INCIDENT: Management reported that a truck ruptured their fuel tank and is in need of assistance with clean-up. Fuel
was potentially leaked into the storm drain. The fire department was notified. Transferring management to Clean Harbors
for further assistance

Sunday 1/8/2023
10:19 pm: Management reported that a truck ruptured their fuel tank and is in need of assistance with clean-up. Fuel was
potentially leaked into the storm drain. The fire department was notified. Transferring management to Clean Harbors for
further assistance. FD is on site and has put down absorbent. Spill is approx. 15 gallons.
11:00 pm: Crew at branch. Pre-tripped vehicles. Loaded up supplies and equipment needed for response.
11:40 pm: Departed branch, headed to job location.
Monday 1/9/2023
1:45 am: Arrived on site and met with management. Located the work site. Took photos of work site. Held crew safety
meeting. Unloaded equipment and supplies. Donned PPE.
1:55 am: Started cleanup. Crew began by vacuuming firefighting foam and collecting absorbents placed prior to arrival and
deploying additional granular absorbents on the area of the shipping/receiving area as this was the only area not previously
addressed prior to arrival. All absorbents were broomed in and collected and placed into 1-55G steel drum. Crew tested the
opening of one small drain with pads and boom, all of which came out clean. No additional work was recommended
regarding the drain.
4:20 am: Finished cleanup and took photos of the site. Labeled and manifested waste. Loaded equipment and supplies.
4:25 am: Met with management to review work performed and sign paperwork.
4:45 am: Crew departed job site and headed back to the branch.
7:31 am: Crew arrived at branch. Offloaded equipment. Post-tripped vehicles. Restocked supplies. Completed paperwork.
8:00 am: Response complete.
Tuesday 1/17/2023
7:00 am – 5:00pm: Driver brings waste for disposal
Waste Generated: One (1) 55-gallon steel drum was transported off site for disposal by Clean Harbors.
