Dear Mr. Parikh:

On January 11, 2021, the Division of Waste Management (DWM) received a Groundwater
Monitoring Report, prepared on your behalf by Henry Nemargut Engineering Services. The report
summarizes groundwater sampling activities conducted at the subject site. The DWM thanks you for this
report.

After review of this report, the DWM is in general agreement with the conclusions and
recommendations proposed. However, additional soil assessment activities are not required at this time.
The following monitoring wells should be sampled during the next sampling event: MW-1, MW-6, MW-
11, MW-13, MW-14, and MW-16. The DWM requests that the next Groundwater Monitoring Report be
submitted to the Division no later than October 1, 2021.

Please submit a pre-approval authorization form before any work has been completed if State Trust
Fund reimbursement is anticipated. If you have any questions regarding trust fund eligibility or
