Site History and Impact On November 15, 2021, a Freight Works Transport tractor trailer was traveling east on Highway 74 in Mooresboro, North Carolina when the truck was involved in an accident. The truck left the highway on the south shoulder and came to rest near the outfall of a storm drain. The accident caused a breach in the truck’s saddle tanks and spill of diesel fuel. The exact amount of fuel spilled is not known however site officials estimated approximately 300 gallons of diesel fuel was released. Figures 1 and 2 illustrate the location of the incident. Soil was impacted in the shallow ditch along the south shoulder of the highway. The spilled fuel continued flow through a deeper section of the ditch to an area of ponded water at the storm drain outfall. The area of ponded water was approximately 25 feet long and 15 feet wide. A soil berm was present on the south end of the outfall area which retained the fuel and did not allow it to continue to the nearby flowing ditch. The flowing ditch was in the wooded area south of the highway and flowed west to east parallel to Highway 74. The spill area was on the North Carolina Department of Transportation right of way. No water supply wells were noted in the immediate area of the spill. Residential properties were noted approximately 350 to 400 feet north of the site. The Mooresboro Town office with recreational fields is located 500 feet south of the site. A UST-62, 24-Hour Notification of Discharge Form was submitted to the North Carolina Department of Environmental Quality-Asheville Regional office. Appendix A includes a copy. A cleanup contractor was mobilized the day incident. First Call Environmental (FCE) of Asheville, North Carolina was contacted and responded. C. Free Product Investigation FCE mobilized to the site and began removing fuel from puddles along the ditch and the area of ponded water. Free product recovery continued November 16, 2021. A total of 2,427 gallons of water and fuel was recovered for disposal. No ongoing free product recovery was necessary. D. Surface Water A storm water outfall area retained the spilled diesel fuel before it reached a nearby flowing ditch. An earthen berm separated the ponded water/fuel and the ditch flowing west to east. The nearest creek was Sandy Run 0.92 miles east of the spill location. E. Initial Response and Abatement Action FCE mobilized to the site the day of the accident and began removal of product in the ditch line and the area of ponded water. Soil excavation on the south shoulder of the highway also took place the day of the spill. Further excavation using a vacuum extraction unit took place on November 17 and 18, 2021. 4 F. Underground Utilities Two phone/fiber optic lines were located near the spill area. One line was located near the immediate shoulder of the highway. The second line was south of the impacted ditch, ran parallel to the highway. G. Excavation and Removal of Contaminated Soil The shallow ditch and roadway shoulder were excavated on November 15, 2021. As excavation of the impacted soil took place, Lighthouse collected post excavation soil samples S-1 through S-9. Further east from sample S-9, the ditch was steep, incised and began to deepen as it reached the storm drain outfall. On November 16 and 17, 2021, Hydrovac excavation was used in the outfall area where free product and water was puddled. Samples S-10 through S-14 were collected to document the post excavation soil conditions. As excavation proceed in the area between samples S-9 and S-10, groundwater and fuel began entering the excavation. The Hydrovac unit was used to remove free product and water. Soil removed from this area was laden with trash and debris, some of which appeared to be burned. At the point the excavation was backfilled, no free phase product was observed in the excavation. Figure 3 illustrates the location of the impacted area and the soil sample locations. Post excavation soil samples were analyzed by RED Lab, LLC using a QED UVF petroleum analyzer with finger printing calibration. Each sample was analyzed for Total Petroleum Hydrocarbon-Gasoline Range Organics (TPH-GRO), Diesel Range Organics (TPH-DRO) and Total BTEX. The results are illustrated on Table 1. The GRO results ranged from <0.27 mg/kg to <0.6 mg/kg. The DRO results ranged from 0.11 mg/kg to 21.9 mg/kg. These results are below or at the North Carolina Department of Environment Quality (NCDEQ) action limits of 50 mg/kg for TPH-GRO and 100 mg/kg for TPH DRO. Appendix B includes the Red Lab laboratory report. H. Groundwater Sampling As noted above, groundwater and free product were noted in the excavation between sample locations S-9 and S-10. The ditch was steep and sharply incised in this area. The ditch was restored by FCE to its original condition after excavation. Access with a drill rig for well installation was not possible. Thus, to obtain a