The petroleum USTs 001, 002 and 003 were located on the north side of the building (Figure 3). USTs 002 and 003 were last operated by Sloop Properties and according to the NC UST Database they were installed in 1972. These were two 4,000-gallon gasoline USTs. UST 001 was also operated by Sloop Properties and was installed in 1978. This was a 10,000-gallon gasoline UST. No release incident number has been associated with this UST system. 3. There are no known spills or releases from non-UST sources at the subject property prior to the release discovered on March 4, 2020. 4. Provide a comprehensive description of the release, including date discovered, cause and source (including tank identification number and contents), and the relationship of historical UST releases, non-UST releases, and off-site releases (indicate incident number) to contamination from current release. The USTs were permanently closed by removal on March 4, 2020. Staining and petroleum odors were observed below the dispensers at both islands. High concentrations of petroleum vapors were measured with a Photoionization Detector (PID) in soil samples collected from underneath four of the six dispensers. Seramur & Associates is not aware of any non-UST releases or off-site releases that could be contributing to contamination from current release. 5. Provide a brief description of site characteristics (including status of facility (active or inactive), land use of site and surrounding area, water supply, topography, vegetation, surface water, wells, buildings, surface cover, soil type, depth to and nature of bedrock, depth to groundwater, direction of groundwater flow, etc.) The facility is currently inactive, and the UST system was temporarily closed for an unknown amount of time. The Performance Properties building is being demolished by the new owners, Millers Creek Crossroads, LLC. Surrounding land use is primarily commercial with some residential properties located east of the property. A public water supply is available to the subject property and surrounding properties. Groundwater was not observed in the UST excavations. The property is located within a small valley in Millers Creek. The nearest body of water is Millers Creek, located less than a half-mile to the south. There is very little vegetation on the property apart from grass along the east side of the property, near NC Hwy 16. The soil consists of sandy silt in the source area up to a depth of 13.7 feet. Bedrock was not encountered and depth to bedrock is unknown. Bedrock in this area is mapped as the Alligator Back Formation that is described as interbedded mica schist and gneiss (Rankin, D. W., Espenshade, G. H., Neuman, R. B., 1972. Geologic map of the west half of the Winston-Salem Quadrangle, North Carolina, Virginia, and Tennessee. U. S. Geological Survey, Map I-709-A, scale 1: 250,000). 6. Summarize initial abatement actions, assessment activities, and corrective actions performed to date and list all reports previously submitted. The USTs were permanently closed by removal on March 4, 2020. Soil removed from the UST excavation was screened in the field for petroleum vapors with a calibrated Photoionization Detector (PID). Excavated soil was temporarily stockpiled near the excavation. Three soil samples were collected from below each of the two 4,000-gallon gasoline USTs. Two soil samples were collected from below the 10,000-gallon gasoline UST. One soil sample was collected under each of the six former dispensers, and five samples along the product lines to the former dispensers (Figure 4). During the UST excavation work a 250-gallon heating oil UST was discovered in the side of the excavation. This UST was also removed, and one soil sample was collected adjacent to the 250-gallon heating oil UST. A total of 20 soil samples were collected and shipped to the laboratory for analysis using the UVF Method for total petroleum as hydrocarbons, gasoline range organics and diesel range organics (TPH GRO and DRO). The PID detected petroleum vapors in soil samples collected below the dispensers. Laboratory analysis detected TPH DRO concentrations above NCDEQ Action Levels in the soil samples collected from the below the dispensers. Petroleum constituents were not detected above the NCDEQ Action Levels in soil samples collected from the UST excavation or along the product lines. Soil contamination appeared to be limited to the area around and under the two dispenser islands. Seramur & Associates re-mobilized to the site on Friday, March 20, 2020 to supervise over-excavation of contaminated soil around the dispenser islands. A total of 102.29 tons of contaminated soil was excavated from below the dispenser islands. Vannoy Construction transported the contaminated soil excavated from the dispenser islands to Evo Corporation: Environmental and Industrial Services, located in Winston Salem, NC. Five post-excavation soil samples were collected from the final excavation, four from the sidewalls and one from the center of the excavation. These soil samples were analyzed for petroleum constituents using EPA Methods 8260 and 8270 and the MADEP VPH and EPH methods. Petroleum constituents were not detected at concentrations above the residential or soil to groundwater MSCCs. Clean fill brought in from offsite was later used to backfill the excavation. C. Site Check Report A site check report has not been completed. D. UST Closure Report The UST-2 form is included in Appendix A. 1. Describe preparations for closure including steps taken to notify authorities, permits obtained, and steps taken to clean and purge the tanks. A Utility locate request was made to NC One-Call. Seramur & Associates personnel made site visits to document the utility locations and check the content of the USTs. A UST closure permit was obtained from the Wilkes County Building Inspection Office. The atmosphere in the USTs were checked for explosive vapors with an Explosimeter. The Lower Explosive Limit (LEL) readings for the gasoline USTs were below 10%. Residual fuel was pumped from the three USTs by Zebra Environmental & Industrial Services. The tanks were cleaned with a pressure washer using soap and water after the fuel had been removed. This water-fuel mix was then also removed from the tanks by Zebra Environmental & Industrial Services and is included in the 918 gallons of residual liquids. The tanks were excavated and removed from the site. Vannoy Construction transported the USTs to Triad Metal Recyclers in Yadkinville, NC for disposal. 