EXECUTIVE SUMMARY
The Han-Dee Hugo #97 site is located at 305 Pritchard Road in Clayton, Johnston County, North Carolina.
The property operates as a petroleum retail facility and convenience store. The site is covered by concrete
and asphalt. One 15,000-gallon gasoline UST, one 8,000-gallon gasoline UST, and one 8,000-gallon diesel
UST are located at the site with four associated gasoline and two gasoline/diesel dispenser islands.
On October 21, 2021, during a site assessment for property refinancing, a release was confirmed following
the receipt of soil laboratory results. Concentrations of benzo(a)anthracene and benzopyrene that
exceeded soil-to-water MCCs were reported. Based on the soil results, NCDEQ required additional
assessment.
In April 2022, a receptor survey was conducted. The surrounding properties are commercial, residential,
highway business, office institutional, mixed use, and public/institutional. No private wells are located
within 1,000 feet of the site. Public water is available to the subject site and to all surrounding properties.
No surface water bodies were identified within a 500-foot radius of the site.
On April 19, 2022, one monitoring well (MW-1) was installed in the former location of TMW-5-20’, the
location of soil contamination identified during previous environmental assessment activities conducted
in September 2021. Soil samples were collected at 10-foot intervals during the installation of monitoring
well MW-1. No reported concentrations exceeded the soil-to-water MCCs.
Please note that on April 19 and 25, 2022, no ground water was measured in MW-1; therefore, no ground
water samples were collected.
Based on the lack of receptors, the site should be classified as low risk. Contaminant concentrations
reported in TMW-5-20’ exceeded the soil-to-water MCC; therefore, public notice should be completed
and no further action should be required. An NRP is not required as none of the estimated concentrations
exceeded the residential SCLs.

Release was discovered on October 21,2021.
An unknown amount of product was released from the leaking UST System.
One 15,000-gallon gasoline, one 8,000-gallon gasoline and one 8,000-gallon diesel USTs are in use at
the site (Table 2).

Development-Mixed Use).

Explain:
The subject property is currently an active convenience store and petroleum retail facility and
likely remain a convenience store in the near future.
Property Surrounding Source Area of Release
The questions below pertain to the area within 1,500 feet of the source area of the release (excludes
property containing source area of the release):
9. What is the distance from the source area of the release to the nearest primary or secondary
residence (permanent or temporary)?
The closest primary residence is 20 Simmental Court, located approximately 360 feet northwest
of the UST basin.
10. What is the distance from the source area of the release to the nearest school, daycare center,
hospital, playground, park, recreation area, church, nursing home or other place of public
assembly?
Riverwood Elementary School is located at 801 New Garden Road, approximately 220 feet south
of the source area at the site.
11. What is the zoning status of properties in the surrounding area?
According to the Town of Clayton Zoning Map, the surrounding areas are zoned Highway business
(B-3), Planned development mixed use (PD-MU), residential (R) and office institutional (O-I).
12. Briefly characterize the use and activities of the land in the surrounding area.
The area surrounding the site is a mixture of mixed-use, public/institutional and residential. Land
use information has been included as Table 3. A zoning map has been included as Figure 2B.
2. RECEPTOR INFORMATION
1. Water Supply Wells:
A receptor survey was conducted on April 19, 2022. Five active water supply wells were identified
within a 1,500-foot radius of the site. None of the wells identified are located within 1,000-feet
of the site. A Site Vicinity Map showing property boundaries for the site and surrounding

