The services outlined in this report were performed by our consultant, ATC Group Services LLC of North Carolina, P.C. (ATC), as directed by the North Carolina Department of Environmental Quality (NCDEQ), based on analytical groundwater concentrations being detected greater than their respective Gross Contaminant Levels (GCL) in groundwater samples obtained during the previous assessment activities. On March 24, 2021, ATC conducted groundwater monitoring activities at the site for monitoring wells MW- 01, MW-02, MW-03, and MW-04. Free product was detected in monitoring well MW-02 with a thickness of 0.91 feet. Laboratory analytical results indicated sampled analytes in monitoring wells MW-01, MW-03, and MW-04 did not exceed their respective 2L Standards. Based on these results, Speedway recommends the completion of an aggressive fluid vapor recovery (AFVR) event using monitoring well MW-02 as the extraction point along with quarterly groundwater sampling including natural attenuation parameters. Figure 1 shows the location of the site on part of a United States Geological Survey (USGS) topographic map. The site is currently an active convenience store with retail fuel sales. Figure 2 depicts the site layout including the location of relevant site features. 1.1 Site Background Underground storage tank (UST) system information is summarized in Table 1. One 10,000-gallon gasoline UST, two 6,000-gallon gasoline USTs, and one 6,000-gallon diesel UST are located in a single UST basin. No historical releases or compliance issues are known prior to this incident. Hydrostatic test failures for each of the mentioned UST spill buckets resulted in the submission of North Carolina Department of Environmental Quality (NCDEQ) Form UST-17A on August 28, 2017. On September 12, 2017, Speedway submitted NCDEQ Form UST-17B that included those failed hydrostatic results. Subsequently, on October 23, 2017, ATC oversaw Carolina Soil Investigation utilize direct hydraulic push technology to conduct soil borings, SB-01, SB-02, SB-03, and SB-04. Samples were collected and sent to ESC Lab Sciences for analysis. NCDEQ Form UST-61 was submitted on December 1, 2017, documenting the activities on October 23, 2017. On December 12, 2017, after reviewing the NCDEQ Form UST-61, the NCDEQ issued a Notice of Regulatory Requirements (NORR) requesting that Limited Site Assessment II (LSA II) activities be conducted. LSA II activities included the installation of three monitoring wells with soil and groundwater sampling. The LSA II Report was prepared and submitted to the NCDEQ on June 17, 2019. After reviewing the LSA II Report, the NCDEQ issued a NORR requesting additional assessment with groundwater sampling take place. In August 2020, monitoring well MW-04 was installed offsite. Groundwater gauging of the monitoring well network detected 0.04 feet of free product in monitoring well MW-02. 1.2 Current Activities On January 27, 2021, the NCDEQ issued Task Authorization No. 43094-2 for groundwater monitoring activities to be conducted at the site. The task authorization associated with these activities is attached as Appendix A. Groundwater Monitoring Report Speedway Store No. 8290 May 10, 2021 ATC Group Services LLC 2 2 Groundwater Sample Collection and Analysis On March 24, 2021, the monitoring well network was gauged using an oil/water interface probe prior to groundwater sample collection. Approximately 0.91 feet of free product were detected in monitoring well MW-02. As a result, groundwater samples were not collected from this monitoring well. Groundwater samples were collected from monitoring wells MW-01, MW-03, and MW-04. Recent groundwater elevation measurements are summarized in Table 2, and historical groundwater elevation measurements are summarized in Table 3. The groundwater samples were collected in laboratory-supplied containers, entered into chain-of-custody control, placed on ice, and shipped to Pace Laboratories [12065 Lebanon Road, Mt. Juliet, Tennessee 37122, North Carolina Laboratory Certification Number ENV375/DW21704/BI0041, (615) 758-5858] for benzene, toluene, ethylbenzene, and total xylenes (BTEX), methyl tert-butyl ether (MTBE), naphthalene, 1,2-dichloroethane (1,2- DCA), and di-isopropyl ether analysis by Environmental Protection Agency (EPA) Method 6200B. Groundwater Monitoring Report Speedway Store No. 8290 May 10, 2021 ATC Group Services LLC 3 3 Results 3.1 Site Geology / Hydrogeology The site is located in the Coastal Plain Physiographic Province of North Carolina. Coastal Plain deposits typically consist of Mesozoic to Cenozoic sedimentary rock and soil. More specifically according to Brown, the site is underlain by the Cretaceous Peedee Formation which is characterized by sand, clayey