(Duke) H.F. Lee plant site at 1677 Old Smithfield Road in Goldsboro, North Carolina (Drawing 1.1). Prior to and during plant decommissioning activities five Areas of Concern (AOC) associated with petroleum releases were identified at the subject site. Duke notified the North Carolina Department of Environmental Quality (NCDEQ), then the Department of Environment and Natural Resources (NCDENR), Underground Storage Tank (UST) Section of these releases and it was determined, for safety reasons, that they would be addressed once demolition activities were complete. It was also decided that these releases would be addressed under the facility’s current incident number (93109) as they are associated with a common petroleum fuel storage and transfer system. In 2017 the majority of plant decommissioning activities were completed such that assessment activities could be conducted safely. A site-wide Phase I Limited Site Assessment was conducted to address all five AOCs. These activities were described in a Phase I Limited Site Assessment (LSA) report dated September 1, 2017. Upon review of the Phase I LSA report, NCDEQ issued a Notice of Regulatory Requirements (NORR) dated February 28, 2019. This report is intended to address the supplemental assessment activities outlined in the February 28, 2019 NORR. The opinions included herein are based on our experience and the information obtained during the study. This report is based on limited observations made on the dates noted using the procedures described herein. If additional information becomes available, we request the opportunity to review the information, reassess the potential environmental concerns and modify our conclusions and recommendations, if appropriate. 2.0 SUMMARY OF FIELD ACTIVITIES The February 28, 2019 NORR outlined the supplemental assessment activities as follows: Collect one groundwater sample from monitoring wells MW-104, MW-201 and MW-501 (Drawing 2.1). Test the collected groundwater samples by EPA Methods 8260 and 8270 as well as MADEP VPH and EPH. Collect one soil sample from a depth just above the water table at locations adjacent to monitoring wells MW-104, MW-201 and MW-501. Test each soil sample by EPA Method 8260, EPA Method 8270, MADEP VPH and EPH. On April 29, 2019 Mid-Atlantic personnel mobilized to the subject site to collect the groundwater and soil samples outlined above. Each monitoring well was gauged for the presence of free phase petroleum product with an oil water interface probe capable of detecting oil thickness to 0.01 feet. Free phase product was not detected in any of the wells. The depth to groundwater at each location was then recorded (Table 2.1). A comparison of the 2017 data to the 2019 data shows the water table was 2 to 3 feet higher in 2019. Each well was purged and samples were collected in accordance with the procedures outlined in Appendix A. Mid-Atlantic was successful using a hand auger to collect the soil sample adjacent to MW- 501 at a depth just above the water table (approximately 4 feet below land surface (bls)). However, repeated attempts to collect soil samples adjacent to MW-104 and MW-201 were unsuccessful. At location MW-104, where the water table was approximately 12.5 feet bls, auguring could not advance beyond 4.5 feet bls. This area was previously filled with gravel and the gravel fill would immediately fill the borehole each time the auger was removed. Three separate attempts to advance the boring were attempted and each was unsuccessful. At location MW-201, where the depth to the water table as approximately 7 feet, three hand auger attempts were thwarted at approximately 2.5 feet bls. A very hard packed gravel layer was encountered at this depth. Attempts were made to break the hard pack with a digger bar, but these attempts also proved fruitless. 3.0 LABORATORY ANALYTICAL RESULTS The laboratory analytical report and chains-of-custody record for the soil and groundwater samples collected at the site are provided in Appendix B. As described above, one soil sample was analyzed. The sample had one detection for acetone at 0.0088J mg/kg. This concentration is below the lowest Maximum Soil Contaminant Concentrations (MSCC). It is also likely that acetone is a laboratory artifact. The groundwater test results are summarized in Table 3.1. This table also includes the results from the sampling conducted during the Phase I LSA (January 30, 2017 sample collection date). A comparison of the April 29, 2019 sample date data to the January 30, 2017 sample date data shows a general decline in contaminant