Phase I Limited Site Assessment Report Prepared on: September 9, 2022 Johnny’s Convenience Mart Project Manager: Scott Driscoll GRI Project No. 6007 i EXECUTIVE SUMMARY The Johnny’s Convenience Mart site is located at 766 NC Highway 111 in Goldsboro, Wayne County, North Carolina. The property operates as a retail convenience store that uses USTs for storage and resale of fuels. There are one 15,000-gallon and one 8,000-gallon gasoline USTs and associated piping and dispensers are currently in use at the site. The site is covered by asphalt, concrete and grass. A release was previously confirmed at the site in January 1995 during the removal of five USTs. NCDEQ incident number 13668 was assigned. The USTs were owned and operated at the time by E.J. Pope & Son, Inc. as part of the operation of the Handy Mart #25 business. In October 2012, E.J. Pope & Son, Inc. received an NFA for the site. The site met the cleanup requirements for a low-risk site under the risk- based assessment and corrective actions guidelines. A NRP was filed with the register of deeds and public notice requirements were met, closing the incident. In September 2021, NCDEQ conducted a UST compliance inspection at the site. Results of the paperwork audit during the inspection noted a non-passing spill containment bucket test on the regular #1 gasoline UST performed in 2018. The spill buckets were requested to be re-tested by NCDEQ. The spill buckets were re-tested on October 1, 2021, and both the regular and premium spill buckets failed tightness tests. The spill buckets were repaired and tested again on October 6, 2021, and received passing results. Based on this information, NCDEQ requested that a site check be performed. On April 13, 2022, GRI supervised the installation of one Type II monitoring well (GMW-1) adjacent to the southwest end of the UST basin near the spill buckets. A soil sample was collected from the borehole at a depth of 5 feet BGS during the installation of the well. All reported contaminant concentrations were less than the applicable laboratory reporting limits for the soil sample. A ground water sample was collected from the monitoring well and was submitted for laboratory analyses. Concentrations of benzene, naphthalene, tert-amyl alcohol (TAA), tert-butyl alcohol (TBA) and lead that exceeded the MACs were reported in the ground water sample collected from GMW-1. No concentrations were reported above the GCLs and no free product was noted. On August 17, 2022 an updated receptor survey was performed at the site. Public water is available to the subject site and to all surrounding properties through the Eastern Wayne Sanitary District (EWSD), PWS ID NC0496065. Thirteen non-potable wells were identified within 1,500 feet of the site. Calls were placed to EWSD and active water accounts were associated with all the addresses where wells were identified. No water supply wells were identified within a 250-foot radius of the site. EWSD receives its water primarily from the Neuse River, however, the water supply is supplemented through wells. None of the EWSD system wells are located within one mile of the site. The site is not located in a designated wellhead protection area. No surface water bodies were identified within 500 feet of the site. The closest primary residence is located 250 feet west of the site at 119 Forest Knolls Road. 10. What is the distance from the source area of the release to the nearest school, daycare center, hospital, playground, park, recreation area, church, nursing home or other place of public assembly? The Goldsboro School of Ballet is located at 782 NC Highway 111 approximately 200 feet south of the site. 11. What is the zoning status of properties in the surrounding area? According to the Wayne County GIS webpage, the surrounding properties are zoned R-30. The R- 30 District is intended to promote a compatible mixture of single family residential and agricultural use areas. 12. Briefly characterize the use and activities of the land in the surrounding area. The area surrounding the site is a mixture of commercial and residential. Land use information has been included in Figures 2 and 3. 