The services outlined in this report were performed by our consultant, ATC Associates of North Carolina, P.C. (ATC), as directed by the North Carolina Department of Environmental Quality (NCDEQ), based on analytical soil and groundwater concentrations being detected greater than their respective Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCC) and 2L Standards in soil and groundwater obtained during the LSA activities. On July 8, 2019, three, two-inch diameter monitoring wells (identified as MW-01, MW-02, and MW-03) were installed at the site. Concentrations of benzene, toluene, ethylbenzene, total xylenes, naphthalene, C5-C8 Aliphatics, C9-C12 Aliphatics, C9-C10 Aromatics, ethylene dibromide (EDB), and lead were detected greater than their respective 2L Standards in the samples collected from the site monitoring wells. (ATC) to perform Phase II Limited Site Assessment (LSA) activities at the Speedway No. 7956 facility located at 107 South Wilson Street, Fremont Wayne County, North Carolina. Figure 1 shows the location of the site on part of a United States Geological Survey (USGS) topographic map. The site currently operates as a Speedway convenience store with retail gasoline, diesel, and kerosene sales. Figure 2 depicts the site layout including the location of relevant site features. The Phase II LSA activities described herein were initiated based on concentrations of benzene, ethylbenzene, total xylenes, naphthalene, lead, C5-C8 Aliphatics, and C9-C12 Aliphatics greater than their respective 2L Standards in the onsite temporary monitoring well completed during the site check addendum activities. On April 10, 2019, Speedway submitted the LSA Report to the North Carolina Department of Environmental Quality (NCDEQ) that documented the installation and sampling of temporary monitoring well SB- 02. On May 8, 2019, the NCDEQ issued a LSA Report Review Letter stating that three additional monitoring wells should be installed at the site to obtain up-gradient and down-gradient groundwater analytical data and monitoring location points that will establish groundwater flow at the site. A copy of the LSA review letter requesting the Phase II LSA activities is provided in Appendix A. 1.1 UST Owner and Operator Information Two underground storage tanks (USTs) are currently in operation at the referenced facility. The USTs and their appurtenances are owned and operated by Speedway LLC of Enon, Ohio. Information regarding each of the tanks is provided below. The USTs and their appurtenances are in operation at the referenced facility. Surface cover at the site consists of asphalt and concrete. 1.4 AST Information No above ground storage tanks (ASTs) were observed on the site. 1.5 Site History On June 9, 2017, Speedway submitted NCDEQ Form UST-17A, which documented a suspected release at the above referenced site due to the premium unleaded UST spill bucket failing a hydrostatic test on June 9, 2017. On July 7, 2017, Speedway submitted NCDEQ Form UST-17B that included the failed hydrostatic test results. On July 24, 2017, ATC used a hand auger to complete a soil boring (identified as SB-01) to 13 feet below grade near Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC the spill bucket end of the premium unleaded tank. The soil samples were placed in laboratory-supplied containers, entered into chain-of-custody control, placed on ice, and shipped via FedEx to ESC Lab Sciences (ESC) [12065 Lebanon Road, Mt. Juliet, Tennessee 37122, North Carolina Laboratory Certification Number ENV375/DW21704/BI0041, (615) 758-5858] for benzene, toluene, ethylbenzene, total xylenes (BTEX), methyl tert-butyl ether (MTBE), and naphthalene analysis by Environmental Protection Agency (EPA) Method 6200B, and Massachusetts Department of Environmental Protection (MADEP) Method for volatile petroleum hydrocarbons (VPH). Laboratory analytical results detected concentrations of xylenes, naphthalene, and C5-C8 Aliphatics greater than their respective Soil-to-Groundwater Maximum Contaminant Concentrations (MSCC). NCDEQ Form UST-61 was submitted to the NCDEQ on August 18, 2017, documenting the advancement of the soil boring and soil sampling activities conducted on July 24, 2017. Laboratory analytical results indicated concentrations of xylene, naphthalene and C5-C8 Aliphatics exceeded their respective Soil-to-Groundwater MSCC in the soil sample collected from soil boring SB-01. On July 13, 2018, the NCDEQ directed Speedway to complete the Site Check Addendum that included groundwater sample collection in the area of boring SB-01. On August 6, 2018, ATC oversaw ARM Environmental (ARM) install an additional boring, identified as SB-02, adjacent to the boring SB-01 to collect a groundwater sample. ARM installed boring SB-02 using direct push technology to a depth of 15 feet below land surface (bls). A temporary well was constructed of one-inch diameter schedule-40 polyvinyl chloride (PVC) riser with 10 feet of 0.010-inch slotted screen. The borehole