2. Describe the closure procedure referencing site and sampling location maps and cross-sections presented in Section J of this report. Clearly state how the selection of sample types and locations was influenced by the nature of the stored substance, the type of initial alarm or cause for suspicion, the type of backfill, the depth to groundwater, and other factors appropriate for identifying the presence and source of a release. If presence of groundwater or bedrock require the installation and sampling of monitoring well(s), describe the process. Seramur & Associates mobilized to the site to excavate the tank pit and collect soil samples on February 28, March 2 and March 4, 2020. Soil was excavated from the sides of and both ends of each UST. The USTs were then removed from the excavation using chains and the trackhoe. Soil samples were collected from below each UST, along the product lines, and from below each dispenser (Figures 4, 6 and 7). A total of 20 soil samples were collected to be analyzed for TPH GRO and DRO. The soil samples collected from the tank excavation were collected using the trackhoe bucket. The soil samples collected from below the dispensers were collected using a decontaminated stainless-steel spoon. The samples collected beneath the product lines and the small heating oil UST were collected with decontaminated hand augers. The extent of the excavation and locations of soil samples are shown on Figures 3, 4 and 6. The UST excavation measured approximately 34’ x 38’ and the dispenser island excavation measured approximately 26’ x 30’. All soil samples were collected while wearing a new pair of disposable Nitrile gloves. The soil samples were placed in containers provided by the lab, labeled and placed on ice. A chain of custody was initiated, and the samples were shipped via FedEx to REDLab, LLC in Charlotte, NC for analysis using the UVF Method. Seramur & Associates, PC re-mobilized to the site to excavate contaminated soil below the dispenser islands and collect additional soil samples on March 20, 2020 (Figure 5). Five samples were collected from the former dispenser island excavation and analyzed for petroleum constituents using EPA Methods 8260 and 8270 and the MADEP VPH and EPH methods. Contaminated soil was excavated from below the former dispensers (extent of excavation is shown on Figures 7 and 8). Soil samples were collected from each side wall and at the center of the excavation (Figure 5). The confirmation samples collected from the dispenser excavation were collected using the trackhoe bucket. The extent of the excavation and locations of soil samples are shown on Figures 5, 7 and 8. All soil samples were collected while wearing a new pair of disposable Nitrile gloves. The soil samples were placed in containers provided by the lab, labeled and placed on ice. A chain of custody was initiated, and the samples were shipped via FedEx to Waypoint Analytical (formerly Prism Laboratories Inc.) in Charlotte, NC for analysis. The selection of sample types and locations was not influenced by the nature of the stored substance. Depth to groundwater is not known. Groundwater was not encountered in the excavation. The UST excavation was backfilled with excavated soil as well as clean fill obtained from off site. The dispenser islands’ excavation was backfilled with clean fill obtained from off site. 3. Note the amount of residual material pumped from the tank and describe the storage, sampling and disposal of the residual material and the disposal of the tank, pumps and piping. Residual fuel was removed from the three USTs by Zebra Environmental & Industrial Services prior to the closures. A total of 918 gallons of a petroleum/water mix were removed from the USTs and ASTs. The tanks were excavated and removed from the site. The USTs were cleaned with a pressure washer and detergent. Vannoy Construction transported the USTs to Triad Metal Recyclers, LLC in Yadkinville, NC for disposal. 5. If a release is determined, document initial response actions. A petroleum odor and staining were noted in surface soil below the two dispenser islands west of the former Performance Property building. Soil excavation was limited to the volume of soil that was necessary to remove contamination at the former dispensers. Soil samples collected below the USTs, samples S-13 through S-21, detected petroleum constituent concentrations below NCDEQ Action Levels (Figures 3 and 4). Elevated petroleum constituent concentrations were measured in soil samples S-1 through S-6, collected from the dispenser islands during the UST closure (Figure 4). Petroleum constituent concentrations above NCDEQ Action Levels were detected in soil samples S-2, S-3, S-4 and S-5 (Figures 3, 4 and 7). Seramur & Associates, PC re-mobilized to the site to excavate contaminated soil below the dispenser islands and collect additional soil samples on March 20, 2020 (Figure 5). The post-excavation assessment is described above in Section 2. A 24-hour report was prepared and submitted to NCDEQ. 