2. RECEPTOR INFORMATION 1. Water Supply Wells: On August 17, 2022, an updated receptor survey was conducted. Public water is available to the subject site and to all surrounding properties through the Eastern Wayne Sanitary District (EWSD), PWS ID NC0496065. Thirteen non-potable wells were identified within 1,500 feet of the site. Calls were made to EWSD and active water accounts were associated with all of the addresses where wells were identified. Public Water Supplies: Are public water supplies available within 1,500 feet of the source area of the release? YES If yes, where is the location of the nearest public water lines and the source(s) of the public water supply. Explain: Municipal water is available to the site and the surrounding properties. Water lines run along the public streets in the area. Public water is supplied by the EWSD. 3. Surface Water: Identify all surface water bodies (e.g., ditch, pond, stream, lake, river) within 1,500 feet of the source area of the release. This information must be shown on the USGS topographic map. One drainage feature and associated low lying area was identified approximately 800 feet north of the site and is identified as Fellows Branch (Figure 2 and 3). 4. Wellhead Protection Areas: Identify all planned or approved wellhead protection areas in the vicinity of the source area. This information must be shown on the USGS topographic map. Wellhead protection areas are defined in 42 USC 300h-7(e). The site is not located in a wellhead protection area. 5. Deep Aquifers in the Coastal Plain Physiographic Region: The site is located in the Coastal Plain Physiographic Region. The surficial aquifer in the vicinity of the site is underlain by the Black Creek Upper confining unit and Black Creek aquifer respectively. 6. Subsurface Structures: No structures with basements were identified in the vicinity of the source area. Underground utilities run along NC-111 and Spring Bank Road. 7. Property Owners and Occupants: The names and addresses of the owners of properties adjacent to the site are presented in Table 4. Property boundaries are shown on Figure 2 and 3. Property cards for the adjacent property owners are attached as Appendix A. 3. SITE GEOLOGY AND HYDROGEOLOGY According to the 1985 Geologic Map of North Carolina, the site is located in the Black Creek Formation. The formation is generally described as greenish gray to black lignitic clays interbedded with lenses of fine grained sands. The surficial aquifer at the site is estimated to be approximately 30 feet thick and underlain by the upper confining unit of the Black Creek and Black Creek aquifer respectively. The site is located approximately 120 feet above mean sea level. On April 13, 2022, GRI supervised the installation of one Type II monitoring well (GMW-1) adjacent to the southwest end of the UST basin. Ground water was encountered at a depth of 9.31 feet below the top of well casing. SITE INVESTIGATION On March 31, 2022, GRI supervised the installation of a soil boring in the vicinity of the non-passing spill buckets to satisfy Site Check requirements. Pea gravel was encountered during the attempt to install the soil boring; therefore, a Type II monitoring well would be necessary to collect the samples to satisfy the site check requirement. On April 13, 2022, GRI supervised the installation of one Type II monitoring well (GMW-1) adjacent to the southwest end of the UST basin. One soil sample was collected during from the borehole during the installation of the well at a depth of 5 feet BGS. The soil sample was submitted for laboratory analyses of VOCs using EPA Method 8260B and VPH using the MADEP Method. A ground water sample was collected from GMW-1 on the same date and was submitted for laboratory analyses of VOCs + oxygenates by EPA Method 6200B, EDB by EPA Method 504.1, total lead by EPA Method 6010 and VPH by the MADEP Method. On August 17, 2022, an updated receptor survey was conducted within 1,500 feet of the site. It was found that public water is available to the subject site and to all surrounding properties through the Eastern Wayne Sanitary District (EWSD); PWS ID NC0496065. Thirteen non-potable wells were identified within 1,500 feet of the site. Calls were made to EWSD and active water accounts were associated with all of the addresses where wells were identified. No water supply wells were identified within a 250-foot radius of the site. EWSD receives its water primarily from the Neuse River, however, the water system is supplemented through wells. None of the EWSD system wells are located within one mile of the site. The site is not located in a designated wellhead protection area. One drainage ditch and associated low lying area, identified as Fellows Branch, lies approximately 800 feet north of the site. 4.1 Previous Soil Sampling – Site Check: All reported contaminant concentrations in the soil sample collected from the borehole of GMW-1 were less than applicable laboratory reporting limits. A soil quality map is presented as Figure 5. A summary of soil sample analytical results is presented in Table 6. An abbreviated summary of the results is below. No concentrations were reported above the GCLs, and no free product was noted at the site. A summary of the analytical results for ground water samples collected on April 13, 2022, is presented in Table 7. A Ground Water Quality Map is included as Figure 6. Please note, no soil or ground water sampling was conducted during the August 2022 LSA activities. 