Hillcrest Drive, Goldsboro, North Carolina Cedar Rock Project Number 256022590 Dear Mr. Perlungher: Cedar Rock Environmental, P.C. (Cedar Rock) is pleased to submit this report describing activities and results for the in-place abandonment of the onsite UST located at 222 S. Hillcrest Drive in Goldsboro, North Carolina. The UST abandonment was performed on September 25, 2018. Included in this report are project documents consisting of a disposal manifest and photographic documentation. This report should be kept with other documents pertaining to the residential property. Soil contamination was observed during a site inspection performed on September 19, 2018. The release was reported to the Department of Environmental Quality (DEQ) using a UST-61 form on September 27, 2018.  Currently, the property cannot be legally conveyed until a Notice of Residual Petroleum (NRP) is filed with the Wayne County Register of Deeds office pursuant to NCGS 143B- 279.11 (d).  The NRP would contain applicable land use restrictions to minimize the risk of exposure to existing contamination.  The land use restrictions prohibit any disturbance of site soils within a specified radius of the estimated extent of soil contamination and prohibit the future operation or installation of any water supply wells within 150 feet of the petroleum release.  Further, the State will not issue a Notice of No Further Action (NFA) letter to officially close out the release incident for “High Risk” properties such as this wherein soil or groundwater contamination exceeding applicable standards remains onsite. The property would be reclassified as “Low Risk” if the water supply well was properly abandoned, and the contaminated soil within the crawlspace were remediated to the point where petroleum vapors were abated. Once reclassified to “Low Risk”, the DEQ would issue a “Conditional” NFA letter to formally close out the release incident.  The “Conditional” provision in the NFA letter will be removed upon receiving a certified copy of the filed NRP and the completion of Public Notification requirements.  Filing the NRP or abandoning the water supply well would not be required if all soil contamination exceeding applicable standards has been removed. Robert A. Payne, L.G. Geologist 1 1.0 INTRODUCTION Cedar Rock Environmental, P.C. (Cedar Rock) was contacted by Mr. Richard Perlungher of Goldsboro, North Carolina to abandon in-place an underground storage tank (UST) located at 222 S. Hillcrest Drive in Goldsboro, North Carolina. The UST was first inspected on September 19, 2018. An inactive UST was observed to be located parallel to and centered approximately 7.0 feet off the southeastern foundation wall and about 16.6 feet northeast of the southern corner of the house. The fill and vent pipes of the UST were observed to be intact with the fill pipe at ground level. The depth to the top of the UST is approximately 18 inches below land surface (bls). The UST has a length of about 63 inches with a diameter of approximately 35 inches and a capacity of about 270 gallons. The UST was measured to be filled to capacity. White flagging was installed to mark the ends of the UST. No other USTs on the property were discovered using a metal detector to survey the outside perimeter of the residence where USTs are usually located. A slight petroleum odor was noted inside the house. Upon entering the crawlspace, standing water and fuel oil were observed with a strong petroleum odor. An area of approximately ten feet by ten feet around the UST has also been impacted with fuel oil on the surface. Once the orientation of the UST was determined, a soil boring was attempted at the northeastern end of the UST using a 3.25-inch diameter stainless steel hand auger. The soil boring was advanced to a depth of approximately 2.0 feet bls, however, the borehole kept collapsing due to groundwater which prevented further advancement. The property is currently using a municipal water supply for potable water. However, there is a water supply well located on the property about 8.5 feet from the petroleum release. Since this well is located within 150 feet of the petroleum release, the property currently would be classified as “High Risk” by the State. The property could be reclassified to “Low Risk” if the supply well was properly abandoned. 2.0 PROJECT The in-place abandonment of the UST was performed on September 25, 2018. The abandonment consisted of the hand excavation of the soils overlying the southwestern end of the UST to expose the topside of the tank. A hole approximately 1.5 feet in diameter was then cut on the top of the UST to allow for removal of the tank contents, cleaning and visual inspection of the UST interior, and subsequent filling. The tank was pressure washed to remove all residual petroleum sludge. Residual oil in the fuel lines was also removed by vacuum. Approximately 220 gallons of fluid and sludge (including rinsate) were removed from the UST. The fluid disposal manifest for the tank contents is included in Appendix A. After the interior of the UST was cleaned, the tank was then entirely filled with an excavatable flowable fill cement slurry. Once filled, the exposed portion of the UST was then covered with site soils (see Appendix B - Photographic Documentation).