annulus was backfilled with No. 2 sand filter pack. Using a water level indicator, ATC measured the depth to groundwater to be 6.09 feet bls in boring SB-02. Subsequently, a groundwater sample was collected from the temporary monitoring well; placed in laboratory-supplied, preserved containers; entered into chain-of-custody; placed on ice; and shipped via FedEx to ESC for analysis of volatile organics by EPA Method 6200B, MADEP VPH analysis, lead analysis by EPA Method 6010D, and ethylene dibromide (EDB) analysis by EPA Method 8011. The well casing and screen were removed from the ground once the groundwater samples were obtained. Laboratory analytical results detected concentrations of benzene, ethylbenzene, total xylenes, naphthalene, lead, C5-C8 Aliphatics, and C9-C12 Aliphatics greater than their respective 2L Standards in sample SB-02; however, no analytes were detected greater than their respective gross contaminant levels (GCLs). Based on these results, the NCDEQ issued a Notice of Reporting Requirements (NORR) requesting LSA activities be conducted. ATC completed LSA activities at the site and recommended further evaluation based on the groundwater concentrations of benzene, toluene, total xylenes, naphthalene, lead, C5-C8 Aliphatics, and C9-C12 Aliphatics being greater than their respective 2L Standards. ATC recommended the installation of three monitoring wells to determine the direction of groundwater flow and determine the horizontal extent of the chemicals of concern. The NCDEQ concurred with ATC’s findings and requested a Limited Site Assessment Phase II be performed at the site. Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC 2 Risk Characterization In accordance with 15A NCAC 2L .0115, a completed Limited Site Assessment Risk Classification and Land Use Form provided by the NCDEQ is included in Appendix B. Answers and explanations included on the form are based on information obtained by ATC during the LSA and Phase II LSA activities. Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC 3 Receptor Information 3.1 Water Supply Wells ATC performed a survey to identify water supply wells within 1,500 feet of the site by visually observing the site and surrounding areas. Water supply well forms were sent to property owners within 1,500 feet of the source area where suspected well structures were observed and to all owners within 500 feet of the source. One water well was confirmed on property approximately 485 feet west from the site at 101 North Vance Street. According to the owner of the property, Mr. Walter Marm, the well is used for irrigation and to fill a swimming pool. Another well was confirmed located at 201 Pine Street, approximately 1,090 feet east from the site. According to the owner, Mr. Fred Johnson, the well is used to fill a small fish pond and for irrigation. Neither of the wells identified is used for potable purposes. Based on the completed and/or returned water supply well surveys, there are no other water supply wells located within 1,500 feet of the site. The water supply well map is included in Appendix C, and the property owners are summarized in Table 2. 3.2 Public Water Supplies ATC contacted Mr. Josh Pulley of the Fremont Water and Sewer Department who confirmed the Town of Fremont is the primary local water provider. The Town has no wells but rather purchases municipal drinking water from the Wayne County Water District. The public water source is a series of wells throughout Wayne County. The closest deep well maintained by the district is approximately four miles west of the site. The well is further discussed in Section 3.4. 3.3 Surface Water ATC reviewed a USGS 7.5-Minute Quadrangle Topographic Map, which is included as Figure 1, to identify surface water bodies in the site vicinity. In addition, ATC conducted a visual reconnaissance for surface water bodies in the area. An intermittent stream, which flows to Great Swamp, was observed approximately 0.6 mile northwest of the site; another intermittent stream, which flows to Nahunta Swamp, was observed approximately 0.64 mile south of the site; and another intermittent stream, which flows to Aycock Swamp, was observed approximately 0.8 mile east of the site. 3.4 Wellhead Protection Areas According the Division of Water Quality Wellhead Protection Website, the site is not located within a wellhead protection program area. However, a wellhead protection area is located approximately 3.8 miles west from the site. The wellhead protection area is due to the presence of Wayne Water District System water well located approximately four miles west from the site. 3.5 Deep Aquifers in the Coastal Plain The risk classification criteria specified in Title 15A NCAC 2L .0115(d) requires an evaluation for the presence of unconfined or semi-confined deep aquifers in the Coastal Plain Physiographic Region which may be used as a source of drinking water. The site is located within the Inner Coastal Plain. Information concerning