Fanous Group, Inc. owned USTs which are operated at the Sandy Cross Mini Mart facility located at 2078 NC-49 North in Burlington, North Carolina. Figure 1 illustrates the location of this facility on the Burlington NE Quadrangle U.S.G.S. Topo Map. This property contains 1 permanent structure which is currently vacant. The site last contained (2) 6,000 gallon + (1) 4,000 gallon gasoline USTs; (1) 550 gallon kerosene UST, and (1) 8,000 gallon diesel USTs which were last in use at this facility in July, 2018. On December 10, 2019, these 5 USTs were closed at this facility. Figure 2 illustrates the overall layout of this site and the locations of the USTs closed at the site in 2019. At least 2 USTs were formerly in use at this site dating back to prior to 1986. A release was noted during UST closure activities for these USTs conducted on 3/25/1986 (Incident #3199). Table 1 summarizes the UST history for the site. A release from the latest UST system was confirmed on 12/10/2020 and reported to on- site NCDEQ personnel based on the presence of free product beneath the T4/T5 USTs (Incident #45316). This release was confirmed upon receipt of analytical results from soil samples collected during the tank closure and abatement activities. The site and adjacent properties are used for residential and commercial purposes. Municipal water supplies are available to most properties within 1,500’; however, private potable supply wells are in use within 1,500’. A Limited Site Assessment (LSA) has been conducted in response to this release in order to classify the risk of the site for the release source originating at the USTs owned by F. Fanous Group Inc. The findings of the LSA conducted for this site are contained herein. 2.0 - RISK CHARACTERIZATION AND LAND USE FORM Part I Groundwater/Surface water/Vapor impact Low Risk 1. Has discharge or release contaminated any water supply wells including any used for non-drinking purposes? NO 2. Is a water supply well used for drinking water located within 1,000 feet of the source area the discharge or release? YES Page - 2 3. Is a water supply well used for any purpose (e.g., irrigation, washing cars, industrial cooling water, filling swimming pools) located within 250 feet of the source area of the discharge or release? Briefly characterize the use and activities of the land in the surrounding area. COMMERCIAL MULTI-USE, FARM & RESIDENTIAL 3.0 - RECEPTOR INFORMATION Page - 5 3.1 Water Supply Wells A supply well survey has been conducted within a 1,500' radius of the release area. This survey was performed by interviews conducted with owners and occupants of properties located within 1,500’ of the release sources. During this reconnaissance (2) water supply wells (potable) were found to be located within a 1,000’ radius. The out-of-service on- site former water supply well is located within 180’ from the UST release source area. An in-use supply well owned and operated by John David Bruner is located 465’ from the UST source area and is the closest potable water supply well. An additional water supply well is believed to be located 520’ from the source area on a property owned by Kimery Rudd Braxton. The house on the property is located > 1000’ from the release source and is occupied; however, attempts to contact the site occupant have been un- successful. Table 3 lists information for the wells identified during this investigation. Figure 5 depicts the approximate locations of these wells on a Google Earth Aerial map. 3.2 Public Water Supplies Public water supplies are available to all properties along NC Highway 49 and annexed properties to the east of NC Highway 49; however, properties within 1,500’ of the release site along Sandy Cross Road do not yet have access to municipal water supply. 3.3 Surface Water The partial Burlington NE Quadrangle U.S.G.S. Topographic Map included as Figure 1 indicates that the release site is located atop a ridge at an approximate elevation of 655’ amsl. Surface waters in the vicinity of the release area generally drain towards the west/northwest towards the headwaters of Boyd’s Creek. Boyd’s Creek empties into the Haw River which lies within the Cape Fear River Basin. 3.4 Wellhead Protection Areas No wellhead protection areas are known to exist within the area of this release. 3.5 Deep Aquifers in the Coastal Plain Physiographic Region This release is not located in the coastal plain. 3.6 Subsurface Structures Subsurface lines/structures are not present within the petroleum affected area at this site. Subsurface utilities are present in the form of water, communication, & storm drain lines. Underground utility lines are mostly located on the east side of NC Hwy 49 and along the southern side of Sandy Cross Road, outside of the impacted areas within the property boundaries. Figure 3 illustrates the locations of subsurface utilities near the subject property. Page - 6 3.7 Land Use The possibility of human exposure to soil contamination at 2078 NC-49 North site is minimal. Petroleum impacted soils identified at the site are predominantly located > 2’ from surface. Contamination at this facility lies within a commercial site. 3.8 Property Owners and Occupants Figure 4 illustrates the surrounding properties, and Table 2 contains information regarding the adjacent property owners. This information was obtained from Alamance County Tax records. 