concentrations at each of the three wells sampled. During the April 2019 sample event, two compounds, tetrachloroethylene and 1-methylnaphthalene were detected at concentrations above North Carolina Groundwater Quality Standards (NCGQS) in the sample collected from MW-104. In the sample collected from MW-201 the concentration of 1- methylnaphthalene (estimated) and C9-C18 Aliphatics exceeded the NCGQS. There were no detections in the sample collected from MW-501. The concentrations of detected compounds recorded during this sampling event did not exceed Gross Contamination Levels (GCL) or the North Carolina Surface Water Quality Standards (NCSWQS) by a factor of ten. Lee Plant Page 3 Goldsboro, North Carolina 4.0 SUMMARY, CONCLUSIONS AND RECOMMENDATIONS The results of this Supplemental Assessment are summarized as follows:  Due to site conditions, only one of the three planned soil samples could be collected;  Petroleum-related compounds were not detected above laboratory detection limits in the one soil analyzed during this assessment;  Groundwater elevations were approximately 2 to 3 feet higher in April 2019 in comparison to January and March 2017;  Free product was not detected in the site monitoring wells;  Tetrachloroethylene and the 1-methylnaphthalene were detected in excess of the NCGQS in the groundwater sample collected from monitoring well MW-104. These detected concentrations were below GCLs and below ten times the NCSWQS;  Both 1-methylnaphthalene and the aliphatic carbon chain C9-C18 were detected in the groundwater sample collected from the monitoring well MW-201 at a concentration exceeding the NCGQS, but below the GCLs and below ten times the NCSWQS; and  Compounds were not detected above laboratory detection limits in the groundwater sample collected from MW-501. Based on the results of our assessment, Mid-Atlantic concludes the following:  Human exposure to released petroleum, petroleum vapors and petroleum-impacted groundwater at the subject site is unlikely; and  Based on the results of this investigation, and in accordance with 15A NCAC 02L.0506 (2)(a), it appears the site should be classified as “low risk” and the land use should be classified as “industrial”. The DWM UST Section will assign risk and land use classifications. Sample Collection A. Monitoring Wells Prior to sample collection, each well is purged of three to five standing well volumes or to dryness to remove stagnant water from the well and well bore in an effort to collect samples that are representative of the water quality in the formation surrounding each well. Purging is performed either with a new, polyethylene bailer dedicated to each well, or with a decontaminated pump. Samples are retrieved from the monitoring well using the dedicated bailer. New nylon string is used on each dedicated bailer, and new nitrile sampling gloves are donned prior to purging and sampling of each well. B. Geoprobe “Screen Point Sampler” The screen point sampler is a “grab” sampling device that is driven into the saturated zone and a surrounding metal sheath is retracted, exposing a screen. Groundwater entering the screen is then drawn to land surface through disposable tubing that is placed through the hollow push rods. The sample is collected from the tubing into the appropriate sampling glassware. C. Water-Supply Wells Water samples are typically collected from the available spigot that is nearest to the well. The water is allowed to run at a high flow from the spigot for approximately 10 to 15 minutes to allow the water in the delivery lines to be purged. The sample flow is then reduced and the samples are collected directly into pre-labeled containers as described below. New nitrile sampling gloves are donned prior to sampling of each well. D. Treatment System Influent/Effluent Samples are typically collected from the influent or effluent of pump-and-treat groundwater remediation systems using designated sampling ports in the influent and effluent water transport lines. The water is typically allowed to run for several seconds to clear potential debris in the sampling port. The sample is then collected directly into sampling containers as described below. New nitrile sampling gloves are donned prior to sample collection. II. Preparation for Laboratory Analysis Groundwater samples are decanted directly into laboratory-prepared, pre-labeled, sampling containers, packed in ice, and shipped to a certified laboratory under chain-of-custody control. Laboratory analyses performed on the samples are specified on the chain-of-custody.