groundwater sample, Lighthouse installed a hand auger boring and a five-foot section of two inch well screen to help filter fine soil particles from the boring. The hand auger boring location is noted as GW-1 on Figure 3. The following day Lighthouse returned and measured water at a depth of 1.5 feet below surface. No free product was detected. A sample of the groundwater was obtained using a disposable bailer and submitted to Pace Analytical in Huntersville, North Carolina for analysis by methods 6200B, 625BNA and MADEP EPH and VPH. After sampling, the hand auger boring was filled with bentonite. The laboratory results are shown on Table 2. The following compound and carbon fractions were detected above the State of North Carolina 2L Class GA standards. Aliphatic C9 -C18 Carbon Fraction at 10,880.1 ug/l Aromatic C11-C22 Carbon Fraction at 3,850 ug/l Benzene at 3.1 ug/l 5 The laboratory report is included as Appendix C. I. Soil and Liquid Disposal FCE organized disposal of the impacted soil. The total amount of excavated soil was 53.21 tons. FCE transported the soil to Environmental Soils located in Lattimore, North Carolina. FCE indicated a total of 2,427 gallons of fuel and water mixed with some soil was pumped from the ditch and storm drain outfall using the Hydrovac unit. The liquid/solid waste was transported to the AEO Solidification Facility in Mooresboro, North Carolina and HERR in Thomasville, North Carolina. Appendix D includes the waste disposal documentation provided by FCE for the soil and liquid disposal. J. Site Observations As noted above, during excavation of the ditch line, soil with debris and trash was encountered near the depth that groundwater was observed entering the excavation. While on site, Lighthouse also observed a heavy presence of orange organic algae in the nearby flowing ditch that trends parallel to the spill area. Numerous bottles and trash were also noted in the flowing ditch. It is our observation that this area was possibility a dump site in the past and was covered with soil during the widening or construction of Highway 74. K. Conclusions The Freight Works Transport tractor trailer accident resulted in a release of diesel fuel on the south shoulder of Highway 74 East in Mooresboro, North Carolina. The spilled fuel impacted soil and flowed east to a stormwater discharge/outflow area. The storm water outflow area held water at the time of the accident which allowed the diesel fuel to accumulate. A soil berm at the south end of the storm water outflow area stopped the flow of the fuel from entering a downstream flowing ditch. A clean up contractor was mobilized and began soil removal on the shoulder of the highway and in the ditch line the day of the incident. Removal of fuel, water and impacted soil in the storm water outfall area began the next day. Post excavation soil samples were collected in all the areas of soil impact. The post excavation soil sample TPH laboratory analysis results were below the state TPH action levels. The groundwater sample analysis indicated 2L Standard exceedances for benzene and the Aliphatic C9- C18 and Aromatic C11-C22 Carbon Fractions. It should be noted that the subsurface in the spill area includes trash and debris. The presence of orange organic algae in the nearby flowing ditch is also an indicator that the trash and debris may be laterally extensive. It is possible the groundwater analytical results were affected by the buried trash and debris. A copy of this report will be submitted to NCDEQ Asheville office. They will evaluate the results of the assessment and respond accordingly. L. Report Limitations This report has been prepared by Lighthouse Technical, P.C. for the use of Freight Works Transport and the North Carolina Department of Environment Quality. The findings and technical interpretations included in this report are specific to the area of investigation. This assessment of subsurface conditions 6 has been performed in accordance with generally accepted practices of assessing the subsurface for contaminants. The conclusions presented in this report are based on subsurface sampling performed at selected locations and depths. The data collected represent the conditions at the sample location, at the time of sample collection only. There may be conditions between borings or samples that differ from those presented in this report. Such conditions could not be predicted by this assessment. Lighthouse by virtue of providing the services described in this report, does not assume the responsibility of the person(s) in charge of the site, or otherwise undertake the responsibility for reporting to any Local, State, or Federal public agencies as required by law or otherwise to disclose, in a timely manner, unless so directed by the client, any information that may be necessary to prevent any danger to public health, safety, or the environment.