4.0 - SITE GEOLOGY AND SOIL INVESTIGATION 4.1 Site Geology The site is situated in the Carolina Slate Belt. According to the Geological Map of North Carolina local geology of the region is underlain with metamorphosed granitic rock. Weathered bedrock was encountered at a depth of 12’ below surface in diesel UST basin area monitoring well MW-13. Competent bedrock was encountered in the gasoline UST source area monitoring well MW-14 boring at a depth of 11’ feet below surface. 4.2 Soils Investigation The soils at the project site consist of silt with varying clay, sand and weathered rock content. Soil boring logs are contained on well construction records included as Appendix B. Soil samples were collected from the perimeter of the UST remedial excavation to obtain risk-based analytical data for the site following IAA excavation. The wall soil samples collected from the perimeter of the T4/T5 remedial excavation were submitted to Pace Analytical, Inc. for analyses by EPA Methods 8260 and MADEP Methods for VPH, as appropriate based on tank contents. Laboratory results from the UST wall samples indicate that contamination at this site exceeds NCDEQ Lowest Maximum Soil Contaminant Concentrations (MSCCs) for 9 compounds/compound groups; however, no compounds were detected at levels above Residential MSCCs. Figure 6 illustrates the locations of the post remedial excavation soil samples collected at the subject site. The soil samples were obtained as specified in the collection, preservation, and handling section of the specific method. Each container was labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory utilizing EPA approved chain of custody procedures. Additional samples from each pit wall were containerized for possible analyses via risk-based methods. Page - 7 On January 18, 2023 during installation of LSA monitoring wells MW-13 and MW-14, additional borings were placed to collect soil samples at piping and dispenser areas indicating elevated TPH contaminant concentrations. Gasoline UST system samples were labeled GP1 and GD1, with kerosene dispenser area soil sample labeled KD, and were submitted to Pace Analytical, Inc. for analyses by EPA Method 8260/8270 and MADEP Methods for VPH/EPH, as appropriate based on tank/piping contents. The soil samples were obtained as specified in the collection, preservation, and handling section of the specific method. Each container was labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory utilizing EPA approved chain of custody procedures. Laboratory results from the LSA soil samples indicate that contamination at this site exceeds NCDEQ Lowest Maximum Soil Contaminant Concentrations (MSCCs) for 8 compounds/compound groups. C9-C22 aromatics were detected in gasoline dispenser sample GD1 at a level above Residential MSCCs. Figure 6 illustrates the locations of the risk-based soil samples collected at the subject site, and Table 4 summarizes the laboratory results for the “Risk-Based” soil samples from 2078 NC-49 North in Burlington, NC. A copy of the soil analytical report and the chain of custody records for the risk-based soil samples collected at 2078 NC-49 North during UST closure is contained in the previously submitted IAA report. Soil analytical results from LSA soil samples are contained in Appendix A. 5.0 – GROUNDWATER INVESTIGATION 5.1 Monitoring Well Installation A total of (2) North Carolina Type II groundwater monitoring wells was installed on-site to complete LSA activities. Per discussion with Mindy Lepard of the Winston Salem Regional Office, (2) source area monitoring wells were installed during LSA activities, since previously installed monitoring well MW-3 was located within 10’ of the GD1 source area. Figure 7 illustrates the locations of the new monitor wells, denoted as MW- 13 and MW-14 which were installed to complete LSA activities. The monitoring wells installed at the site were constructed of 2-inch Schedule 40 PVC pipe with 3’-4’ feet of casing and 8’-10’ of 0.010 inch slotted screen. Based on the assumption that the contaminants being addressed were primarily hydrocarbon constituents with specific gravities of less than 1.0, the Type II monitoring wells were installed so that the screened interval intersected the shallow groundwater table at the time of installation. Well construction records for the monitor wells constructed at this site for LSA activities are included in Appendix B. Free product was not detected in any of the monitoring wells sampled at the site during LSA activities conducted on January 18, 