GRI Project No. 5370 i EXECUTIVE SUMMARY The Grantham Supply & Supermarket site is located at 3388 US 13 South in Goldsboro, Wayne County, North Carolina. The property operates as a petroleum retail facility, hardware and convenience store. The site is covered by concrete and asphalt. One 8,000-gallon gasoline, one 8,000-gallon off-road diesel and one 8,000-gallon highway diesel USTs are currently in use at the site with three associated dispensers. On June 18, 2018, during annual compliance testing activities, the regular gasoline tank #1 spill bucket failed tightness testing. A liner was installed and the spill bucket passed tightness testing on August 13, 2018. A site check was required by NCDEQ following the June 2018 compliance testing. Two previous incidents (13426 and 16371) have been documented at this site. The site risk classification was ranked as low risk and both incidents were closed with the filing of a Notice of Residual Petroleum (NRP) in May 2002. On April 17, 2020, GRI conducted a site check. A ground water sample was collected from a preexisting monitoring well (RW-1) located within the UST basin. The Maximum Allowable Concentrations (MACs) as specified in T15A NCAC 2L.0202 were below the concentration of requested method constituents reported in the ground water sample collected from RW-1. All of the reported contaminant concentrations were below the Gross Contamination Levels (GCLs). Based on this information, the ground water contamination is consistent with previous releases documented at the site. UST Owner and Operator Information: In accordance with the Site Check Report guidelines, the underground storage tank (UST) owner and operator information has been summarized in the attached Table 1. 2. UST System Information: In accordance with the Site Check Report guidelines, the UST system information has been summarized in the attached Table 2. There are one 8,000-gallon gasoline UST, one 8,000-gallon off-road diesel and one 8,000-gallon highway diesel USTs currently in use at the site with two associated dispensers. One 1,000-gallon kerosene UST was removed from the site in December 1994. A copy of the NCDEQ Registered Tank Report is shown below: 3. Non-UST Information: There are currently no above ground petroleum storage tanks (ASTs) and no known ASTs formerly at the site. 4. Comprehensive Description of the Release: The spill bucket associated with the 8,000-gallon regular #1 gasoline UST failed tightness testing during triennial compliance testing conducted on June 18, 2018. A liner was installed and the spill bucket passed integrity testing on August 13, 2018. A Notice of Violation directing a site check was issued by NCDEQ on March 23, 2020. Copies of NCDEQ correspondence are included as Appendix A. Two previous incidents (13426 and 16371) been documented at this site. The site was raked as low risk and both incident closed with the filing of a Notice of Residual Petroleum (NRP) in May 2002. On April 17, 2020, a ground water sample was collected from a preexisting monitoring well located in the UST basin. Description of the Site Characteristics: The Grantham Supply & Supermarket site is located at 3388 US 13 South in Goldsboro, Wayne County, North Carolina. (Figure 1). The property operates as a petroleum retail facility, hardware and convenience store. The site is covered by concrete and grass. There are one 8,000-gallon gasoline, one 8,000-gallon off-road diesel and one 8,000-gallon highway diesel USTs currently in use at the site with three associated dispensers. The surrounding properties are commercial, agricultural and public institutional. A Site Map has been included as Figure 2. B. SITE CHECK PROCEDURES 1. Results of Tank and Line Tightness Testing: According to available information, the spill bucket was repaired and passed integrity testing on August 13, 2018. 2. Summary of Site Check Procedure: GRI performed a site check on April 17, 2020, that included the collection of a ground water sample from an existing monitoring well (RW-1) located within the UST basin. 3. Soil Excavation Activities: No soil excavation activities were conducted. C. SITE INVESTIGATION 1. Field Screening: No Field screening was conducted during the April 2020 site check. 2. Soil Sampling Information: A soil sample was not collected on April 17, 2020 site check. 3. Ground Water and Surface Water Sampling Information: On April 17, 2020, RW-1 was gauged, purged and sampled. Depth to ground water was measured at 4.84 feet bgs. A ground water sample was collected form a preexisting monitoring well RW-1 located within the UST basin. The ground water sample was submitted for laboratory analyses of volatile organic compounds (VOCs) and 5-oxygenates by EPA Method 6200B, semi-VOCs by EPA Method 610 + 10 highest tentatively identified compounds, EDB by EPA Method 504.1, lead by EPA Method 6010B, as well as VPH and EPH using the MADEP Methods. The Maximum Allowable Concentrations (MACs) as specified in T15A NCAC 2L.0202 were below the concentrations of benzene, naphthalene, TAA, TBA, 1- methylnaphthalene, C5-C8 aliphatics, C9-C10 aromatics and C11-C22 aromatics reported in the ground water sample collected from RW-1. All of the reported contaminant concentrations were below the Gross Contamination Levels (GCLs). No free product was observed at the site. A ground water quality map is presented as Figure 3. Quality-Control Measures: Clean, disposable nitrile gloves and a new disposable bailer were used in the collection of the ground water sample. The sample was collected in laboratory supplied containers and shipped to the laboratory in a cooler following proper chain-of-custody procedures. D. SUMMARY AND CONCLUSIONS  The site is commercial and surrounding properties are commercial, agricultural and public institutional.  On June 18, 2018, during triennial compliance testing activities, the regular gasoline tank #1 spill bucket failed tightness testing. A site check was required by NCDEQ following the June 18, 2018, compliance testing. The non-passing UST spill bucket was repaired and passed integrity testing on August 13, 2018.  