GRI Project No. 4195 i EXECUTIVE SUMMARY The Carolina Market site is located at 501 Old Epsom Road in Henderson, Vance County, North Carolina. The site currently functions as a grocery store. The site is covered by concrete and asphalt. A UST system consisting of two 4,000-gallon gasoline and one 2,000-gallon gasoline USTs, associated dispensers and product piping was removed from the site on May 2, 2017. Land use in the area consists of commercial and residential properties. During UST closure activities, on-site screening of the removed soil confirmed a release of an unknown amount of product had occurred. Concentrations of requested method constituents that exceeded the RALs were reported in soil samples collected during the UST closure activities. Ground water was encountered during the May 2017 UST closure activities. A monitoring well was installed within the former UST basin on January 24, 2023, and a ground water sample was collected in accordance with NCDEQ guidelines. The concentration of naphthalene reported in the ground water sample collected from MW-1 exceeded the MAC. All reported contaminant concentrations were less than the GCLs. On January 24, 2023, a receptor survey was completed at the site. The site is not located within a wellhead protection area. No water supply wells are located within a 1,500-foot radius of the site. Public water is available to the subject site and to all surrounding properties. No surface water bodies were identified within a 1,500-foot radius of the site with the exception of a fenced-in manmade retention pond. Based on this information, the site risk classification should be ranked as low risk. Property Surrounding Source Area of Release The questions below pertain to the area within 1,500 feet of the source area of the release (excludes property containing source area of the release): 9. What is the distance from the source area of the release to the nearest primary or secondary residence (permanent or temporary)? The closest primary residence is at 602 Park Avenue, Located approximately 100 feet East of the former UST basin. 10. What is the distance from the source area of the release to the nearest school, daycare center, hospital, playground, park, recreation area, church, nursing home or other place of public assembly? Iglesia Adventista Del Septimo Dia is a church located at 620 Epsom Road, approximately 480 feet south of the source area at the site. 11. What is the zoning status of properties in the surrounding area? According to the Vance County GIS webpage, the surrounding areas are zoned as high density residential (R6) and general industrial (I2). 12. Briefly characterize the use and activities of the land in the surrounding area. Water Supply Wells: A receptor survey was conducted on January 24, 2023. No water supply wells were identified in the area. 2. Public Water Supplies: Are public water supplies available within 1,500 feet of the source area of the release? YES If yes, where is the location of the nearest public water lines and the source(s) of the public water supply. Explain: Municipal water is available to the site and the surrounding properties. Water lines run along the public streets in the area. Public water is supplied by the City of Henderson. 3. Surface Water: Identify all surface water bodies (e.g., ditch, pond, stream, lake, river) within 1,500 feet of the source area of the release. This information must be shown on the USGS topographic map. There are no surface water bodies other than a man-made retention pond within 1,500 feet of the site. The retention pond is not easily accessible and lies within a fenced area. 4. Wellhead Protection Areas: Identify all planned or approved wellhead protection areas in the vicinity of the source area. This information must be shown on the USGS topographic map. Wellhead protection areas are defined in 42 USC 300h-7(e). There are no wellhead protection areas in the vicinity of the source area. 5. Deep Aquifers in the Coastal Plain Physiographic Region: The site is located in the Piedmont Physiographic Region. 6. Subsurface Structures: No structures with basements were observed in the vicinity of the source area. The water main and other underground utilities run along public streets in the area. 7. Property Owners and Occupants: The names and addresses of the owners of properties adjacent to the site are presented in Table 3. Property boundaries are shown on Figure 2. Property cards for the adjacent properties are attached as Appendix A. 3. SITE GEOLOGY AND HYDROGEOLOGY According to the 1985 Geologic Map of North Carolina, the site is located in the foliated to massive granitic rock formation of the Piedmont Physiographic region. Rock in this formation may also be associated with the Rolesville suite, and/or the Wise and Lemon Springs intrusives. During the initial abatement activities, one Type II monitoring well (MW-1) was installed at the site to a total depth of 12 feet BGS on January 24, 2023. The well was completed with a ten-foot screened interval, set to bracket the water table at the time of installation. The well was installed within the former UST basin. The depth to ground water during the January 2023 gauging event was 0.91 feet below the top of well casing. The monitoring well location is shown on Figure 3. Well construction and gauging information is presented in Table 4. Copies of the well permit, well construction record and boring log are included as Appendix B. 4. SOIL SAMPLING No soil samples were collected during the LSA activities due to the shallow depth to water within the former UST basin. The results of soil samples collected during the May 2017 UST closure activities were included in the IAAR. A table presenting a summary of the soil sample analytical results, as well as a soil quality map are included as Appendix C. 5. GROUND WATER SAMPLING The concentration of naphthalene reported in the ground water sample collected from MW-1 exceeded the MAC. All reported contaminant concentrations were less than the GCLs. A summary of ground water sample analytical results is presented in Table 4. A Ground Water Quality Map is included as Figure 4. The complete laboratory report for ground water samples collected during the January 2023 LSA activities has been included as Appendix D. 6. CONCLUSIONS AND RECOMMENDATIONS  The site contains one building that serves as a grocery store. Land use in the area consists of industrial and residential properties.  On January 24, 2023, a receptor survey was completed at the site. The site is not located within a wellhead protection area, there are no water supply wells and public water is available to the subject site and to all surrounding properties.  No surface water bodies with the exception of a man-made retention pond were identified within a 1,500-foot radius of the site. Please note, the retention pond lies within a fenced area and is not easily accessible to the public.  Due to the shallow depth to water during January 2023 LSA activities, no soil samples were collected. Results of soil samples collected during May 2017 UST closure activities were presented in the IAAR.  Based on this information, the site risk classification should be ranked as low risk. LIMITATIONS This report has been prepared for the exclusive use of Bowers & Burrows, Inc. for specific application to the referenced site in Vance County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by NCDEQ and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of geology and hydrogeology practices in the State of North Carolina, available information and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report. Issuance of this Permit does not supersede any other agreement, permit, or requirement issued by another agency. 3. The well(s) shall be located and constructed as shown on the attachments submitted as part of the Permit application. 4. Each well shall have a Well Contractor Identification Plate in accordance with 15A NCAC 02C .0108(o). 5. Well construction records (GW-1) for each well shall be submitted to the Division of Water Quality’s Information Processing Unit within 30 days of the well completion. 6. When the well is discontinued or abandoned, it shall be abandoned in accordance with 15A NCAC 02C .0113 and a well abandonment record (GW-30) shall be submitted to the Division of Water Quality’s Information Processing Unit within 30 days of the well abandonment. 7. If the well penetrates any water-bearing zone that contains contaminated waters, the upper three feet of the well shall be grouted within one day after the casing is set or the well abandoned.