2023. Page - 8 5.2 Monitor Well Sampling and Analyses On January 18, 2023, source area monitor wells MW-3, MW-13 and MW-14 were sampled for analyses utilizing EPA Methods 6200B, 625 + 10 peaks, Lead, and MADEP Methods for VPH/EPH as appropriate based on UST system contents. Before sampling, the monitor wells were developed and purged using disposable balers. Following development of the groundwater monitoring well, samples were obtained from the disposable balers. The supply well was also sampled with a disposable baler. All samples were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of-custody procedures. Laboratory analytical results from the monitoring well network installed at the site indicate the presence of 16 compounds/compound groups above 15 NCAC 2L Standards. Benzene was noted at a concentration of 2,030 ug/l in MW-3, above its 2L Standard of 1 ug/l and above its GCL Standards of 5,000 ug/l. C5-C8 Aliphatics were noted at a concentration of 26,000 ug/l in MW-3 above their 2L Standard of 400 ug/l. No compounds were detected at levels above GCLs. Table 5 summarizes the laboratory analytical results for the groundwater samples collected for LSA Activities. Appendix A contains a copy of the laboratory analytical report and chain of custody record for the groundwater samples collected during LSA activities at this site. 6.0 - CONCLUSIONS AND RECOMMENDATIONS 6.1 General Summary Limited Site Assessment activities at 2078 NC-49 North have been completed. From a review of all information gathered during this project, Henry Nemargut Engineering Services makes the following conclusions: o A well reconnaissance of the site was performed which identified (2) potable wells within 1,000’ of the release source area. An in-use supply well owned and operated by John David Bruner is located 465’ from the UST source area and is the closest potable water supply well. An additional water supply well is believed to be located 520’ from the source area on a property owned by Kimery Rudd Braxton. The house on the property is located > 1000’ from the release source and is occupied; however, attempts to contact the site occupant have been un- successful. Municipal water supplies are not available to any properties within 1500’ of this site. Page - 9 o Public water supplies are available to all properties along NC Highway 49 and annexed properties to the east of NC Highway 49; however, properties within 1,500’ of the release site along Sandy Cross Road do not yet have access to municipal water supply. o Results from the risk-based samples obtained during IAA/LSA activities were above current NCDEQ lowest MSCCs for 9 compounds/compound groups for samples collected in the former UST remedial excavation and former dispenser locations. C9-C22 aromatics were detected in gasoline dispenser sample GD1 at a level above Residential MSCCs. o Per discussion with Mindy Lepard of the Winston Salem Regional Office, (2) source area monitoring wells were installed during LSA activities, since previously installed monitoring well MW-3 was located within 10’ of the GD1 source area. These wells were screened to intersect the top of the shallow water table observed at the site. Free product was not detected in any of these monitoring wells during a site check conducted on January 18, 2023. o Laboratory analytical results from LSA monitoring well network indicate the presence of 16 compounds/compound groups above 15 NCAC 2L Standards. Benzene was noted at a concentration of 2,030 ug/l in MW-3, above its 2L Standard of 1 ug/l and above its GCL Standards of 5,000 ug/l. C5-C8 Aliphatics were noted at a concentration of 26,000 ug/l in MW-3 above their 2L Standard of 400 ug/l. No compounds were detected at levels above GCLs. 6.2 Recommendations Based upon a review of all information gathered during this project, Henry Nemargut Engineering Services makes the following recommendations: o A site ranking should be assigned by the NCDEQ to determine the need for any further action at this site. o A copy of this report has been forwarded to the following address: Ms. Mindy Lepard NCDEQ – UST Section 450 West Hanes Mill Rd, Suite 300 Winston Salem, NC 27105-7407 Page - 10 6.3 Limitations This report has been prepared for the exclusive use of F. Fanous Group Inc. for the specific application to the referenced site located in Alamance County, North Carolina. The evaluation was conducted based on the scope of work and level of effort desired by the client and with resources adequate only for the scope of work. Our findings have been developed in accordance with generally accepted standards for Limited Site Assessments in the State of North Carolina, available information and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions at the precise locations sampled and the time the sample was collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters in the laboratory reports. No additional information can be logically inferred from this data.
