EXECUTIVE SUMMARY/PRIOR RELEASE SUMMARY The purpose of this report is to present the findings of contaminated soil removal activities for the Maple Hill Truck Fuel Spill (B&B Farms) that occurred on the NCDOT ROW along NC Highway 53 between NC Highway 50 and Old Maple Hill Road (the site – see Exhibit 1). Terracon completed this report to summarize the findings of contaminated soil removal activities performed at the site on December 29, 2022. The site is currently a grassed area and stormwater ditch in the NCDOT ROW along NC Highway 53. The site is depicted in Exhibit 2. A diesel fuel release and trucking accident from a truck owned by B&B Farms occurred on December 13, 2022. Approximately 50 to 70 gallons of diesel fuel is thought to have been released and the fuel release impacted soil along the shoulder of the road and in a drainage ditch in an area approximately 40 feet long by approximately 5 to 10 feet wide. Initial response/abatement activities occurring on December 13, 2022 included the wrecker service that responded to the wreck and release by placing petroleum absorbent materials on the stained soil areas and using soil to berm the stormwater ditch at each end of the spill area. A&J Wrecker Service and Truck Repair, LLC contacted Terracon on December 21, 2022 to provide oversight of the contaminated soil removal activities, regulatory sampling and NCDEQ reporting requirements. Terracon supervised the removal of the contaminated soil on December 29, 2022. On December 29, 2022 approximately 75 tons of contaminated soil was excavated and later hauled to Carlisle Farms, Inc. (NC Permit No. SR0600035) for disposal. During the contaminated soil removal activities, Terracon field screened the soil during removal with a Photoionization Detector (PID) and used visual senses to look for staining and olfactory senses for petroleum odors to determine when to halt the excavation contractor from further excavating soil. The five confirmation soil samples were analyzed by a certified lab for Total Petroleum Hydrocarbons -Gasoline Range Organics (TPH-GRO) by Environmental Protection Agency (EPA) Method 5030 and for Total Petroleum Hydrocarbons -Diesel Range Organics (TPH-DRO) by EPA Method 3550. The five confirmation soil samples were analyzed below the laboratory reporting limits and/or below NCDEQ’s Total Petroleum Hydrocarbon (TPH) Action Levels. Groundwater was not encountered during the excavation activities. The excavation area was backfilled with offsite soil and grass seed and erosion control materials were placed over the backfilled pit on December 29, 2022. C. SITE HISTORY AND CHARACTERIZATION The purpose of this scope of work was to present the findings of contaminated soil removal activities for the Maple Hill Truck Fuel Spill (B&B Farms) that occurred on the NCDOT ROW along NC Highway 53 between NC Highway 50 and Old Maple Hill Road (the site – see Exhibit 1). Terracon completed this report to summarize the findings of contaminated soil removal/soil sampling activities performed at the site on December 29, 2022. This Initial Abatement Action Report was prepared in general accordance with the 15A NCAC 2L .0115 regulations and the NCDEQ Underground Storage Tank (UST) Section Guidelines for Site Checks, Tank Closure and Initial Response and Abatement for UST Releases, May 17, 2021 (Change 11 - 2021), under the supervision of Mr. Carl F. Bonner, PE, a North Carolina registered Professional Engineer (NC Registration No. 16252). C.1 Site Setting The site is currently a grassed area and stormwater ditch in the NCDOT ROW along NC Highway 53 (between NC Highway 50 and Old Maple Hill Road North) in Maple Hill, Pender County, NC. The site is depicted in Exhibit 2. The spill site is in a rural area with two large parcels (between approximately 120 and 200 acres) of undeveloped woodland to the north, south, east and west of the site. The closest structure is a residence located approximately 1,200 feet southwest of the release area (residence addressed as 15651 NC Highway 53 West). Adjoining properties nearest the site are depicted in Exhibit 2. C.2 Release Summary On-site Releases Besides the current release occurring in December 2022, no other reported releases or incidents were identified on the site on NCDEQ’s Waste Management Site Locator Tool. Initial Abatement Action Report Maple Hill Truck Fuel Spill (B&B Farms) Terracon Project No.: 72227146 NCDEQ Incident No.: 96891 Report Date: January 31, 2023 Responsive ■ Resourceful ■ Reliable 4 Off-site Releases Based on our review of NCDEQ’s Waste Management Site Locator Tool, no off-site releases or off-site reported incidents were identified on properties within approximately 2,000 feet off the site. The closest reported incident appears to be a Leaking Underground Storage Tank (LUST) incident located approximately 2,100 feet southwest of the site. The LUST incident is NCDEQ Incident Number 32084 that occurred at the Scotchman #1 facility located at 15515 NC Highway 53 West approximately 2,100 feet southwest of the site. C.3 Previous Investigations Prior to the December 2022 truck fuel spill, no prior investigations have occurred at the site that have been reported to NCDEQ. C.4 Water Supply Wells Terracon is not aware of water supply wells that would be in the site area. A receptor survey has not been conducted for the site. C.5 Surface Water A small quantity of rainwater was observed in the ditch that extends through the site area. Sheens were not noted on the water in the ditch. The 1981 Maple Hill, NC topographic map (attached with this report) for the site area indicates that the two large, wooded parcels around the site contain large areas showing swampland symbols. The closest “blue line” stream appears to be an unnamed tributary of Angola Creek. The “blue line” stream is located approximately 1,500 feet northwest of the site. Groundwater was not encountered during the soil excavation activities to clean up the fuel spill. Surface waters are depicted