5. CONCLUSIONS AND RECOMMENDATIONS  On September 13, 2021, NCDEQ conducted a UST compliance inspection at the site. Ground water was encountered at a depth of 9.31 feet below the top of well casing. A ground water sample collected from GMW-1 revealed concentrations of benzene, naphthalene, TAA, TBA and lead that exceeded the MACs. No concentrations were reported above the GCLs and no free product was noted.  One soil sample (GMW-1-5’) was collected during the installation of GMW-1 at a depth of 5 feet BGS. All of the reported contaminant concentrations in the soil sample collected from the GMW-1 borehole were less than the applicable laboratory reporting limits for the analyses.  Based on this information, there does not appear to be evidence that the new release at the site would change the risk ranking for the site, and the site should remain a low-risk site. GRI recommends that the incident should be closed. 6. LIMITATIONS This report has been prepared for the exclusive use of JB SZ LLC for specific application to the referenced site in Wayne County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by NCDEQ and with resources adequate only for that scope of work. Site Check Report Prepared on: May 13, 2022 Johnny’s Convenience Mart Project Manager: Scott Driscoll GRI Project No. 6007 ii EXECUTIVE SUMMARY The Johnny’s Convenience Mart site is located at 766 NC Highway 111 in Goldsboro, Wayne County, North Carolina. The property operates as a petroleum retail facility and convenience store. The site is covered by asphalt, concrete and grass. There are one 15,000-gallon and one 8,000-gallon gasoline USTs and associated piping and dispensers in use at the site. One 750-gallon unknown contents, one 1,000-gallon unknown contents, one 3,000-gallon gasoline and two 6,000-gallon gasoline USTs were removed from the site in January 1995. Incident Number 13668 was issued following the January 1995 UST closure activities and was closed in October 2012 upon the filing of a ground water NRP. On September 13, 2021, NCDEQ conducted a UST compliance inspection at the site. Results of the paperwork audit during the inspection noted a non-passing spill containment bucket test on the regular #1 gasoline UST performed in 2018. The spill buckets were requested to be re-tested by NCDEQ. The spill buckets were tested on October 1, 2021, and both the regular and premium spill buckets failed tightness tests. The spill buckets were repaired and tested again on October 6, 2021, and received passing results. Based on this information, NCDEQ requested that a site check be performed. On March 31, 2022, GRI supervised the installation of soil borings in the vicinity of the spill buckets to satisfy the site check requirement. Pea gravel was encountered during the attempt to so install the soil borings; therefore, the installation of a Type II monitoring well and collection of a ground water sample would be necessary to satisfy the site check requirement. On April 13, 2022, GRI supervised the installation of one Type II monitoring well adjacent to the southwest end of the UST basin near the spill buckets. Ground water was encountered at a depth of 9.31 feet below the top of well casing. One soil sample (GMW-1-5’) was collected from a depth of 5 feet during the installation of GMW-1. The soil sample was submitted for laboratory analyses of VOCs by EPA Method 8260B and VPH using the MADEP Method. All of the reported contaminant concentrations were less than the applicable laboratory reporting limits for the soil sample. A ground water sample was collected from the monitoring well, and was submitted for laboratory analyses of VOCs + oxygenates using EPA Method 6200B, EDB by EPA Method 504.1, total lead by EPA Method 6010 and VPH by the MADEP Method. Concentrations of benzene, naphthalene, tert-amyl alcohol (TAA), tert-butyl alcohol (TBA) and lead that exceeded the MACs were reported in the ground water sample collected from GMW-1. No concentrations were reported above the GCLs and no free product was noted. Based on this information, there does not appear to be evidence of a new release at the site that would change the current risk ranking. Results of the paperwork audit during the inspection noted a non-passing spill containment bucket test for the regular #1 gasoline UST that was performed in 2018. The spill buckets were requested to be re-tested by NCDEQ. Superior Services tested the spill buckets on October 1, 2021, and both the regular and premium spill buckets failed tightness tests. The spill buckets were repaired and tested again on October 6, 2021, by Quality Maintenance Service with passing results. Based on this information, NCDEQ requested that a site check be performed. 