This subject property is located at 12104 E. Old Highway 64 in Lexington, NC and is currently being used as a church building. The location of the project site is illustrated on the partial Denton Quadrangle U.S.G.S. Topographic Map which is included as Figure 1. One (1) 7,500 gallon gasoline UST, two (2) 3,000 gallon gasoline USTs, one (1) 2,000 gallon (unknown) UST, one (1) 2,000 gallon diesel UST, and one (1) 1,000 gallon kerosene UST were formerly located at this facility. The USTs were removed from the project site in November of 1989 by Southern Pump and Tank Company. The site layout and the former locations of the USTs are illustrated on Figure 2. A UST Closure Report and Limited Site Assessment were previously submitted to NCDEQ by the Structures Group, and a Comprehensive Site Assessment was previously submitted to the NCDEQ by Paragon Environmental Consultants, Inc. A Corrective Action Plan was prepared by Paragon Environmental Consultants, Inc. and was submitted to the NCDEQ on June 12, 2008. The site has been ranked as High Risk based on supply wells in use for consumption. Information about the supply wells is contained in Table 1. Contained herein are the results of a monitor well sampling event as requested by the NCDEQ. 1.2 Scope of Services This Groundwater Monitoring Report contains documentation concerning the following activities which have been conducted at this facility: o A monitor well sampling event conducted by Paragon on March 7, 2023 o Laboratory analyses of groundwater samples conducted by Waypoint Analytical, LLC of Charlotte, NC o The approximate groundwater flow direction and hydraulic gradient at the site calculated by Paragon Page - 2 2.0 Field Activities 2.1 Ground Water Flow Direction and Gradient In order to estimate the groundwater flow direction in the vicinity of the petroleum release, elevations of the monitor well casings were measured using surveying equipment. Static water levels were obtained from each of the wells and the data was used to compute a potentiometric surface map. Figure 3 illustrates the current potentiometric surface map as calculated based on the measurements taken on March 7, 2023. Groundwater data and monitoring well information, which are contained in Table 2, were also used to estimate the hydraulic gradient at the project site. Using the horizontal distance between the monitor wells and the static water levels, the groundwater was estimated to be flowing toward the east with a hydraulic gradient of approximately 0.0938 feet per feet. 2.2 Monitor Well Sampling and Analyses On March 7, 2023, monitor wells MW-1, MW-3, MW-4, and MW-6 were sampled for analyses utilizing EPA Method 6200B plus MTBE and IPE. Three monitor wells were unable to be sampled due to improvements to the building structure made within the last year. Monitor well MW-2 has been covered by a thick layer of gravel laid near the foundation of the building or possibly destroyed. Monitor well MW-5 was covered by a concrete pad or possibly destroyed by the installation of a new septic system. Existing well EW-2 has been covered by a new addition to the front of the building that now connects the former beauty shop building to the main building on this property. Before collecting the groundwater samples, the monitor wells were purged by removing a minimum of three well volumes to ensure that the samples were representative of the actual groundwater conditions. The wells were developed using disposable plastic bailers suspended with unused nylon string. Following development of the monitoring wells, groundwater samples were obtained from the disposable bailers. All samples were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date, and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of- custody procedures. The laboratory analytical results for monitor well MW-1 reported one EPA Method 6200B compounds above the 2L Standards. Benzene was detected at a level of 3.69 micrograms per liter (ug/L), as compared to the 2L Standard of 1 ug/L. The laboratory analytical results for monitor well MW-4 reported MTBE at a concentration of 96 ug/L, which exceeds the 2L Standard of 20 ug/L. All other target compounds were below the 2L Standards in monitor wells MW-1 and MW-4 according to EPA Method 6200B. Monitor wells MW-3 and MW-6 were below the 2L Standards for all EPA Method 6200B compounds. An isoconcentration map illustrating the estimated extent of Benzene at this facility is contained as Figure 4. The locations of the supply wells are depicted on Figure 5. Table 1 contains current information about the supply well owners and their mailing addresses. Table 3 summarizes the laboratory analytical results for the sampling event conducted on March 7, 2023, and Appendix A contains a copy of the laboratory report and the chain-of- custody record for the groundwater samples obtained on this date. Page - 3 2.3 Plume Development The results for Benzene in the source area monitor well is at a historical low concentration. The Benzene concentration in monitor well MW-1 was at a level of 30.5 ug/L in January of 2022, and the current concentration in MW-1 was reported at 3.69 ug/L for Benzene. Benzene continues to decrease in the source area monitor well and is now only slightly in excess of the 2L Standard. The concentration of MTBE in MW-4 is higher than the previous sampling event conducted in January of 2022; however, MW-4 has reported MTBE concentrations in excess of the 2L Standards for many previous sampling events. The other monitor wells sampled at this facility showed no change since the previous sampling event as all of these wells have been below the laboratory detection limits for the last several events. Table 4 contains the time based analytical results for Benzene at the subject site. 3.0 Conclusions and Recommendations 3.1 General Summary A Groundwater Monitoring event at Kountry Korner located at 12104 E. Old Highway 64 in Lexington, NC has been completed. From a review of all information gathered during this monitoring project, Paragon Environmental Consultants, Inc. makes the following conclusions: o A Benzene concentration which slightly exceeds the 2L Standard is present in the source area monitor well. A MTBE concentration in excess of the 2L Standard is present in MW-4. All other monitor wells sampled on March 7, 2023 are below the 2L Standards for all compounds according to EPA Method 6200B. o The analytical results for Benzene in the source area monitor well showed the lowest contaminant concentrations ever reported. 3.2 Recommendations Paragon recommends continuing the sampling events to be conducted on a semi-annual basis unless directed otherwise by the NCDEQ. The assessment was conducted based on the scope of work and level of effort desired by the client. Our findings have been developed in accordance with generally accepted standards in the practice of Groundwater Monitoring Reports in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters indicated in the laboratory reports. No additional information can be logically inferred from this data.
