial) issued August 24, 2018. PURPOSE OF SITE VISIT: Comprehensive Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS The facility is a construction and demolition debris landfill (C&DLF). The facility is permitted to receive construction and demolition debris, inert debris, land clearing debris, and asphalt from Wake, Durham, Orange, Johnston, Franklin, Chatham, Alamance, Caswell, Person, Granville, Vance, Warren, Nash, Wilson, Wayne, Harnett, and Lee counties. The facility receives approximately 850 tons of waste per day. 1. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. The correct signage is posted at the entrance of the facility listing the facility name, permit number, hours of operation, emergency contact information, and a list of unacceptable wastes. 3. The facility accepts waste Monday through Friday from 7am until 4pm. 4. The interior roadways are of all-weather construction and are maintained in good condition. 5. The facility permit, operations plan, water quality and landfill gas monitoring reports, and training records were available onsite and were reviewed with Mr. Plessinger. 6. The following are certified facility personnel: a) James Levitt, Certified Landfill Operations Specialist, LF-2018045, exp. 5/17/2024. b) Terry Basnett, Certified Landfill Operations Specialist, LF-2008002, exp. 2/8/2026. c) David Kearney, Certified Transfer Station Operations Specialist, TS-2017058, exp. 12/7/2024. d) Stacey Virta, Certified Landfill Operations Specialist, LF-2018031, exp. 3/20/2024. e) Donald Plessinger, Certified Manager of Landfill Operations, Cert No. 093857, exp. 8/6/2023. 7. Waste screening records were verified up to 5/31/2023. Facility staff screen 1% of incoming loads. It is recommended that in addition to incoming load descriptions, records of rejected loads and removal of prohibited wastes are also maintained within the operating record. 8. Windblown waste is well managed and very little windblown waste was observed outside of the active working area during this inspection. 9. The facility utilizes a water truck for dust control. 10. The working face and exposed waste areas appeared to be approximately one-half acre in combined size. 11. During this inspection, a load which appeared to consist of source separated pallets and cardboard mixed with some MSW waste was dumped in the disposal area as inspectors looked on. Mr. Plessinger stated that this load might have been dumped by mistake and that he would find out where it came from. Ensure any incoming loads of source separated wooden pallets and cardboard are redirected to a facility that is generally used for the management of recovered/recyclable materials. 12. Permanent edge of waste markers were clearly visible and adequately spaced. 13. Erosion rills were observed on the northwest side slope of the landfill and excess sediment was observed within the stormwater drainage channel along the toe of the western and northwestern slopes. Ensure that erosion rills are repaired, properly graded, and vegetated as soon as they are observed, and as conditions allow. Stormwater drainage channels should be cleaned of sediment and maintained as designed to ensure they function properly. 14. Several water quality monitoring wells were inspected by the Section and Mr. Plessinger and determined to be in compliance. 15. The vegetative cover had been recently mown. 16.