5. Description of the Site Characteristics: The Johnny’s Convenience Mart site is located at 766 NC Highway 111 in Goldsboro, Wayne County, North Carolina (Figure 1). The property operates as a petroleum retail facility and convenience store. The site is covered by asphalt, concrete and grass. One 15,000-gallon gasoline, and one 8,000-gallon gasoline USTs and associated product piping and dispensers are operated at the site. One 750-gallon unknown contents, one 1,000-gallon unknown contents, one 3,000-gallon gasoline and two 6,000-gallon gasoline USTs were removed from the site in January 1995. The surrounding properties are mainly residential and light commercial/business. A Site Map has been included as Figure 2. B. SITE CHECK PROCEDURES 1. Summary of Site Check Procedure: On March 31, 2022, GRI supervised the installation of a soil boring in the vicinity of the non-passing spill buckets in order to satisfy the Site Check requirements. Pea gravel was encountered during the attempt to so install the soil borings; therefore, a Type II monitoring well would be necessary to take the samples to satisfy the site check requirements. On April 13, 2022, GRI supervised the installation of one Type II monitoring well adjacent to the southwest end of the UST basin near the spill buckets. Ground water was encountered at a depth of 9.31 feet below the top of well casing. Well construction information is contained in Table 3. The location of the well is shown on Figure 2. A well construction record and boring log are included in Appendix B. A copy of the site specific Health and Safety Plan is included as Appendix C. 2. Field Screening: The soil sample collected during the April 13, 2022 Site Check activities was not screened with a PID. 2. Soil Sampling Information: One soil sample (GMW-1-5’) was collected during the installation of GMW-1 at a depth of 5 feet BGS on April 13, 2022, and was submitted for laboratory analyses of VOCs using EPA Method 8260B and VPH using the MADEP Method. All of the reported contaminant concentrations were less than the applicable laboratory reporting limits for the analyses. A soil quality map is presented as Figure 3. A summary of soil sample analytical results is presented in Table 4. The laboratory report is provided as Appendix D. An abbreviated summary of the soil analytical results is below. Ground water Sampling Information A ground water sample was collected from GMW-1 on April 13, 2022, and was submitted for laboratory analyses of VOCs + oxygenates by EPA Method 6200B, EDB by EPA Method 504.1, total lead by EPA Method 6010 and VPH by the MADEP Method. Concentrations of benzene, naphthalene, tert-amyl alcohol (TAA), tert-butyl alcohol (TBA) and lead that exceeded the MACs were reported in the ground water sample collected from GMW-1. No concentrations were reported above the GCLs, and no free product was noted at the site. A summary of the analytical results for ground water samples collected on April 13, 2022, is presented in Table 5. A Ground Water Quality Map based on the April 2022 data is included as Figure 4. Quality-Control Measures: Decontaminated drill and sampling barrels and clean, disposable nitrile gloves were used for the collection of the soil sample and ground water samples. The samples were collected in laboratory supplied containers and strict chain-of-custody procedures were followed. D. SUMMARY AND CONCLUSIONS  On September 13, 2021, NCDEQ conducted a UST compliance inspection at the site. One soil sample (GMW-1-5’) was collected during the installation of GMW-1 at a depth of 5 feet BGS.  All of the reported contaminant concentrations in the soil sample collected from GMW-1-5’ were less than the applicable laboratory reporting limits for the analyses.  Concentrations of benzene, naphthalene, TAA, TBA and lead that exceeded the MACs were reported in the ground water sample collected from GMW-1 on April 13, 2022. No concentrations were reported above the GCLs and no free product was noted.  Based on this information, there does not appear to be evidence of a new release at the site that would change the current risk ranking. In order to confirm the risk ranking, an updated receptor survey could be completed prior to NCDEQ issuing a NFA for the site. E. LIMITATIONS This report has been prepared for the exclusive use of JB SZ LLC for the specific application to the referenced site in Wayne County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by NCDEQ and with resources adequate only for that scope of work.
