2. BACKGROUND
The City of High Point Potts Avenue site is located at 1329 Potts Avenue in High Point, Guilford
County, North Carolina (Figure 1 and Figure 2). The City of High Point purchased the subject
property for the construction of a proposed elevated water storage tank for the City’s water
system. Based on a review of historic aerial photographs, several industrial commercial structures
were previously located on the site prior to 1998. During 2015, the site was mostly wooded and
no structures existed. During 2015, due diligence activities discovered two USTs on the subject
property. Each UST was estimated to be approximately 550-gallons in size and formerly used as
heating oil tanks. The subject USTs were previously owned and operated by an unknown party.
The two 550-gallon heating oil USTs were located end to end and occupied a common UST basin.
The City of High Point contracted the removal of these USTs, which occurred on November 10,
2015. Known UST System and Other Release Information is summarized in Table 1. Table 2
summarizes UST Owner/Operator Information.Figure 3 depicts the location of the USTs on a site
aerial photograph based figure. Photographs of the site are included in Appendix I.
3. SITE UST INFORMATION AND CHARACTERIZATION
3.1 UST Information
The site contained two 550-gallon heating oil USTs, each 6-feet long and 4 feet in diameter.
Information pertaining to these USTs is provided in Table 2.
3.2 UST Owner and Operator Information
As a result of the purchase of the subject site, both the 550-gallon heating oil USTs were owned
by the City of High Point. The City of High Point never operated the subject USTs. No
information is currently known regarding the prior ownership or operation of the subject USTs.
Table 2 summarizes this information.
3.3 UST Release
During November 2015 the USTs were permanently closed by removal. Analytical results of two
soil samples collected beneath the UST indicated concentrations of Total Petroleum
Hydrocarbons in the Diesel Range (TPH-DRO) of 4,870 and 14,700 mg/kg, for samples UST-A and
UST-B, respectively.
3.4 Site Characteristics
Based on the topography of the area, the site appears to slope toward the south (Figure 1), and
the groundwater flow direction would likely mirror the topography, with flow toward the south.
The site is located in an industrial and commercial area with some residential land use to the east
of the subject site. The prior site structures were previously demolished and the site was vacant
and mostly wooded.
According to the 1982 Geologic Map of Region G, North Carolina, the site is located in the
Caroline Slate Belt and the rock underlying the site is comprised of a felsic intrusive complex (icf).

4. UST CLOSURE AND INITIAL ABATEMENT ACTION
4.1 UST Closure Activities
On November 10, 2015, S&ME was on-site to observe and document the closure procedures used
for the removal of the 550-gallon heating oil USTs. The closure and subsequent initial abatement
action activities were conducted by EVO Corporation (EVO). The tops of the USTs were located
approximately three feet below land surface. On November 10, 2015, EVO used a vacuum truck
to remove residual liquids in the tanks, which was mostly water, with a slight sheen, exhibiting a
fuel oil odor. Approximately 800 gallons of fluids (mostly water, identified as “petroleum contact
water”) was disposed off-site by EVO. A copy of the liquid disposal manifest is included in
Appendix II.
On November 10, 2015, the two steel USTs were removed and crushed for disposal. The tanks
were observed to exhibit severe corrosion with multiple holes visible. The crushed USTs were
transported off-site for disposal by EVO. A copy of the UST disposal documentation is included in
Appendix II. Photographs of the removed USTs are included in Appendix I.
Following removal of the USTs, S&ME evaluated the underlying soils for evidence of impacts,
obtaining soil head-space field screening data with a photo-ionization detector (PID) for the
detection of volatile organic vapors. Field screening data was obtained for the soil samples
collected for closure confirmation. Two closure soil samples (sample IDs = UST-A and UST-B)
were collected from soils located immediately below the 550-gallon heating oil USTs. The two
closure samples were submitted to Research and Analytical Laboratories, Inc. for laboratory
analyses for total petroleum hydrocarbons in the diesel range (TPH-DRO by Method 8015/3550).
Results of the closure samples indicated the following:
•As summarized in Table 3, TPH-DRO was detected above the North Carolina Department
of Environmental Quality (DEQ) Action Limit of 10 mg/kg TPH in both samples UST-A and
UST-B.
The locations of the soil samples are indicated on Figure 3.
4.2 Soil Excavation and Sampling
Between November 10-23, 2015, suspect petroleum hydrocarbon impacted soils were excavated
as an initial abatement action, using soil head-space field screening data as a guide for the
excavation. The excavation continued and was terminated based on client discretion.
Approximately 1,859.24 tons of soil was removed and transported off-site for disposal.
Groundwater was not encountered during the excavation activities. A copy of the soils disposal
documentation is included in Appendix II.
Twelve soil samples were collected to assess the initial abatement actions. Soil samples W1-11,
W2-18, S1-22, S2-18, E1-18, E2-12, N1-20 and N2-10 were collected from the excavation
sidewalls, B1-20, B2-12, CENTER-18 and TB-18 were collected from the bottom of the excavation.
These soil samples were submitted for laboratory analyses by Method 8260, Method 8270, and
MADEP VPH and EPH.

