for Petroleum Underground Storage Tanks Sandy Cross Mini Mart 2078 North NC Highway 49

Risk Classification: High Ranking (RRA): H250R
Dear Mr. Barnwell:
The subject incident is directed to continue with assessment and remediation. The last report received for this incident
was a Groundwater Monitoring Report dated May 14, 2020. The risk posed by the discharge or release at the subject
site is classified by the Department of Environmental Quality (Department) as high, as stipulated under Title 15A
NCAC 2L .0406. Title 15A NCAC 2L .0407(a) requires you to notify the Department of any changes that might affect
the risk classification that has been assigned.
Based on the determination that the risk posed by the discharge or release at the subject site is high, you must comply
with assessment requirements pursuant to Title 15A NCAC 2L .0407(c), 2L .0110, and 2L .0111. Please submit a
Monitoring Report in accordance with Title 15A NCAC 2L .0407(b), 2L .0110, and 2L .0111, and the most recent
version of the UST Section Assessment Guidelines. The Monitoring Report must be received but this office within 60
days of the date of receipt of this notice. Failure to comply in the manner and time specified may result in the
assessment of civil penalties and/or the use of other enforcement mechanisms.
Because a release or discharge has been confirmed, a Licensed Geologist or a Professional Engineer, certified by the
State of North Carolina, is required to prepare and certify all reports submitted to the Department in accordance with
Title 15A NCAC 2L .0103(e) and 2L .0111(b).