sand and clay, greenish gray to olive black, massive glauconitic, locally fossiliferous and calcareous. There are patches of sandy molluscan-mold limestone in the upper part of the formation. The current depth to groundwater in the existing monitoring wells ranges from 4.03 feet below land surface (bls) in monitoring well MW-01 to 5.32 feet bls in monitoring well MW-02. Groundwater in the Coastal Plain is usually encountered at shallow depths. Natural groundwater quality in the Coastal Plain for the surficial aquifer is the most susceptible to contaminants (NCDENR, 2010). Flow rates are variable due to the confining units between aquifers and the lithology of each individual aquifer system. Data from Table 2 was used to create a groundwater elevation contour map, which is included as Figure 3. Based on the groundwater elevation contour map, groundwater generally flows west, which is consistent with historical groundwater flow. 3.2 Groundwater Analytical Results Results of the most recent laboratory analysis and previous sample event of the groundwater samples collected from the site monitoring wells are summarized in Table 4 and presented in Figure 4. Groundwater analytical results for the samples collected during the current monitoring event indicate the following:  No concentrations were detected above their respective North Carolina 2L Standards in monitoring wells MW- 01, MW-03, and MW-04. A copy of the laboratory analytical report and chain-of-custody documentation is included in Appendix B. Groundwater Monitoring Report Speedway Store No. 8290 May 10, 2021 ATC Group Services LLC 4 4 Findings and Conclusions Based on data collected during the recent groundwater monitoring activities, the following conclusions are made: > Free product was detected in monitoring well MW-02 at a thickness of 0.91 feet; > Sampled analytes in monitoring wells MW-01, MW-03, and MW-04 did not exceed their respective North Carolina 2L Standards. Groundwater Monitoring Report Speedway Store No. 8290 May 10, 2021 ATC Group Services LLC 5 5 Recommendations Based on the findings and conclusions of this report, the following recommendations are made:  Petroleum impacted groundwater concentrations exists in the area of monitoring well MW-02;  An aggressive fluid vapor recovery (AFVR) event should be conducted using monitoring well MW-02 as the extraction point; and  Quarterly groundwater sampling including natural attenuation parameter should be conducted. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description J The identification of the analyte is acceptable; the reported value is an estimate. 1 Cp 2Tc 3Ss 4Cn 5Sr 6Qc 7Gl 8Al 9Sc ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: ATC Group Services - SC - Speedway SPEEDWAY 8290 L1331232 03/30/21 11:29 15 of 17 ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: ATC Group Services - SC - Speedway SPEEDWAY 8290 L1331232 03/31/21 18:40 15 of 17 Pace Analytical National 12065 Lebanon Rd Mount Juliet, TN 37122 Alabama 40660 Nebraska NE-OS-15-05 Alaska 17-026 Nevada TN000032021-1 Arizona AZ0612 New Hampshire 2975 Arkansas 88-0469 New Jersey–NELAP TN002 California 2932 New Mexico ¹TN00003 Colorado TN00003 New York 11742 Connecticut PH-0197 North Carolina Env375 Florida E87487 North Carolina ¹DW21704 Georgia NELAP North Carolina ³41 Georgia ¹923 North Dakota R-140 Idaho TN00003 Ohio–VAP CL0069 Illinois 200008 Oklahoma 9915 Indiana C-TN-01 Oregon TN200002 Iowa 364 Pennsylvania 68-02979 Kansas E-10277 Rhode Island LAO00356 Kentucky ¹ ⁶KY90010 South Carolina 84004002 Kentucky ²16 South Dakota n/a Louisiana AI30792 Tennessee ¹ ⁴2006 Louisiana LA018 Texas T104704245-20-18 Maine TN00003 Texas ⁵LAB0152 Maryland 324 Utah TN000032021-11 Massachusetts M-TN003 Vermont VT2006 Michigan 9958 Virginia 110033 Minnesota 047-999-395 Washington C847 Mississippi TN00003 West Virginia 233 Missouri 340 Wisconsin 998093910 Montana CERT0086 Wyoming A2LA A2LA – ISO 17025 1461.01 AIHA-LAP,LLC EMLAP 100789 A2LA – ISO 17025 ⁵1461.02 DOD 1461.01 Canada 1461.01 USDA P330-15-00234 EPA–Crypto TN00003 ACCREDITATIONS & LOCATIONS ¹ Drinking Water ² Underground Storage Tanks ³ Aquatic Toxicity ⁴ Chemical/Microbiological ⁵ Mold ⁶ Wastewater n/a Accreditation not applicable * Not all certifications held by the laboratory are applicable to the results reported in the attached report. * Accreditation is only applicable to the test methods specified on each scope of accreditation held by Pace Analytical. 1 Cp 2Tc 3Ss 4Cn 5Sr 6Qc 7Gl 8Al 9Sc ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: ATC Group Services - SC - Speedway SPEEDWAY 8290 L1331232 03/30/21 11:29 16 of 17 ACCOUNT:PROJECT:SDG:DATE/TIME:PAGE: ATC Group Services - SC - Speedway SPEEDWAY 8290 L1331232 03/31/21 18:40 16 of 17
