EXECUTIVE SUMMARY
The Speedway #8262 site is located at 10816 North Carolina Highway 55 in Grantsboro, Pamlico County,
North Carolina. The property operates as a petroleum retail facility and convenience store. The site is
covered by concrete and grass. There are one 10,000-gallon gasoline, two 6,000-gallon gasoline, one
6,000-gallon diesel, and one 6,000-gallon kerosene USTs currently in use at the site with nine associated
dispenser islands.
On March 11, 2021, during annual compliance testing activities, ground water was discovered in the STP
sump associated with the 10,000-gallon gasoline UST. Therefore, a site check was required by the NCDEQ.
Repairs were completed on the STP sump and passing hydrostatic testing results were received on March
11, 2021.
On April 13, 2021, GRI conducted a site check in the area of the 10,000-gallon gasoline STP sump. One
monitoring well (MW-20) was advanced to approximately 12 feet BGS, adjacent to the STP Sump. One soil
sample (MW-20-3’) was collected at three feet BGS. No petroleum constituent concentrations were
detected above laboratory reporting limits in soil sample MW-20-3’.
A ground water sample was collected from monitoring well MW-20 on April 13, 2021. The MAC was below
the concentration of lead detected in ground water sample MW-20. All other reported constituents were
below applicable standards.
A previous release was documented at the site in 1995. That incident was closed with a NRP in 2011. Due
to the lack of petroleum constituents detected in ground water sample MW-20, the detected lead
concentration is likely naturally occurring, and not associated with a petroleum release. Based on this
information, GRI recommends no further action for this site.

A. SITE HISTORY AND CHARACTERIZATION
1. UST Owner and Operator Information:
In accordance with the Site Check Report guidelines, the UST owner and operator information has been
summarized in the attached Table 1.
2. UST System Information:
In accordance with the Site Check Report guidelines, the UST system information has been summarized
in the attached Table 2. There are one 10,000-gallon gasoline, two 6,000-gallon gasoline, one 6,000-
gallon diesel, and one 6,000-gallon kerosene USTs currently in use at the site, with six gasoline and
three diesel dispenser islands. A copy of the NCDEQ Registered Tank Report is shown below:
3. Non-UST Information:
There are currently no ASTs and no known ASTs formerly at the site.
4. Comprehensive Description of the Release:
Annual compliance testing was completed on March 11, 2021. Based on observations in the field, ground
water breached the sidewall of the STP sump for the 10,000-gallon gasoline UST. The STP sump was
repaired and received passing compliance test results on March 11, 2021. A copy of the UST Suspected
Release 7 Day Notice (UST-17B) form is included as Appendix A.
A site check was required according to the NCDEQ. On April 13, 2021, one monitoring well (MW-20) was
advanced to approximately 12 feet BGS, adjacent to the STP sump. One soil sample and one ground water
sample were collected from monitoring well MW-20. Upon receipt of the laboratory analytical results on
April 27, 2021, a potential release was confirmed at the site. A copy of the UST-61 Form has been included
as Appendix B.

5. Description of the Site Characteristics:
The Speedway #8262 site is located at 10816 North Carolina Highway 55 in Grantsboro, Pamlico County,
North Carolina. A Topographic Map has been included as Figure 1. The property operates as a petroleum
retail facility and convenience store. The site is covered by concrete and grass. There are one 10,000-
gallon gasoline, two 6,000-gallon gasoline, one 6,000-gallon diesel, and one 6,000-gallon kerosene USTs
currently in use at the site, with six gasoline and three diesel dispenser islands. The surrounding
properties are commercial and residential. A Site Map has been included as Figure 2.
B. SITE CHECK PROCEDURES
1. Results of Tank and Line Tightness Testing:
On March 11, 2021, ground water was discovered in the STP sump associated with the 10,000-gallon
gasoline UST. The STP sump was repaired and received passing hydrostatic test results on March 11, 2021.
A copy of the retesting result is provided as Appendix C.
2. Summary of Site Check Procedure:
GRI performed a site check on April 13, 2021, that included the installation of one monitoring well and
the collection of one soil and one ground water sample. The monitoring well was installed to 12 feet BGS
adjacent to the STP sump associated with the 10,000-gallon gasoline UST. A soil sample (MW-20-3’) was
collected during well installation at three feet BGS. The monitoring well was completed with two feet of
riser and 10 feet of screen. Ground water equilibrated at approximately 3.05 feet BGS. The soil boring log
and well construction record for MW-20 are provided as Appendix D.
3. Soil Excavation Activities:
No soil excavation activities were conducted.
C. SITE INVESTIGATION
1. Field Screening:
Soil sample (MW-20-3’) collected on April 13, 2021, was not screened with a PID for VOCs.
2. Soil Sampling Information:
Soil sample (MW-20-3’) was collected on April 13, 2021, and submitted for laboratory analyses of GRO by
EPA Method 8015D and BTEX by EPA Method 8260B. No petroleum constituent concentrations were
detected above laboratory reporting limits in soil sample MW-20-3’. No free product was noted at the
site. A soil quality map is presented as Figure 3. A summary of soil sample analytical results is presented
in Table 3. The laboratory analytical report is provided as Appendix E. A summary of the soil sampling
data from the April 2021 Site Check is shown below:

3. Ground Water and Surface Water Sampling Information:
A ground water sample was collected from monitoring well MW-20 on April 13, 2021, and submitted for
analysis of VOCs by EPA Method 6200B, EDB by EPA Method 504.1, total lead by EPA Method 6010 and
VPH by the MADEP Methods. The MAC was below the concentration of lead detected in the ground water
sample collected from MW-20. All other reported constituents were below applicable standards. A ground
water quality map is presented as Figure 4. Well construction details and gauging information is
presented in Table 4. A summary of ground water sample analytical results is presented in Table 5. The
laboratory analytical report is provided as Appendix E. A summary of the ground water sampling data
from the April 2021 Site Check activities is shown below:

4. Quality-Control Measures:
Clean, disposable nitrile gloves were used for the collection of the soil samples. The samples were
collected in laboratory supplied containers and shipped to the laboratory in a cooler following proper
chain-of-custody procedures. A laboratory supplied trip blank was included in the sample cooler. No
detectable concentrations of requested method constituents were reported in the trip blank.
D. SUMMARY AND CONCLUSIONS
The site is commercial and surrounding properties are commercial and residential.
On March 11, 2021, during annual compliance testing activities, ground water was discovered in
the STP sump associated with the 10,000-gallon gasoline UST. The STP sump was repaired and
received passing compliance test results on March 11, 2021.
A Site Check was conducted on April 13, 2021, to determine if a petroleum release had occurred.

E. LIMITATIONS
This report has been prepared for the exclusive use of Speedway, LLC for the specific application to the
referenced site in Pamlico County, North Carolina. The assessment was conducted based on the scope of
work and level of effort specified by the NCDEQ and with resources adequate only for that scope of work.
Our findings have been developed in accordance with generally accepted standards of geology and
hydrogeology practices in the State of North Carolina, available information, and our professional
judgment. No other warranty is expressed or implied.
The data that are presented in this report are indicative of conditions that existed at the precise locations
sampled and at the time the samples were collected. In addition, the data obtained from samples would
be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No
additional information can logically be inferred from these data.

Ø This form must be used to report to DEQ, within 7 days of discovery, the investigation results for a Suspected Release from any of the following:
a)Failed or inconclusive leak detection or tightness test results. This includes positive test results, such as “Increase”, which indicates the level in the
UST system has increased more than 0.2 gph.
b)Unusual operating conditions which can include, but are not limited to, the erratic behavior of dispensing equipment; the unexplained presence of
water in the tank; the presence of fuel in containment sumps or interstitial spaces; or the degradation of any equipment or element of an underground
storage tank system to the point where that equipment or element can not reasonably be expected to perform its intended function.
c)Internal inspection results such as, perforations, corrosion holes, weld failures, or other similar defects that indicate a release could have occurred.

1 Conduct a tank and/or line tightness test and attach the results, along with all of the supporting data sheets for the test method, to this form for submittal.
The summary page listing the results by itself is not acceptable. If the tightness test results are fail,then you must repair or replace your UST system and
conduct a site check in accordance with 15A NCAC 2N .0603.
2 Provide sales receipts or other comparable documentation that shows fuel was being dispensed from the UST during the test.
3 Provide invoices (with a description of the work completed) or other documentation that shows the equipment has been repaired.
4 Correct your leak detection results using the correct tank chart and submit copies with this form.
5 Repair the UST in accordance with the manufacturer’s instructions or a national standard (e.g., PEI API 1631) and conduct a site check in accordance with
15A NCAC 2N .0603. If the UST is not repaired, then it must be permanently closed in accordance with the latest version of the UST Sections GuidelinesforTank Closure.
6 Correct your leak detection results using the amount your meter was adjusted following calibration and submit copies of the meter calibration and leak
detection results with this form.
7 Conduct a tightness test (e.g.,hydrostatic test) of the sump and submit the test result along with this form. If the test results are fail, then you must repair
or replace the containment sump and conduct a site check in accordance with 15A NCAC 2N .0603.
8 Submit documentation from your UST equipment contractor explaining how they determined the UST was tilted. Correct your leak detection results using a
tank chart adjusted for the tilt and submit with this form.
Note: Tank, piping, spill bucket and containment sump replacements must be done in accordance with 15A NCAC 2N .0900

Inspection of RUL tank 1 STP identified the sidewall was breached. Groundwater penetrated the sump. The sump has
been repaired and a passing hydro test was completed. See attached. A site check was also completed, results are pend-

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
(dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry report basis for soils].
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
RDL (dry)Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)
Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and
Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be
detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
J4 The associated batch QC was outside the established quality control range for accuracy.
J5 The sample matrix interfered with the ability to make any accurate determination; spike value is high.
V The sample concentration is too high to evaluate accurate spike recoveries.