4.3 Initial Abatement Soil Sample Analytical Results
Table 4 and Table 5 provide a summary of the soil sample analytical results, for samples collected
from the limits of the final excavation. These tables include the corresponding NCDEQ Soil to
Groundwater, Residential, and Industrial Commercial; Maximum Contaminant Concentrations
(MSCCs). As summarized in Table 4 and Table 5:
•Analytical results for analysis by MADEP methods detected concentrations of Alphatics
C5-C8,and Alphatics C9-C18 at sample location TB-18 that are greater than the NCDEQ
MSCCs for “Soil-to-Groundwater” and less than the “Residential” MSCCs. The Aromatic
C9-C22 concentration was greater than the “Residential” MSCC but less than the
“Industrial-Commercial” MSCC.
•Analytical results for analysis by Method 8260 detected concentrations of P-
Isopropyltoluene and Naphthalene at sample location TB-18 that are greater than the
NCDEQ MSCCs for “Soil-to-Groundwater” and less than the “Residential” MSCC.
•Analytical results for analysis by Method 8270 detected concentrations of 2-
Methylnaphthalene, Naphthalene, and 1-Methylnaphthalene at sample location TB-18
that are greater than the NCDEQ MSCCs for “Soil-to-Groundwater” and less than the
“Residential” MSCC.
•The concentrations reported for the remaining 11 soil samples were below the
corresponding reporting limits and/or the lower of the MSCCs for each analyte.
A copy of the laboratory reports and chain of custody records are included in Appendix III.
5. SITE CHECK REPORT
Based on information reviewed by S&ME, and the completed initial abatement actions, a site
check was not conducted.
6. UST CLOSURE REPORT
This report is submitted in lieu of a UST Closure Report.
7. FREE PRODUCT INVESTIGATION AND RECOVERY REPORT
No free product was encountered or observed during the UST closure or during removal of the
TPH impacted soils.
8. GROUNDWATER INVESTIGATION
Confirmed groundwater was not encountered during these activities.

9. CONCLUSIONS
Based on the results of the UST closure activities, the following site conditions are noted:
1. Two 550-gallon suspect heating oil UST were discovered by the City of High Point prior to
purchase of the subject property. The City of High Point never operated the subject USTs.
2. Two 550-gallon single-wall steel USTs were permanently closed by removed on
November 10, 2015. Based on soil head-space field screenings with the PID, suspect
petroleum hydrocarbons were indicated beneath the USTs.
3. Closure confirmation soil samples collected beneath each UST confirmed a release. TPH-
DRO was detected at concentrations greater than the NCDENR Action Level of 10 mg/kg
TPH in the soil samples collected directly below each UST.
4. Between November 10 and November 23, 2015, approximately 1,859.24 tons of soil was
removed and transported off-site for proper disposal as an initial abatement action.
•Twelve soil samples were collected from the limits of the initial abatement action soil
excavation, and submitted for the analyses using appropriate NCDEQ Risk-Based
analytical methods. Analytical results for these samples reported:
1. Analytical results for analysis by MADEP methods detected concentrations of
Alphatics C5-C8,and Alphatics C9-C18 at sample location TB-18 that are greater
than the NCDEQ MSCCs for “Soil-to-Groundwater” and less than the “Residential”
MSCCs. The Aromatic C9-C22 concentration was greater than the “Residential”
MSCC but less than the “Industrial-Commercial” MSCC.
2. Analytical results for analysis by Method 8260 detected concentrations of p-
Isopropyltoluene and Naphthalene at sample location TB-18 that are greater than
the NCDEQ MSCCs for “Soil-to-Groundwater” and less than the “Residential”
MSCC.
3. Analytical results for analysis by Method 8270 detected concentrations of 2-
Methylnaphthalene, Naphthalene, and 1-Methylnaphthalene at sample location
TB-18 that are greater than the NCDEQ MSCCs for “Soil-to-Groundwater” and
less than the “Residential” MSCC.
4. The concentrations reported for the remaining 11 soil samples were below the
corresponding reporting limits and/or the lower of the MSCCs for each analyte.
•Free-product was not observed.
•Evidence of groundwater was not observed.