aquifers in the area is summarized below. Additional information regarding the site geology and hydrogeology is included in Section 4.0. As part of the LSA, ATC reviewed a hydrogeological cross-section obtained from the on-line hydrogeologic framework database maintained by the NCDEQ Division of Water Resources. Based on the hydrogeological cross-section for the region of the site, the surficial aquifer in the site vicinity extends to a depth of approximately 50 feet bls; the Yorktown confining unit extends to approximately 52 feet bls; the Yorktown Aquifer extends approximately 56 feet bls; the Black Creek confining unit extends to 57 feet bls; the Black Creek Aquifer extends to 61 feet bls; the Upper Cape Fear confining unit extends to approximately 70 feet bgs; and the Upper Cape Fear Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC aquifer extends to approximately 93 feet bls. A copy of the hydrogeological cross-section is included in Appendix C. 3.6 Subsurface Structures Prior to drilling activities, One-Call of North Carolina, the public utility locating service, and a private utility locating company were contacted to locate underground utilities at the site. Underground utilities (water, electric, fiber- optic, sewer, telephone) were located on-site; however, the utilities did not intersect the water table. 3.7 Land Use The subject site is located in the Town of Fremont, Wayne County, North Carolina. The site is accessible from South Wilson Street from the southeast and East Wayne Street from the southwest. The site currently operates as a Speedway convenience store with retail gasoline sales. A Land Use Classification Form is included in Appendix B. The properties surrounding the subject facility are zoned GB (General Business) and R-10 (Residential Single Family). Residences and commercial retail properties characterize the surrounding area. Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC 4 Subsurface Investigation 4.1 Site Geology According to the Geologic Map of North Carolina (1985) produced by Philip Brown, et al, the referenced site is located within the Coastal Plain Physiographic Region of North Carolina. More specifically, the site is underlain by the Yorktown Formation: fossiliferous clay with varying amounts of fine-grained sand, bluish gray, shell material commonly concentrated in lenses; mainly in area north of the Neuse River. Based on observations made during soil boring activities, the residual soil at the site consists mainly of orange-tan-brown to gray sandy clays. 4.2 Soil Boring and Sample Collection ATC contracted Taylor Wiseman & Taylor (TWT) and ARM Environmental (ARM) to complete private utility locates, air knifing (soft-dig technology), and soil boring/monitoring well installation activities at the site. On July 8 through 9, 2019, three, two-inch diameter monitoring well (identified as MW-01, MW-02, and MW-03) were installed at the site. The location of monitoring wells is shown on Figure 2. Prior to breaking ground, the area was investigated for potential underground utilities using ground penetrating radar (gpr) and then cleared to a depth of six feet bls using air knifing (soft-dig) technology. After clearing the well location, ARM conducted the soil boring using a hollow stem-auger (HSA) drill rig to advance the borings to a depth of 13 feet bls. Soil from the borings was examined and screened for volatile petroleum hydrocarbons using a photo-ionization detector (PID). The boring logs, along with the NCDEQ Well Construction Records, detailing soil descriptions and PID readings are included in Appendix D. The soil samples were placed in laboratory-supplied containers, entered into chain-of-custody control, placed on ice, and shipped via FedEx to Pace for BTEX, MTBE, naphthalene, and di-isopropyl ether analysis by EPA Method 8260B, and MADEP VPH analysis. 4.3 Soil Analytical Results Results of the laboratory analysis of the soil samples collected during the site check are summarized in Table 3 and shown on Figure 3. The following summary of findings is based on the laboratory analysis of the soil samples collected on July 8, 2019:  Concentrations of benzene were detected greater than the respective Soil-to-Groundwater MSCC in the soil collected from sample MW-01. A copy of the laboratory analytical report and chain-of-custody documentation is included in Appendix E. 4.4 Monitoring Well Installation The borings for monitoring wells MW-01, MW-02, and MW-03 were completed as a two-inch wells that were grouted in place at a depth of 13 feet bls. The monitoring wells were each installed with 10 feet of 0.010 slotted polyvinyl-chloride (PVC) well screen. The annular spaces within the borings were filled with pre-washed silica sand, and a bentonite seal was placed above the sand. The remainder of the boring annulus were filled with grout. The monitoring wells were finished with an eight-inch diameter well vault and a two by two foot concrete pad. Investigatory derived waste (IDW) generated during the monitoring well installation activities was containerized in five, properly labelled and sealed, DOT approved, steel 55-gallon drums. On July 24, 2019, the drums containing the well installation soils and well development water were transported under non-hazardous waste manifest by Chemviron Midwest Inc. for proper disposal. 