6. Document any soil excavation activities. Soil was excavated from the sides and both ends of each of the three USTs. Excavated soil was stockpiled on site and the USTs were removed from the excavation using chains and the trackhoe. The former tank pit was then backfilled with fill from the tank pit and clean fill sourced from offsite. Soil was also excavated from below the former dispensers to remediate contaminated soil in this area. The contaminated soil was then shipped to a remediation facility and the excavation was backfilled with clean fill sourced from offsite. 7. Document site investigation. The USTs were closed as described above. Contaminated soil was removed from below the dispenser islands and post excavation confirmation soil samples were collected. Contaminated soil has been removed from the property. Groundwater and bedrock were not encountered in the excavations. Soil sample locations are shown on Figures 4 and 5. North-South and East-West cross-sections are shown in Figures 6, 7 and 8. Initial soil samples were collected on February 28, March 2 and March 4, 2020 (Figure 4). Five additional soil samples were collected after removing contaminated soil from below the dispensers on March 20, 2020 (Figure 5). The sample times and dates are listed on the Chain of Custody Records (Appendix B). Initial soil samples were stored on ice and shipped to REDLab, LLC in Charlotte, NC via FedEx. Confirmation soil samples were stored on ice and shipped to Prism Laboratories Inc. in Charlotte, NC via FedEx. Samples S-2, S-3, S-4 and S-5 contained DRO concentrations above the NCDEQ Action Levels (Figure 4). The post-excavation, confirmation soil samples collected from the dispenser island excavation did not contain petroleum constituents above the MSCCs. 8. Present conclusions and recommendations, referencing maps The source of soil contamination was the dispenser islands. Excavation limits are shown in Figures 3, 7 and 8. Elevated concentrations of TPH DRO were detected in soil samples S-2, S-3, S-4 and S-5, collected below the former dispenser islands (Figure 4). The post-excavation confirmation soil samples collected from the dispenser island excavation did not contain petroleum constituents above the MSCCs. No further assessment work is necessary for this release. E. Free Product Investigation and Recovery Report Free product was not observed during the UST closure. F. Groundwater and Surface Water Investigation There was no groundwater or surface water investigation completed during the UST closure. G. Initial Response and Abatement Action The tanks were excavated and removed from the tank pit excavation. The PID did not detect petroleum vapors above 1.0 ppm in the soil removed from the tank pit excavation. This soil was used to backfill the excavation. The initial soil samples were collected under the tanks, along the lines and below the dispensers. These samples were analyzed by REDLab, LLC in Charlotte, NC. Contaminated soil below the dispenser islands was excavated and transported to a remediation facility. These samples were collected to be analyzed for volatile organic compounds by Waypoint Analytical in Charlotte, NC. A 24-hour report was prepared for NCDEQ. The post-excavation confirmation soil samples collected from the dispenser island excavation did not contain petroleum constituents above the MSCCs. H. Excavation of Contaminated Soil A petroleum odor and some staining were observed in the soil below the dispensers. Soil excavation was limited to the volume of soil that was necessary to remove contamination from below the former dispensers. A total of 102.29 tons of contaminated soil was excavated and transported to Evo Corporation in Winston-Salem for remediation (Appendix D). The PID did not detect petroleum vapors in the soil samples collected from the dispenser island excavation. Clean fill from offsite was used to backfill the excavation. The post-excavation confirmation soil samples collected from the dispenser island excavation did not contain petroleum constituents above the MSCCs. I. Conclusions Elevated concentrations of diesel range organics were detected in samples S-1, S-2, S-3, S-4, S-5 and S-6. Only the diesel range organics were detected above the NCDEQ Action Levels. This soil has been excavated and sent to a remediation facility. Seramur & Associates does not recommend any further assessment work for this release. The atmosphere in the USTs was checked for explosive vapors with an Explosimeter. The Lower Explosive Limit (LEL) readings for the gasoline USTs were below 10%. The tanks were cleaned using a pressure washer to remove residual fuel and fumes. Soil samples from the excavations were collected out of the trackhoe bucket. Dispenser sampling was completed with decontaminated steel spoons. Product line sampling was completed with decontaminated hand auger buckets. The hand auger buckets and steel spoons were decontaminated with non-phosphate detergent and distilled water and isopropanol rinses. Soil samples collected for laboratory analysis were placed in laboratory sample jars with Teflon-lined lids, chilled and Chain-of-Custody records were initiated. Initial closure soil samples were transported to REDLab, LLC in Charlotte, North Carolina for analyses. Post-excavation confirmation soil samples were transported to Waypoint Analytical in Charlotte, North Carolina for analyses. A new pair of disposable Nitrile gloves was worn while collecting each sample. The depth and horizontal locations of the soil samples were identified either by measuring with a fiberglass tape, estimating right angles and referencing site features or with the use of a total station.