Two previous incidents (13426 and 16371) been documented at this site. The site was raked as low risk and both incidents were closed with the filing of a NRP in May 2002.  A Site Check was conducted on April 17, 2020, which consisted of collecting a ground water sample from a preexisting monitoring well located within the UST basin.  The MACs were below the concentrations of benzene, naphthalene, TAA, TBA, 1- methylnaphthalene, C5-C8 aliphatics, C9-C10 aromatics and C11-C22 aromatics reported in the ground water sample collected from RW-1. All of the reported contaminant concentrations were below the GCLs.  Based on this information, the ground water contamination is consistent with previous releases documented at the site. With a NRP already in place, no further assessment is warranted for this incident. E. LIMITATIONS This report has been prepared for the exclusive use of Grantham Supply & Supermarket for the specific application to the referenced site in Wayne County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by NCDEQ and with resources adequate only for that scope of work. Grantham: On March 10, 2020, I conducted a compliance inspection at the above-referenced facility. I observed that underground storage tanks (USTs) at Grantham Supply & Supermarket are not in compliance with North Carolina UST regulations (North Carolina Administrative Code [NCAC] 2N "Criteria and Standards Applicable to Underground Storage Tanks). According to those rules (15A NCAC 2N .0203) Grantham Supply & Supermarket is identified as the owner and/or operator of UST(s) at Grantham Supply & Supermarket. The following violations of the state rules (15A NCAC 2N) were observed on March 10, 2020 at Grantham Supply & Supermarket. Following each violation are the actions required to correct the violations: Violation 1: 15A NCAC 2N .0504 LD17; Failure to conduct 0.2 gallons per hour leak rate tests, according to the automatic tank gauge requirements of federal regulation 40 CFR 280.43(d)(1) (as incorporated by 15A NCAC 2N .0504) at least every 30 days as required by federal regulation 40 CFR 280.41(a) (as incorporated by 15A NCAC 2N .0502). Comment: Tank #1 Reg, Hwy Diesel, #3 Off Rd: November and December 2019 tank leak detection records were not available for review. Corrective Action(s): 1) The Division of Waste Management UST Section has determined that the 0.2 gallons per hour leak rate tests have not been conducted for the subject tank(s). The 0.2 gallons per hour leak rate tests are required to be conducted at least once a month. Within 30 days of receipt of this notice, submit copies of the tests for the month following the date of receipt of this notice to the inspector at the address provided. Please include in your response the date that monthly automatic tank gauge testing has been initiated. The Division of Waste Management UST Section recommends that you conduct tests as often as possible during the month to ensure that you will obtain tests that are conducted within the testing parameters established by the automatic tank gauge system's third party standard evaluation. Violation 2: 15A NCAC 2N .0603 RLS7; Failure to investigate a suspected release in accordance with federal regulation 40 CFR 280.52 (as incorporated by 15A NCAC 2N .0603) after a failing tightness test of the spill containment, the presence of fuel in spill containment buckets or the degradation of the spill containment equipment to the point where that equipment can not reasonably be expected to perform its intended function was observed. Comment: Tank #1 Reg: Regular spill bucket failed the integrity test completed on 6-18-2018. Liner was installed and passed on 8-13-2018. Corrective Action(s): Tank owners or operators must investigate a suspected release as directed below. 1) The spill containment bucket failed a tightness test or the condition of the spill containment bucket has deteriorated such that it may need repairing or replacing. All replaced spill buckets must meet the secondary containment requirements of 15A NCAC 2N .0900. You must complete the actions listed below. If any spill containment bucket fails the tightness test, you must complete all of the following tasks: (a) Conduct a site check around the spill containment bucket for the tanks listed above in accordance with 40 CFR 280.52(b) (as incorporated by 15A NCAC 2N .0603) using the sampling protocol and methodology of the most recent version of the UST Section Guidelines for Site Checks, Tank Closure, and Initial Response and Abatement and submit, within 30 days of receipt of this notice, a site check report or initial abatement report (whichever is applicable) to the inspector at the address provided Notice of Deficiency: Deficiency 1: NCGS 143-215.94NN-SS OPTR4; Failure to maintain compliance with UST regulations which requires Primary Operator retraining as required by NCGS 143-215.94NN - 143.215.94SS. Corrective Action(s): Please perform the following corrective action: 1) Within 30 days of receipt of this notice, submit to the inspector at the address provided documentation that shows the other violations listed on this notice have been corrected. If they are not corrected within the next 30 days, then the Primary Operator designated for this facility must go to the following website http://tankschool.nc.gov and register for one of the next Tank School courses being conducted over the next 90 days. Tank School must be completed and a Primary Operator training certificate achieved within 90 days of receipt of this notice. Corrective actions must be completed and reported to the inspector at the address provided within 30 days of receipt of this notice, unless otherwise noted in one of the corrective actions listed above. Assessment of civil penalties may be recommended for violations described in this NOV, as well as, operating permit revocation/denial unless the violations are corrected.
