ECS previously completed an underground storage tank (UST) Closure and Soil Remediation Report for the site (ECS Project G-7159-B) dated April 11, 2003. Soil impacts were identified from the post-excavation soil samples collected after the removal of approximately 91.64 tons of petroleum impacted soil at concentrations above the North Carolina Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs). ECS subsequently completed a Phase I Limited Site Assessment (LSA) at the site in 2005 which included the installation and sampling of a monitoring well (MW-1) in the vicinity of the former UST basin. Groundwater impacts above the North Carolina 2L Groundwater Quality Standards (NC2LGWQS) were identified in the groundwater sample collected from monitoring well MW-1. Based on the results of the Phase I LSA, a Phase II LSA consisting of a receptor survey and the installation and sampling of two downgradient (MW-3 and MW-4) and one upgradient (MW-5) monitoring wells was conducted in 2006. There is no record of monitoring well MW-2 in the historical information available to ECS. The results of the groundwater sample collected from monitoring well MW-1 detected petroleum compounds at concentrations above their respective NC2LGWQS. The results of the groundwater samples collected from monitoring wells MW-3 through MW-5 did not detect targeted compounds at concentrations above the laboratory reporting limits. Known and unknown tentatively identified compounds (TICs) were detected in each of the groundwater samples collected. Additionally, nine water supply wells were identified within 1,000 feet of the source area of the release. A Notice of Regulatory Requirement (NORR) correspondence from the North Carolina Department of Environmental Quality (NCDEQ) dated September 16, 2019 indicated that the site Incident, Incident Number 30141 is classified as a High Risk site with an applicable Risk, Rank, and Abatement (RRA) Score of H240D. Additionally, the NORR requested that based upon the site risk posed by the groundwater impacts that the following tasks be performed: 1.) To update the current groundwater condition at the site, a groundwater monitoring event is to be conducted; and 2.) An updated receptor survey is to be completed. A copy of the NORR is included in Appendix A. ECS was requested by Mr. Treadway and his council Mr. Steele to provide Environmental Services at the site to evaluate current site conditions and to comply with the September 16, 2019 NORR requests. Groundwater Monitoring Report Brightwood Inn and Restaurant 6501 Burlington Road Whitsett, Guilford County, North Carolina NCDEQ Incident No. 30141 ECS Project No. 49-10492 January 31, 2020 2 2.0 SCOPE OF SERVICES This groundwater assessment consisted of collecting four (4) groundwater samples from the on- site monitoring wells (Figure 3) located at the site and updating the receptor survey in the vicinity of the site. On November 22, 2019, ECS personnel mobilized to the site to sample four groundwater monitoring wells as outlined in ECS Proposal 49-15684-P (Amended) dated October 24, 2019. It was discovered that monitoring well MW-1 had been destroyed. Sampling MW-1 is critical as it was historically impacted with petroleum compounds above NCDEQ groundwater standards. On January 17, 2020, ECS observed the installation of monitoring well MW-1R. Drilling activities were conducted using a Geoprobe® drill rig. The subcontractor used for this project was 3D Environmental Investigations, LLC (3D). Drilling activities were performed under the supervision of a North Carolina licensed well driller employed by 3D. A photographic log documenting the well installation is provided in Appendix B. 2.1 Monitoring Well Permit Based on a telephone conversation with Ms. Donna Mack with the Guilford County Department of Environmental Health (GCDEH), the implementing agency for the NCDEQ in Guilford County, a monitoring well permit is on file for this site. No additional permit is required for a replacement well. 2.2 Utility Locate Prior to commencing with field activities, ECS contacted North Carolina One-Call to locate/mark public utilities at the site. Required separation distances between subsurface activities and marked utilities (typically 30-inches) were maintained during the field activities. 