Johnson The Presbyterian Homes, Inc. 2109 Sandy Ridge Road Colfax, NC 27235 Re: Acknowledgement of Report Receipt Friends Homes Property-913 Ridgecrest Drive 913 Ridgecrest Drive Greensboro, Guilford County Incident Number: 47408 Risk Classification: Low Dear Mr. Johnson: The UST Section, Division of Waste Management, is in receipt of the UST Closure Report dated November 23, 2020. The report has been reviewed and will be maintained in the Guilford County Department of Health and Human Services Office. After report review, our office has determined that while a confirmation soil sample was collected at the base of the excavation, no confirmation soil samples were collected from each side wall as required after excavation of contaminated soil. Therefore, contaminated soil above the soil to groundwater or residential maximum soil contaminant concentrations (MSCCs) may be present. To obtain a clean closure no further action determination, one confirmation soil sample must be collected on each sidewall of the excavation and tested for risk-based analyses in accordance with the latest version of the Guidelines for Site Checks, Tank Closures and Initial Response and Abatement (Guidelines). If petroleum contaminant concentrations are greater than the soil to groundwater MSCCs, you must proceed to a Limited Site Assessment in accordance with the Guidelines which can be found on the following website: https://deq.nc.gov/about/divisions/waste-management/ust/whats-new. If you wish to proceed to closure without further action, a Notice of Residual Petroleum (NRP) is required to be recorded at the Guilford County register of deeds. The NRP will restrict soil in the vicinity of the former UST and groundwater beneath the property. A final NFA will be issued upon receipt of a certified copy of the recorded NRP and proof of public notice. If you have questions regarding the information contained in this letter, please contact Ashley Dinkins at (336) 641-5557 and at the Guilford County Department of Health and Human Services office, located at 400 W. Market Street, Suite 300, Greensboro, NC 27401. Sincerely, Carin Lee Kromm, L.G.