4.5 Groundwater Sample Collection and Analysis On July 16, 2019, groundwater samples were collected from monitoring wells MW-01, MW-02, and MW-03. The groundwater samples were collected in laboratory-supplied containers, entered into chain-of-custody control, Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC placed on ice, and shipped via FedEx to Pace for BTEX, MTBE, naphthalene, and isopropyl ether (IPE) analysis by EPA Method 6200B; 1,2-dibromoethane (EDB) analysis by EPA Method 504.1; lead by EPA Method 6010D; and MADEP VPH analysis. 4.6 Groundwater Analytical Results Results for laboratory analysis of the groundwater samples collected from monitoring wells MW-01, MW-02, and MW-03 are summarized in Table 4 and are presented in Figure 4. The following summary of findings is based on the laboratory analysis of the groundwater samples collected on July 16, 2019:  Concentrations of benzene, toluene, ethylbenzene, total xylenes, naphthalene, and petroleum constituents as VPH were detected greater than their respective 2L Standards in monitoring wells MW- 01, MW-02, and MW-03;  Concentrations of EDB were detected greater than its 2L Standard in the groundwater samples from monitoring wells MW-01 and MW-03;  Concentrations of lead were detected greater than its 2L Standard in the groundwater samples from monitoring wells MW-01 and MW-02;  Concentrations of some of the chemicals of concern were detected greater than its 10 times 2L Standard in groundwater samples collected from each of the monitoring wells; and  Concentrations of petroleum constituents were not detected greater than their respective GCLs in the groundwater samples collected from the site monitoring well network. A copy of the laboratory analytical report and chain-of-custody documentation is included in Appendix E. 4.7 Site Survey On July 30, 2019, following the well installations, ATC remobilized to the site to conduct a top-of-casing (TOC) site survey on the newly installed monitoring wells. A copy of the potentiometric surface map is included as Figure 5 4.8 Site Hydrogeology On July 16, 2019, gauging of the site monitoring well network was performed using an oil/water interface probe prior to sampling. Depth-to groundwater in site monitoring wells MW-01 through MW-03 ranged from 4.56 feet to 6.04 feet bls. Data from the July 16, 2019, gauging event was used to create a groundwater elevation contour map presented as Figure 5. Free product was not observed in the site wells. Based on elevation data obtained on July 9, 2019, groundwater flow appears to be generally towards the north. Phase II Limited Site Assessment Report Speedway No. 7956 August 21, 2019 ATC Group Services LLC 5 Findings and Conclusions Based on data collected during the recent Phase II activities, the following conclusions are made:  On July 8 through 9, 2019, three, two-inch diameter monitoring wells (identified as MW-01, MW-02, and MW-03) were installed at the site;  Free product was not detected during the Phase II LSA activities;  Concentrations of benzene, toluene, ethylbenzene, total xylenes, naphthalene, and petroleum constituents as VPH were detected greater than their respective 2L Standards in monitoring wells MW- 01, MW-02, and MW-03;  Concentrations of EDB were detected greater than its 2L Standard in the groundwater samples from monitoring wells MW-01 and MW-03;  Concentrations of lead were detected greater than its 2L Standard in the groundwater samples from monitoring wells MW-01 and MW-02;  Concentrations of some of the chemicals of concern were detected greater than its 10 times 2L Standard in groundwater samples collected from each of the monitoring wells; and  Concentrations of petroleum constituents were not detected greater than their respective GCLs in the groundwater samples collected from the site monitoring well network. NATIONWIDE.GLOSSARY OF TERMS Guide to Reading and Understanding Your Laboratory Report The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Abbreviations and Definitions (dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry report basis for soils]. MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description J The identification of the analyte is acceptable; the reported value is an estimate. J3 The associated batch QC was outside the established quality control range for precision. NATIONWIDE.GLOSSARY OF TERMS Guide to Reading and Understanding Your Laboratory Report The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description B The same analyte is found in the associated blank. J The identification of the analyte is acceptable; the reported value is an estimate.