2.3 Monitoring Well Installation A soil boring was drilled using the Geoprobe® equipped with 4.25 inch solid-stem augers to a depth of 15 feet below the ground surface (bgs).The well was constructed with 2-inch I.D. schedule 40 PVC flushed-threaded casing and screen. The PVC screen and casing were lowered to the bottom of each borehole. A 10-foot length of slotted well screen with machined 0.010-inch slot widths and threaded bottom plug were installed at the bottom of each well. A solid section of PVC casing was placed above the screened interval and extended to a point approximately 0.5 foot below the ground surface. The annular space around each well screen was filled with washed fine filter sand to approximately 2 feet above the top of the screen. A minimum 2-foot thick seal of bentonite pellets was placed immediately above the sand pack and hydrated. Cement grout was placed from the top of the bentonite seal to 0.75 foot below the ground surface The top of each well casing was equipped with a lockable, water-tight well plug and steel manhole cover. A 2-foot square concrete pad was constructed flush with the ground surface. A monitoring well construction diagram and NCDEQ Form GW-1 (Well Construction Record) is included in Appendix C. Groundwater Monitoring Report Brightwood Inn and Restaurant 6501 Burlington Road Whitsett, Guilford County, North Carolina NCDEQ Incident No. 30141 ECS Project No. 49-10492 January 31, 2020 3 2.4 Groundwater Sampling Activities On January 20, 2020, monitoring wells MW-1R, MW-3, MW-4 and MW-5 were purged using a new disposable polyethylene bailer and new length of nylon cord. The wells were developed/purged by evacuating a minimum of five well volumes using the bailer and cord. The wells were developed/purged to remove sand, silt and other fine sediments which may have entered the wells during construction and to promote communication between the surrounding formation and the sand pack surrounding the well’s screened interval. After the purging was completed, a groundwater sample was collected from each well using the bailer and cord, and transferred into sample containers provided by the laboratory. The groundwater sample containers were labeled and placed into a cooler with ice to maintain the samples at approximately 4° Celsius (C°). The samples were submitted to Research and Analytical Laboratories, Inc. (R&A), a North Carolina certified laboratory to be analyzed for volatile organic compounds (VOCs) and semi-volatile organic compounds (SVOCs) using EPA Methods 6200B and 625 BNA (base neutral and acid extractable) plus ten TICs, respectively, extractable and volatile petroleum hydrocarbons (EPH and VPH) using the Massachusetts Department of Environmental Protection (MADEP) Methods and lead. ECS maintained chain-of- custody procedures throughout the sample collection and transportation process. Chain-of Custody Records are included in Appendix D. 2.5 Receptor Survey Update Visual evidence of water supply wells, such as well houses or “fake” rocks, was observed within 1,000 feet of the source area during the site reconnaissance. On December 10, 2019, water supply well questionnaires were sent via parcel post to the four property owners representing nine parcels where water supply wells were previously identified (Table 1 and Figure 4). One questionnaire was returned as undeliverable. One questionnaire was returned and did identify an in-use water supply well within 1,000 feet of the release. The water supply well questionnaires are included in Appendix E. 3.0 LABORATORY ANALYTICAL RESULTS Laboratory analysis of groundwater sample MW-1R detected benzene, I-Propylbenzene, naphthalene, N-Propylbenzene, 1-Methylnaphthalene, 2-Methylnaphthalene,C5-C8 and C9-C18 aliphatic hydrocarbons, and C9-C22 aromatic hydrocarbons above their NC2LGWQS. Laboratory analysis of groundwater samples MW-3 and MW-5 detected lead above its NC2LGWQS. Laboratory analysis of groundwater sample MW-4 detected 1,2-Dichloroethane above its NC2LGWQS. A summary of the laboratory analytical results are shown on Table 2 and the laboratory data sheets are included in Appendix D. Groundwater Monitoring Report Brightwood Inn and Restaurant 6501 Burlington Road Whitsett, Guilford County, North Carolina NCDEQ Incident No. 30141 ECS Project No. 49-10492 January 31, 2020 4 4.0 CONCLUSIONS Based on the laboratory analytical results and receptor survey; ECS concludes the following: • Groundwater remains impacted above regulatory standards by petroleum compounds beneath the site. • It is ECS’ opinion that the lead detected across the site is not related to the release of petroleum given the absence of petroleum related compounds in samples MW-3 through MW-5. • In-use water supply wells are located within 1,000 feet of the release. • It is ECS’ opinion that the site will remain classified as a High Risk site. 5.0 RECOMMENDATIONS Based on the results of the groundwater monitoring event and the requirements of the NORR, ECS recommends the following: • ECS recommends that a copy of this report be submitted to the NCDEQ for their review. 6.0 QUALIFICATIONS OF REPORT The activities and evaluative approaches used in this assessment are consistent with those normally employed in environmental assessment projects of this type. Our evaluation of site conditions has been based on our understanding of the site project information and the data obtained during our field activities. This report was prepared for the express use of Mr. Paul K. Treadway. The use of this report by any undesignated third party or parties will be at such party’s sole risk and ECS disclaims liability for any such third party use or reliance.