on the USGS Topographic map excerpt presented as Exhibit 1. C.6 Land Use Information Adjoining properties to the site are undeveloped woodland as depicted in Exhibit 2. D. INITIAL RESPONSE AND ABATEMENT ACTION ACTIVITIES On December 13, 2022 after the truck was removed, a wrecker service placed oil absorbent material over the stained soil areas and used soil to berm the ditch at each of the spill area. Contaminated soil removal and confirmation soil sampling activities were conducted on December 29, 2022. The contaminated soil removal activities and confirmation soil sampling is further discussed below. Initial Abatement Action Report Maple Hill Truck Fuel Spill (B&B Farms) Terracon Project No.: 72227146 NCDEQ Incident No.: 96891 Report Date: January 31, 2023 Responsive ■ Resourceful ■ Reliable 5 D.1 Excavation of Contaminated Soil On December 29, 2022, Terracon mobilized to the site along with personnel from our client, A&J Wrecker Service and Truck Repair, LLC, and personnel from Walking Tall Trucking, LLC, a subcontractor for A&J Wrecker Service and Truck Repair, LLC used for performing the contaminated soil excavation and hauling of the contaminated soil to a licensed disposal facility. Walking Tall Trucking, LLC utilized a track hoe to remove contaminated soil in the area of the release and to load the contaminated soil into their dump trucks. On December 29, 2022 approximately 75.7 tons of contaminated soil was excavated and later hauled to Carlisle Farms, Inc. (NC Permit No. SR0600035) for disposal. Copies of the waste manifests and weigh tickets are included in Appendix A. Please see Exhibit 2 in the Exhibits appendix section for a site diagram indicating the site location, soil clean-up location and other pertinent information. During the contaminated soil removal activities, Terracon field screened the soil with a Photoionization Detector (PID), using visual senses to look for staining, and olfactory senses for petroleum odors to determine when to halt the excavation contractor from further excavating contaminated soil. Please see Table 1 in the Table appendix section of this report for a summary of field screening readings. The general locations of the field screening readings are indicated on Exhibit 3. The limits of the soil excavation was approximately 40 feet long by approximately 5 to 10 feet wide by approximately 1 to 2.5 feet deep. Terracon collected five confirmation soil samples from the excavation pit on December 29, 2022. After sample collection, the excavation area was backfilled with offsite soil and grass seed and erosion control materials were placed over the backfilled pit on December 29, 2022. Groundwater was not encountered in the excavation pit during over-excavation activities. D.2 Sampling Activities On December 29, 2022, Terracon collected five confirmation soil samples from the bottom of the excavation pit after contaminated soil over-excavation activities were finalized. The five confirmation soil samples were designated by Terracon as SS-1, SS-2, SS-3, SS-4 and SS-5. The confirmation soil samples were collected a varying depths. Confirmation soil sample locations are provided on Exhibit 4. D.2.1 Free Product Evaluation Free product was not observed during the soil removal activities. Initial Abatement Action Report Maple Hill Truck Fuel Spill (B&B Farms) Terracon Project No.: 72227146 NCDEQ Incident No.: 96891 Report Date: January 31, 2023 Responsive ■ Resourceful ■ Reliable 6 D.2.2 Soil Sampling Activities On December 29, 2022, Terracon collected five confirmation soil samples from the bottom of the excavation pit after contaminated soil over-excavation activities were finalized. The five confirmation soil samples were designated by Terracon as SS-1, SS-2, SS-3, SS-4 and SS-5. The confirmation soil samples were collected a varying depths (between 1 to 2.5 feet below land surface). Confirmation soil sample locations are provided in Exhibit 4. The confirmation soil samples were collected by a gloved hand from the bucket of a track hoe used to remove the contaminated soil. Gloves were changed prior to each sample collection point. The soil samples were packaged and placed in a cooler for delivery to Pace Analytical Services, Inc. (Pace) in Huntersville, North Carolina for analysis. The five confirmation soil samples were sent to Pace Analytical, Inc. to be analyzed for by EPA 3550 for TPH-DRO and EPA 5030 for TPH-GRO. A composite “Stockpile” soil sample was not requested to be collected by Terracon for the soil disposal facility. Terracon understands the soil disposal facility collected their own waste profile sample. D.2.3 Groundwater Sampling Activities Groundwater was not encountered during the soil removal activities. Groundwater samples are not known to have been collected at the site area. D.2.4 Analytical Results Confirmation soil sample SS-1 was analyzed to contain TPH-DRO at 6.9 mg/kg. This TPH-DRO level analyzed in SS-1 is below NCDEQ’s TPH Action Levels. TPH-GRO in SS-1 was analyzed to be below the laboratory reporting limits and below NCDEQ’s TPH Action Level. Confirmation soil samples SS-2, SS-3, SS-4 and SS-5 were analyzed to be below the laboratory method detection limits and below NCDEQ’s TPH Action Level. The soil analytical results for the five confirmation soil samples are summarized in Table 2 in the Table appendix and are compared to NCDEQ’s current TPH Action Levels. The laboratory analytical report and chain of custody are provided in Appendix C. E. CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of the Initial Abatement Action activities performed in December 2022, Terracon provides the following findings: A diesel fuel release and trucking accident from a truck owned by B&B Farms occurred on December 13, 2022. Based on the results of the Initial Abatement Action services conducted in December 2022 and the confirmation soil sample results, Terracon recommends that the incident be closed out with a “Notice of No Further Action” statement.