properties has been included as Figure 2A. Water supply well information has been provided in
Table 4.
2. Public Water Supplies:
Are public water supplies available within 1,500 feet of the source area of the release? YES
If yes, where is the location of the nearest public water lines and the source(s) of the public water
supply. Explain:
Municipal water is available to the site and the surrounding properties. Water lines run along the
public streets in the area. Public water is supplied by the Town of Clayton.
3. Surface Water:
Identify all surface water bodies (e.g., ditch, pond, stream, lake, river) within 1,500 feet of the
source area of the release. This information must be shown on the USGS topographic map.
No surface water bodies were observed within 1,500 feet of the source area.
4. Wellhead Protection Areas:
Identify all planned or approved wellhead protection areas in the vicinity of the source area. This
information must be shown on the USGS topographic map. Wellhead protection areas are defined
in 42 USC 300h-7(e).
On May 17, 2022, GRI personnel accessed the NCDEQ SWAP Program’s website to identify
wellhead protection areas in the vicinity of the subject site. The site is not located within a
wellhead protection area.
5. Deep Aquifers in the Coastal Plain Physiographic Region:
The site is not located in the Coastal Plain Physiographic Region.
6. Subsurface Structures:
No structures with basements were observed in the vicinity of the source area. The water main
and other underground utilities run along Pritchard Road and Covered Bridge Road and several
underground utility lines were observed on the property.
7. Property Owners and Occupants:
The names and addresses of the owners of properties adjacent to the site are presented in Table
5. Property boundaries are shown on Figure 2A. Property cards for the adjacent property owners
are attached as Appendix A.
3. SITE GEOLOGY AND HYDROGEOLOGY
According to the 1985 Geologic Map of North Carolina, the site is located in the Raleigh Belt. The bedrock
is generally described as foliated to massive granitic rock. The soil samples collected during well
installation activities were red and tan silty clays to silt. The depth to ground water during the April 2022
gauging event was greater than 40 feet bgs. A copy of the well construction record has been included as
Appendix B. A Site Map is included as Figure 3. Well construction and gauging information is presented

4. SITE INVESTIGATION
On September 16, 2021, during a site assessment for property refinancing, GRI personnel installed six soil
borings (TMW-1 through TMW-6) around the UST basin and dispenser areas. Based on field observations,
the soil at the site was found to consist primarily of silts to approximately 49 feet below bgs the terminal
depth of TMW-2. All remaining boring locations were drilled to 20 feet using direct push technology. No
ground water was encountered; therefore, soil samples were collected from TMW-1-20’ through TMW-
6-20’ approximately 20 feet BGS and submitted for laboratory analysis of VOCs by EPA Method 8260 and
semi-VOCs by EPA Method 8270. No petroleum odors were observed in any of the soil samples collected.
Estimated concentrations of benzo(a)anthracene and benzo(a)pyrene reported in the soil sample
collected from TMW-5-20’ exceeded the soil-to-water MCCs as specified in 15A NCAC 02L.0202. None of
the concentrations exceeded the residential SCLs. Locations of the soil samples are shown on Figure 4
and a summary of the soil laboratory results are show in Table 7. Based on these results, the NCDEQ
requested an LSA. A copy of the NORR has been included in Appendix C.
On April 19, 2022, one monitoring well (MW-1) was installed by a North Carolina licensed well contractor.
MW-1 was installed to a total depth of 40 feet bgs with a 25-foot screened interval set to bracket the
water table at the time of installation in the vicinity of TMW-5, the location with soil contamination
identified during September 2021 site assessment.

1. Soil Sampling:
On April 19, 2022, three soil samples (SB-1 10’, SB-2 20’, SB-3 30’) were collected at 10-foot intervals
during the installation of monitoring well MW-1. The soil samples were submitted for analysis of VPH
by the MADEP method. Soil samples (SB-1 10’ and SB-3 30’) were also submitted for analysis of VOCs
by EPA Method 8260B/8270.
No reported contaminant concentrations exceeded the soil-to-water MCCs. A Soil Quality Map has
been included as Figure 4. A summary of the soil sample analytical results is presented in Table 7. A
copy of the laboratory analytical report from the April 2022 soil sampling event has been included in
Appendix D. A copy of the current soil sampling data collected during the LSA activities is shown
below:

2. Ground Water Sampling:
On April 19 and 25, 2022, no water was measured in MW-1; therefore, no ground water samples were
collected.
3. Quality Control:
The soil samples were immediately placed in laboratory-supplied containers after collection, which
were then placed in iced coolers. New disposable bailers were used to sample the monitoring well
and new nitrile gloves were worn during soil and ground water sampling. Samples were logged using
proper chain-of-custody procedures. The samples were transported to a North Carolina certified
laboratory for analysis.
5. CONCLUSIONS AND RECOMMENDATIONS
The site is an active convenience store and petroleum retail facility. Land use in the area consists
of commercial, residential, office and public/institutional properties.
In April 2022, a receptor survey was conducted. No potable wells were identified within 1,000
feet of the site. Public water is available to the subject site and to all surrounding properties. No
surface water bodies were identified within 500 feet of the site.

In September 2021, during a site assessment for refinancing, estimated concentrations of
benzo(a)anthracene and benzo(a)pyrene in the soil sample TMW-5-20’ exceeded the soil-to-
water MCCs.
April 19, 2022, during installation of MW-1 in the former location of TMW-5-20’, soil samples
were collected at 10-foot intervals to 30 feet bgs. No reported contaminant concentrations
exceeded the soil-to-water MCCs.
On April 19 and 25, 2022, no ground water was present in MW-1; therefore, no ground water
samples were collected.
Based on the lack of soil-to-water MCC violations in any of the soil samples collected during the
LSA, it was deemed unnecessary to install MW-1 deeper.
Based on the lack of receptors, the site should be classified as low risk. Contaminant
concentrations reported in TMW-5-20’ exceeded the soil-to-water MCC; therefore, public notice
should be completed and no further action should be required. An NRP is not required as none
of the estimated concentrations exceeded the residential SCLs.
6. LIMITATIONS
This report has been prepared for the exclusive use of Sampson-Bladen Oil Company, Inc. for specific
application to the referenced site in Johnston County, North Carolina. The assessment was conducted
based on the scope of work and level of effort specified by NCDEQ and with resources adequate only for
that scope of work. Our findings have been developed in accordance with generally accepted standards
of geology and hydrogeology practices in the State of North Carolina, available information and our
professional judgment. No other warranty is expressed or implied.
The data that are presented in this report are indicative of conditions that existed at the precise locations
sampled and at the time the samples were collected. In addition, the data obtained from samples would
be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No
additional information can logically be inferred from these data.

Dear Mr. Clark:
Information received by this regional office of the Underground Storage Tank Section, Division of
Waste Management, on October 25, 2021, confirms a release or discharge from a petroleum underground
storage tank (UST) system at the above-referenced location. Records indicate that you are the owner or
operator of this UST system. Therefore, as a responsible party, you must comply with the initial response
and abatement action requirements of the Title 15A of the North Carolina Administrative Code (NCAC),
Subchapter 2L .0404 and, if applicable, the assessment and reporting requirements of Title 15A NCAC 2L
.0405, within the timeframes specified in the attached rules. (Be aware that if Title 15A NCAC 2L .0405
is applicable, you must comply with its requirements even if you do not receive formal notification from
the UST Section.)
Initial abatement action requirements include the preparation and submittal of an Initial Abatement
Action (IAA) Report, in accordance with Title 15A NCAC 2L .0404 and the most recent version of the
Guidelines for Site Checks, Tank Closure, and Initial Response and Abatement for UST Releases, within
90 days of discovery of the release.
Because a release or discharge has been confirmed, a Licensed Geologist or a Professional
Engineer, certified by the State of North Carolina, is required to prepare and certify all reports submitted to
the Department of Environmental Quality in accordance with Title 15A NCAC 2L .0103(e) and 2L
.0111(b).
Please note that before you sell, transfer, or request a “No Further Action” determination for a
property that has not been remediated to below “unrestricted use” standards, you must file a Notice of
Residual Petroleum ("Notice") with the Register of Deeds in the county where the property is located (North
Carolina General Statutes 143B-279.9 and 143B-279.11).
