The adjoining properties are residential and commercial. Municipal water is available to the site and surrounding properties. The site risk classification is currently ranked as Intermediate risk due to the presence of free product at the site. On April 2, 2018, eighteen monitoring wells were gauged, purged and/or sampled. Free product was observed in MW-1, MW-2 and MW-3. Concentrations of requested method constituents that exceeded the Maximum Allowable Concentrations (MACs) specified in T15A NCAC 2L.0202 were reported in the ground water samples collected from MW-4, MW-5, MW-11, MW-12, MW-13, MW-17 and MW-27. The concentration of benzene reported in the ground water sample collected from MW-4 exceeded the Gross Contamination Level (GCL). Based on this information, GRI recommends that additional free product recovery events be conducted in order to remove free product and reduce GCL concentrations at the site. Semi-annual ground water monitoring should continue at the site, the next semi-annual ground water monitoring event is tentatively scheduled for October 2018. A total of twenty-six Type II monitoring wells (MW-1 through MW-5 and MW-7 through MW-25), one temporary monitoring well (TMW-1) and two Type III monitoring wells (MWD-6 and TW-1) have been installed at the site. Please note, MW-15 was destroyed and MW-15R was installed as a replacement well. The site risk classification is currently ranked as Intermediate risk due to the presence of free product at the site. Two potable water supply wells (WSW-1 and WSW-2) were identified within a 1,000-foot radius of the site, however, the wells appear to be up-gradient of the release. Municipal water is available to the site and surrounding properties. Past attempts to connect WSW-1 and WSW-2 to the municipal water supply have been unsuccessful. Free product has historically been encountered in six of the monitoring wells (MW-1 through MWD-6) at the site. The site is located within the designated wellhead protection area for the Town of Plymouth. Assessment activities performed at the site prior to the August 2006 updated receptor survey were completed by past consultants. Although Geological Resources, Inc. (GRI) cannot verify the accuracy of information obtained from previous reports, for the purposes of this report, it is assumed to be correct. Please refer to previous submittals for further historical information regarding the site. 2.0 CURRENT SITE ASSESSMENT The purpose of this report is to present the results of the ground water monitoring event conducted on April 2, 2018. The site is located at 277 US Hwy 64 in Plymouth, Washington County, North Carolina (Figure 1). The activities were conducted in accordance with GRI proposal number 18-017, which was submitted to NCDEQ on January 8, 2018, and approved as Task Authorization No. 13374-016 on February 26, 2018. A Site Map showing the locations of the monitoring wells and structures onsite is included as Figure 2. 2 3.0 GROUND WATER QUALITY On April 2, 2018, eighteen Type II monitoring wells (MW-1 through MW-5, MW-7, MW-8, MW-10 through MW-MW-14, MW-15R, MW-17 MW-20, MW-21, MW-26 and MW-27) were gauged, purged and/or sampled. Monitoring wells MW-1, MW-2 and MW-3 contained 1.52, 0.34 and 0.38 feet of free product, respectively, and were not sampled. The depths to ground water in the Type II monitoring wells during the April 2018 sampling event ranged from 3.69 to 6.19 feet below the top of casings. Ground water elevations in the Type II monitoring wells relative to a temporary benchmark with an assumed datum of 100.00 feet ranged from 92.78 to 96.32 feet. Based on this information, ground water flow direction appears to be convergent toward a hydraulic depression in the vicinity of MW-12. The average horizontal hydraulic across the site was approximately 0.02 feet per foot. A Water Table Surface Map for the April 2018 sampling event has been included as Figure 3. A summary of ground water elevation data is presented in Table 1. Laboratory analyses were performed on the ground water samples collected from the monitoring wells during the April 2018 sampling event for volatile organic compounds using EPA Method 6200B. Concentrations of individual BTEX constituents, MTBE, naphthalene, n-butylbenzene, sec-butylbenzene, 1,2-dichloroethane, isopropylbenzene, IPE, p-isopropyltoluene, n-propylbenzene, 1,2,4-trimethylbenzene and/or 1,3,5- trimethylbenzene that exceeded the Maximum Allowable Concentrations (MACs) specified in T15A NCAC 2L.0202 were reported in the ground water samples collected from MW-4, MW-5, MW-11, MW-12, MW-13, MW-17 and MW-27. The concentration of benzene reported in the ground water sample collected from MW-4 exceeded the Gross Contamination Level (GCL). A Ground Water Quality Map based on data from the April 2018 sampling event has been included as Figure 4. An isoconcentration map for benzene is included as Figure 5. A summary of ground water sample analytical results is presented in Table 2. A complete laboratory report has been included as Appendix A. 4.0 CONCLUSIONS AND RECOMMENDATIONS  The average depth to ground water in the Type II monitoring wells in April 2018 was 4.82 feet. Semi-annual ground water monitoring should continue at the site, the next semi-annual ground water monitoring event is tentatively scheduled for October 2018. 5.0 LIMITATIONS This report has been prepared for the exclusive use of North East Oil Company, Inc. for specific application to the referenced site in Washington County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by NCDEQ and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of environmental practices in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No additional information can logically be inferred from these data. Please note that field data collected prior to the August 2006 updated receptor survey were not obtained under the supervision of GRI. Although the accuracy of these data cannot be verified, for the purpose of this report GRI assumes that they are correct.
